Book excerpt: The Department for Transport has approved expenditure of over £11 billion between 1998 and 2021 for the development of new and existing trunk roads and motorways by the Highways Agency, and just under £1.7 billion on major road schemes proposed and developed by local authorities in five year Local Transport Plans. Following on from a NAO report on this topic (HCP 321, session 2006-07; ISBN 9780102944600) published in March 2007, the Committee's report examines the steps taken by the Department for Transport and the Highways Agency to improve value for money and oversight of the roads programme and contracting methods and project management capability. By September 2006, the Agency's 36 completed schemes in the Targeted Programme of Improvement cost 40 per cent more than estimated initially, and for schemes still to be completed, latest forecasts indicate that final costs could be 27 per cent more than original estimates. The main causes for costs exceeding estimates are increases in construction costs, higher than forecast land prices and compensation to landowners, inflation and changes in the scope of the project. The report finds that the DfT has not been rigorous enough in its oversight of the Agency's delivery of major road schemes, allowing it too much latitude on delivery and cost plans, and has failed to monitor in-year expenditure against progress and delivery milestones. The Agency is overly reliant on consultants for project management expertise and needs to develop its in-house capability.

Book excerpt: In October 2003, the Department of Health (the Department) agreed a new national contract for NHS medical consultants in England. This report examines the contract negotiation; the cost implications; the effectiveness of the implementation process; and the extent to which the expected benefits for patients and the NHS had been realised. It was intended that employers would get greater control and management of their consultants' workload, and patients would benefit from a more flexible and responsive service. The Department hoped to reward consultants who made the biggest contribution to NHS work and reduce the average number of hours worked, in exchange for increased productivity. These benefits were dependent on a mandatory and rigorous process of workload planning for individual consultants (job planning). The implementation of the contract was rushed and the NHS has yet to see many of the intended benefits. Over the first three years, the Department allocated an additional £715 million to NHS trusts which was £150 million more than originally estimated. Although consultants' pay has, on average, increased by 27 per cent (from £86,746 to £109,974) and their working hours have decreased, there are no measurable improvements in productivity. The Department has succeeded in increasing the number of consultants working in the NHS, from 28,750 in October 2003 to 31,990 by September 2005, but the number of hours consultants work in private practice has neither increased nor significantly decreased. Other intended benefits have not been realised: for example the proportion of time consultants spend on direct clinical care is less than intended, and the contract has not been used to extend and develop new services for patients.

A report by the Chief Medical Officer in 2000, ('An organisation with a memory', ISBN 0113224419) estimated that one in ten patients admitted to hospital were unintentionally harmed and that a blame culture and lack of a national system for sharing experience were key barriers to reducing the number of patient safety incidents. In Government's response included plans, timetables and targets to promote patient safety and the establishment of the National Patient Safety Agency. This report finds that insufficient progress has been made. In particular there is a question mark over the National Patient Safety Agency because of cost over-runs and delays in its National Reporting and Learning System and the limited feedback it has so far provided to trusts.

Book excerpt: In Britain, between 1980 and 1998, the number of people classified as obese tripled to 21per cent of women and 17 per cent of men. It is estimated that this costs the economy, as a whole, �2 billion and the NHS �0.5 billion in treatment. However the response of the NHS is patchy, with no national guidelines and only 28% of health authorities taking action to address the problem. There is little activity related to the management of obesity outside of general practice but only a small proportion of GPs follow a protocol. This report recommends that there should be strategies to reduce obesity and that the Department of Health should build on the plan in the National Service Framework on coronary heart disease and work with partners and professional bodies to clarify responsibilities. It should also work with the National Institute of Clinical excellence to disseminate information. The Department of Health should also lead a cross government strategy to promote the benefits of physical activity and there should be local targets to encourage cycling and walking. It should also work with the food industry to improve the balance of diet. Much of this work needs to be aimed at school children to promote a healthy lifestyle throughout life and guidance to schools on commercial sponsorship should be strengthened so that there is no conflict with messages on healthy eating.

However their high use can have a detrimental impact on patient care and satisfaction. In 2001 the Department of Health anticipated that a growth in the NHS workforce would lead to a decline in the use of temporary staff but in spite of this intention temporary nurses still account for the same percentage of the nursing budget (around 9%). On the basis of a report by the Comptroller and Auditor General, the Committee has investigated the extent of the use of temporary nurses; whether the process is properly planned and managed; and whether there are safety and quality implications. One of the conclusions is that there has a lack of planning and it is only as a result of the problem with deficits that the NHS has taken a more co-ordinated approach
