Book excerpt: The author attempts to educate the reader about refugees who have survived torture and about the newly evolving treatment methods specific to helping them. The dissertation begins with an examination of torture: its definitions, goals, physical and psychological methods, as well as codes and laws that prohibit torture. Nevertheless, torture is proliferating, with the compliance of some members of the medical and psychiatric professions. The author documents this prevalence and compliance and outlines the recent history of methods to rehabilitate torture survivors. In the United States, torture survivors are primarily refugees. The author provides background to understanding refugee experiences in terms of their mental health ramifications. She summarizes the social displacement syndrome, psychosocial stages of refugee adjustment, and common forms of psychopathology among refugees. Analyzing the psychological sequelae of torture in terms of the Concentration Camp Syndrome, the Torture Syndrome, Post-Traumatic Stress Disorder, the concept of the survivor, and specific coping behaviors, the author also considers the physical, familial, existential, and prognostic impact of torture and forced exile. Inspired largely by the work of Amnesty International, major therapeutic approaches to torture treatment are currently being developed. The author examines cognitive restructuring as used at Denmark's Rehabilitation Center for Torture; testimony as used in Chile's Fundacion de Ayuda Social de las Iglesias Christians; biological interventions as used at the Indo-Chinese Psychiatry Clinic in Boston; the eclectic model espoused by the Center for Victims of Torture in Minneapolis; and the community mental health approach used by clinicians at the Marjorie Kovler Center for the Treatment of Survivors of Torture. The author integrates these issues with the presentation of a clinical case study of a Namibian asylum seeker tortured for his political activities, analyzing the psychotherapeutic approach in terms of impact of repression, sociopolitical conditions, the torture trauma syndrome, refugee mental health issues, personality, and countertransference. Finally, the author synthesizes research and clinical experience into basic principles for the treatment of torture survivors. Elements of the model include building trust, validation of torture experiences, therapist flexibility, eschewing therapist neutrality, attention to cultural and political context, reassignment of blame, deprivatization of pain, resocialization, existential focus, empowerment, multidisciplinary treatment, and attention to the unique countertransference difficulties inherent in working with survivors of torture.

Book excerpt: Torture, Psychoanalysis and Human Rights contributes to the development of that field of study called ‘psycho-social’ that is presently more and more committed to providing understanding of social phenomena, making use of the explicative perspective of psychoanalysis. The book seeks to develop a concise and integrated framework of understanding of torture as a socio-political phenomenon based on psychoanalytic thinking, through which different dimensions of the subject of study become more comprehensible. Monica Luci argues that torture performs a covert emotional function in society. In order to identify what this function might be, a profile of ‘torturous societies’ and the main psychological dynamics of social actors involved – torturers, victims, and bystanders – are drawn from literature. Accordingly, a wide-ranging description of the phenomenology of torture is provided, detecting an inclusive and recurring pattern of key elements. Relying on psychoanalytic concepts derived from different theoretical traditions, including British object relations theories, American relational psychoanalysis and analytical psychology, the study provides an advanced line of conceptual research, shaping a model, whose aim is tograsp the deep meaning of key intrapsychic, interpersonal and group dynamics involved in torture. Once a sufficiently coherent understanding has been reached, Luci proposes using it as a groundwork tool in the human rights field to re-think the best strategies of prevention and recovery from post-torture psychological and social suffering. The book initiates a dialogue between psychoanalysis and human rights, showing that the proposed psychoanalytic understanding is a viable conceptualisation for expanding thinking of crucial issues regarding torture, which might be relevant to human rights and legal doctrine, such as the responsibility of perpetrators, the reparation of victims and the question of ‘truth’. Torture, Psychoanalysis and Human Rights is the first book to build a psychoanalytic theory of torture from which psychological, social and legal reflections, as well as practical aspects of treatment, can be mutually derived and understood.
