Book excerpt: A public health event, such as a widespread disease outbreak or health problems resulting from a weather-related emergency, could have catastrophic consequences for the nation. These potential threats can be partially mitigated by having a national public health situational awareness capability-that is, a capability for public health officials to be able to access real-time information about emerging threats to enable them to make timely, responsive decisions to prepare for and respond to emergencies. PAHPRA required HHS to establish a near real-time electronic nationwide public health situational awareness capability through an interoperable network of systems. PAHPRA also included a provision for GAO to evaluate HHS's progress in developing such a capability. This report addresses what progress HHS has made toward establishing the network. GAO analyzed documents describing HHS's plan for enhancing public health situational awareness and evaluated evidence of actions taken by HHS to establish the network required by PAHPRA. GAO also examined the department's IT planning and management processes and guidance, and interviewed HHS officials.

Book excerpt: In April 2004, President Bush called for the Department of Health and Human Services (HHS) to develop and implement a strategic plan to guide the nationwide implementation of health information technology (IT). The plan is to recommend methods to ensure the privacy of electronic health information. GAO was asked to summarize its January 2007 report. The report describes the steps HHS is taking to ensure privacy protection as part of its national health IT strategy and identifies challenges associated with protecting electronic health information exchanged within a nationwide health information network. GAO recommended that HHS define and implement an overall privacy approach that identifies milestones for integrating the outcomes of its initiatives, ensures that key privacy principles are fully addressed, and addresses challenges associated with the nationwide exchange of health information. In its initial comments, HHS disagreed with this recommendation and stated that it had established a comprehensive privacy approach. In recent discussions with GAO, the National Coordinator for Health IT agreed with the need for an overall approach to protect health information and stated that the department was initiating steps to address the recommendation.

For the health community, a primary concern is how to provide care to individuals during such high demand, when the health system's resources are exhausted and there are more patients than the system can accommodate. The IOM's Forum on Medical and Public Health Preparedness for Catastrophic Events held a workshop June 10-11, 2009, to assess the capability of and tools available to federal, state, and local governments to respond to a medical surge. In addition, participants discussed strategies for the public and private sectors to improve preparedness for such a surge. The workshop brought together leaders in the medical and public health preparedness fields, including policy makers from federal agencies and state and local public health departments; providers from the health care community; and health care and hospital administrators. This document summarizes the workshop.

Although life expectancy and survival rates in the United States have improved dramatically over the past century, Americans live shorter lives and experience more injuries and illnesses than people in other high-income countries. The U.S. health disadvantage cannot be attributed solely to the adverse health status of racial or ethnic minorities or poor people: even highly advantaged Americans are in worse health than their counterparts in other, "peer" countries. In light of the new and growing evidence about the U.S. health disadvantage, the National Institutes of Health asked the National Research Council (NRC) and the Institute of Medicine (IOM) to convene a panel of experts to study the issue. The Panel on Understanding Cross-National Health Differences Among High-Income Countries examined whether the U.S. health disadvantage exists across the life span, considered potential explanations, and assessed the larger implications of the findings. U.S. Health in International Perspective presents detailed evidence on the issue, explores the possible explanations for the shorter and less healthy lives of Americans than those of people in comparable countries, and recommends actions by both government and nongovernment agencies and organizations to address the U.S. health disadvantage.

Among other things, PAHPA directs the Secretary of the Department of Health and Human Services (HHS) to develop a National Health Security Strategy (NHSS), to be initially presented to Congress in 2009 and subsequently revised every four years afterward. National Health Security Strategy Identifies Goals and Strategic Objectives for the Nation As noted in the National Health Security Strategy document, national health security is achieved when the Nation and its people are prepared for, protected from, ready to respond to, and able to recover from incidents with potentially catastrophic health consequences. The NHSS is designed to achieve two goals: 1. Build community resilience; 2. Strengthen and sustain health and emergency response systems. These goals are supported by ten strategic objectives, which address areas that require urgent and focused attention and improvement. The strategic objectives describe what must be accomplished to address current gaps in national health security over the next four years and to maintain improvements in health security over the longer term. The ten strategic objectives are: 1. Foster informed, empowered individuals and communities 2. Develop and maintain the workforce needed for national health security 3. Ensure situational awareness 4. Foster integrated, scalable health care delivery systems 5. Ensure timely and effective communications 6. Promote an effective countermeasures enterprise 7. Ensure prevention or mitigation of environmental and other emerging threats to health 8. Incorporate post-incident health recovery into planning and response 9. Work with cross-border and global partners to enhance national, continental, and global health security 10. Ensure that all systems that support national health security are based upon the best available science, evaluation, and quality improvement methods.
