Il en conclut, à la suite des travaux de Pierre Marty, Michel Fain et Léon Kreisler, qu’il n’y a pas d’uniformité structurale de l’enfant asthmatique mais — et c’est l’objet de ce livre de les mettre en évidence — des modalités relationnelles et identificatoires assez fréquentes pour être considérées comme caractéristiques. Cet ouvrage montre ainsi l’apport de la psychanalyse à la connaissance d’un déséquilibre psychosomatique chez l’enfant.

Book excerpt: L’objet de cette recherche concerne l’étude des facteurs psychologiques qui accompagnent l’émergence et l’évolution de l’asthme chez le bébé et le jeune enfant. Dans la perspective psychosomatique développée par P.Marty, cette recherche se propose d’étudier dans une population de 20 nourrissons asthmatiques âgés de 5 â 24 mois, l’impact désorganisateur des événements de vie survenus autour de la naissance de l’enfant, la qualité de la mentalisation maternelle et des modalités interactives ainsi que le développement psychique des nourrissons asthmatiques. Le dispositif de recherche s’appuie sur l’étude des représentations maternelles (Entretien « R ») complété par l’étude des qualités formelles du discours (Edicode), l’évaluation des troubles psycho-fonctionnels (Symptom Check-List), l’étude du développement psychomoteur de l’enfant (Brunet Lézine-Révisé) et des troubles de la première enfance (Classification Diagnostique 0-3 ans). Les résultats de cette recherche soulignent que les mécanismes mentaux et interactifs qui accompagnent l’asthme du nourrisson sont hétérogènes et ne peuvent se limiter au seul « modèle allergique ». L’asthme survient dans un contexte de « situation de contrainte » qui pèse sur la dyade mère-bébé en raison de la conjonction des événements de vie, de la réactivation d’une problématique maternelle inconsciente et de la faillite des tiers à contenir l’angoisse maternelle. Face à ce débordement de la fonction pare-excitations maternelle, le bébé asthmatique développe, très précocement, des comportements hyper-adaptatifs lui permettant d’auto-réguler ses états d’excitations internes. Dans un certain nombre de cas on observe une difficulté du nourrisson asthmatique à « matérialiser » sur le plan corporel ou comportemental les conflits intra-psychiques maternels. Ce sont ces nourrissons chez qui les mécanismes adaptatifs tiennent une place excessive et entravent le bon déroulement de l’émergence progressive du processus de psychisation dont témoigne le gauchissement des auto-érotismes.
