A ‘substance’ is a chemical compound that alters behaviour or mood when ingested. This group includes alcohol, nicotine, caffeine, heroin and cocaine along with chocolate and soft drinks. The vast majority of users of the ‘safe’ drugs are not perceived as addicts, they can be equally as addictive and harmful to one’s health. There is a clear distinction between a user and an abuser of drugs. ‘Substance use’ suggests that the individual ingests the drug in moderate amounts that have no significant effect on their social, educational or occupational functionality. The drug has the ability, based on the quantity ingested to create impaired judgement, mood changes and lowered motor ability; this result is referred to as intoxication. ‘Substance abuse’ as an extension of ‘use’ is really defined on how the use of the drug affects the individual’s relationships, his work or education and whether it causes him to place himself or others in physically dangerous situations (Barlow & Durand: 2005). ‘Drug dependence’ is often described as addiction. There is however some disagreement in how we best define substance dependence (Woody & Cacciola: 1997). One definition would explain how the individual requires systematically greater quantities of the drug to experience the same level of intoxication (tolerance), and will behave negatively if the drug is not ingested (withdrawal) (Franklin & Frances: 1999). An alternate view on substance dependence pertains to the behaviour that focuses on finding the required drug as a dependence indicator. A different view of substance dependence focuses on the drug seeking behaviour itself as an indication of dependence. The repetitive ingestion of the drug, an increased need for more of the drug and the likelihood that re-use of the drug will occur after a period of abstinence; are all behaviours that help to define the extent of the drug dependence. These behaviours have a psychological component and these in conjunction with physiological components of tolerance and withdrawal make for a better understanding of substance dependence. Book excerpt: Males and females exhibit discrete attitudes and skills, experience dissimilar emotional and psychological needs, and react differently to peer pressure, lack of self-realization, or other personal and social expectations. In addition, they are differently influenced by family history, and diverge in the perception of self-image and health risks. To complicate the matter on gender dichotomy, male testosterone levels markedly vary over the course of the day, while female levels of sex hormones significantly fluctuate depending upon the menstrual cycle, the pre- or post-menopausal age, and the use of oral contraceptives. All of these factors interact with genetic background and sex hormonal fluctuations, and determine the differences observed in their predisposition to develop an addiction. This term is traditionally associated to the abuse of legal and illegal substances. However, a compulsion toward the engagement in a non-drug-related rewarding behavior, usually involving a natural reward, also activates the brain reward system and engenders persistent behavior, thus resulting in a diminished control over it. These latter behaviors are defined as “behavioral addictions”. This definition encompasses any behavior characterized by the followings: i) feeling of tension or arousal before the action; ii) gratification and/or relief at the time of performing the act; iii) inability to resist an urge or drive even against great obstacles or dangers; iv) absence of consideration for the negative consequences that may affect family, friends, and/or work. As such, behavioral addictions include compulsive food intake and sexual activity, pathological gambling and Internet addiction, excessive exercising, compulsive buying and pyromania. These behaviors, which are often classified as "impulse control disorders", result in actions that are harmful to oneself and/or others, share common features (e.g. compulsiveness, impulsivity, impaired decision-making, craving, tolerance, withdrawal, high rates of relapse), and involve dysfunction of several brain circuits. Derangement from functional neurobiological mechanisms underpinning both sensitivity to reward and inhibitory control can also lead to compulsive behaviors. For instance, pathological gambling and other impulse control disorders (e.g., hypersexuality, compulsive painting, eating and buying) are often reported in Parkinson's disease patients. Gender-dependent differences in the rate of initiation and frequency of misuse of addicting drugs have been widely described. Yet, men and women also differ in their propensity to become addicted to other rewarding stimuli (e.g. sex, food) or activities (e.g. gambling, exercising). The goal of the present Research Topic is to explore and summarize current evidence for gender (and sex) differences not only in drug addiction, but also in other forms of addictive behaviors. Thus, it will include studies showing gender-dependent differences in drug addiction, food addiction, compulsive sexual activity, pathological gambling, Internet addiction and physical exercise addiction. Psychiatric comorbidity, potential risk factors and the underlying neural mechanisms will be also examined, with particular emphasis to the role of sex hormones in modulating addictive and compulsive behaviors.

In our western civilization, the bulk of attention has focused on those indi viduals who specifically seek treatment or those who have become so disabled by these problems that they require treatment. These indi viduals usually qualify for a psychiatric diagnosis of alcohol or other substance abuse. However, just as it has been recognized that primary substance abuse is frequently associated with other diagnosable psychi atric disorders, such as sociopathy or attention deficit disorder (residual type) and that the origins of substance abuse are multivariate, we have also begun to become aware that many other individuals in our society with psychiatric or other problems also suffer, to varying degrees, from substance abuse. These problems may be considered secondary by vari ous specialists or treatment personnel; but nevertheless, they are prob lems, and what disorder is primary or secondary in a given individual may often be very difficult to determine in a meaningful fashion. Thus, within the past decade, research studies have reported significant inci dences of substance abuse/or misuse in high school and college-aged populations, in medical populations, and in individuals with other psy chiatric disorders such as schizophrenia, depression, and the anxiety and personality disorders. Yet to date little has been done to bring together and systematize this widely scattered data that describes the presence of substance abuse problems in various populations.
