Book excerpt: Adults who stutter (AWS) are vulnerable to the development of various psychopathological symptoms, although prevalence data are mixed, and even less clarity exists as it relates to the factors that potentially influence their occurrence. The current study sought to shed light on the prevalence of self-reported psychopathology in AWS and aimed to identify relationships between affective, behavioral and cognitive (ABC) experiences of stuttering and psychological distress. This was based on AWS’ reports of speech situation-specific anxiety and speech disruption, the use of behaviors to avoid or escape stuttering, and one’s communication attitude. The self-reports administered to 40 AWS via Qualtrics, an online survey software, were subtests of the Behavior Assessment Battery for Adults who Stutter (BAB; Vanryckeghem & Brutten, 2018) and the Brief Symptom Inventory-18 (Deragotis, 2001). Participants’ BAB scores approximated the normative data suggesting that the current sample corresponded to the psychosocial profile of AWS. Participants’ indications of psychological distress were not found to differ from that of a non-clinical normative sample. Ten percent of participants met clinical thresholds for what is considered clinically significant distress. Overall, the current data provide evidence that, as a group, levels of psychopathology in this population approximate community samples, highlighting the existence of psychologically distressed subgroups of AWS. Regression analyses uncovered that the ABC factors of stuttering positively predicted participants’ reports of levels of psychopathology. Among these factors, speech situation-specific anxiety had the strongest relationship to psychological distress, followed closely by one’s report of situation-specific speech disruption. To a lesser, but still significant extent, a PWS’ communication attitude predicted psychopathology. Finally, a weaker and less clear positive relationship between PWS’ use of coping behaviors to avoid and/or escape stuttering and their levels of psychological distress was found.

Available in PDF, EPUB and Kindle. Book excerpt: Social anxiety disorder (SAD) is a debilitating condition, and it is estimated that approximately half of adults who stutter have SAD. Thus, there is a need for the assessment and treatment of SAD in this population. Cognitive-behavioral therapy (CBT) has shown promise in decreasing anxiety symptoms among adults who stutter and have SAD, but exposure, the key ingredient for successful CBT for SAD, has been understudied and underemphasized. The aims of this study were to develop an exposure therapy protocol specifically for people who stutter and have SAD and to evaluate its efficacy for reducing anxiety and stuttering severity. Utilizing a multiple baseline design, six participants were randomized to receive zero, two, or four sessions of progressive muscle relaxation therapy. This served to establish the staggered start and to account for the common factors of therapy. All participants received ten sessions of exposure therapy. Participants recorded daily social anxiety levels, and anxiety and stuttering severity were assessed at major assessment points. All participants demonstrated substantial reductions in social anxiety and substantial improvements in the affective, behavioral, and cognitive experiences of stuttering following exposure therapy. No reliable change was observed for stuttering frequency. Results suggest that exposure therapy may be useful for people who stutter and have SAD, but will not necessarily influence their speech fluency. These findings underscore the importance of the assessment and treatment of SAD among adults who stutter and suggest that the integration of care between psychologists and speech-language pathologists may prove beneficial for this population.

Book excerpt: Introduction: Stuttering impacts almost every aspect of life, stimulating fear and speaking avoidances, anxiety, frustration, anger, guilt, and/or shame in those affected. Because stuttering triggers anxiety and other psychological and emotional reactions and limits participation in society, it follows that psychological counseling could enhance stuttering treatment outcomes. The Comprehensive Stuttering Program (CSP; Boberg & Kully, 1985; Kully, Langevin, & Lomheim, 2007) offered at the Institute for Stuttering Treatment and Research (ISTAR) uses speech restructuring and stuttering modification methods to reduce stuttering, and cognitive behavioral techniques to address the psychoemotional and social consequences of stuttering. However, in addition to the CSP, clients are provided the option to participate in psychological counseling offered by a resident Registered Psychologist. Despite the potential for counseling to enhance stuttering treatment outcomes in the CSP and other stuttering treatment programs, no research has been published to date on clients' experiences with, or views toward, adjunct psychological counseling services. Research Question: The purpose of this study was to gain an understanding of clients' experiences with, and perceptions of, psychological counseling as an adjunct to the speech therapy for stuttering. Method: Participants were adults or young adults between the ages of 13 to 38 years of age who stuttered. Six of the participants took part in psychological counseling at ISTAR, whereas three chose not to do so. Semi-structured interviews were used to gather data. Following data collection, interviews were transcribed and data was analyzed using grounded theory as a guiding framework. iii Results: Three central themes emerged from the data. The first was 'Participation decisions as determined within the context of clients' life stories, personal experiences, perceptions of counseling, and the way that the counseling service was promoted at ISTAR.' Decisions to participate (or not) involved factors that have been associated with help-seeking in related fields (e.g., perception of benefits, presence of negative life events) as well as novel factors (e.g., 'why not' mentality, seeing the service as a 'limited resource'). The second theme was 'Counseling as a necessary optional component in stuttering treatment,' as illustrated by the experiences and opinions of those who participated in counseling, and the surmised benefits of the service according to those who did not participate. The final theme was 'Counseling as a great service that begs wider promotion and enhancement.' Participants made suggestions for enhancing the counseling service at ISTAR, but grappled with, and discussed, barriers to modifying the service that are relevant to ISTAR and to other centers providing stuttering treatment. Inherent in the three core themes was the belief that clients at ISTAR would, or could, benefit from psychological counseling with a mental health professional who is specifically trained to deal with emotional and psychological issues; however, participants indicated that counseling needs to remain an optional component despite being an integral part of a treatment program.

His introduction to stuttering corrective procedures first came at the age of fifteen under the direction of Frederick Martin, M.D., who at that time was Superintendent of Speech Correction for the New York City schools. A few years later, he worked with J. Stanley Smith, L.L.D., a stutterer and philanthropist, who, for altruistic reasons, founded the Kingsley Clubs in Philadelphia and New York that were named after the English author, Charles Kingsley, who also stuttered. The Kingsley Clubs were small groups of adult stutterers who met one night a week to try out treatment ideas then in effect. In fact, they were actually practicing group therapy as they talked about their experiences and exchanged ideas. This exchange gave each of the members a better understanding of the problem. The founder often led the discussions at both clubs. In 1928 Malcolm Fraser joined his older brother Carlyle who founded the NAPA-Genuine Parts Company that year in Atlanta, Georgia. He became an important leader in the company and was particularly outstanding in training others for leadership roles. In 1947, with a successful career under way, he founded the Stuttering Foundation of America. In subsequent years, he added generously to the endowment so that at the present time, endowment income covers over fifty percent of the operating budget. In 1984, Malcolm Fraser received the fourth annual National Council on Communicative Disorders' Distinguished Service Award. The NCCD, a council of 32 national organizations, recognized the Foundation's efforts in "adding to stutterers', parents', clinicians', and the public's awareness and ability to deal constructively with stuttering."
