Elles sont dans la majorité des cas d'origine génétique : elles peuvent être isolées ou bien faire partie d'un syndrome polymalformatif comme le plus souvent celui de la dysplasie ectodermique. Un diagnostic clinique complet devra être réalisé, dès le plus jeune âge du patient, afin de mettre en évidence les différentes conséquences physiologiques, esthétiques et psychologiques. Il permettra d'établir un plan de traitement adéquat et évolutif durant la croissance. Les décisions thérapeutiques nécessitent une collaboration multidisciplinaire. Des centres de références et compétences des maladies rares ont été mis en place afin d'apporter une aide au diagnostic et à l'orientation de ces jeunes patients. La prise en charge est souvent longue et complexe. Les différentes thérapeutiques conservatrices, orthodontiques, prothétiques et implantaires nécessitent une mise en confiance de l'enfant et une implication de son entourage.

Le texte précise les conditions nécessaires pour en bénéficier. Ces critères sont si restrictifs que la majorité des patients atteints d'agénésies dentaires multiples se retrouvent seuls à assumer le coût de leur thérapeutique. Or le traitement des agénésies dentaires représente un coût financier majeur que seules peu de personnes peuvent assumer. Au travers des 22 cas cliniques décrits, on peut constater qu'un seul et unique enfant a été admis dans le protocole établi par le décret. Il est par conséquent nécessaire que le législateur revoie les critères d'inclusion dans le protocole de prise en charge des agénésies dentaires multiples.

Dans la pratique de notre exercice, il est important de savoir repérer les cas d'agénésies multiples le plus tôt possible, afin que l'orientation et la prise en charge multidisciplinaire se fasse également très tôt. Après quelques rappels et généralités concernant les différents stades de la denture, la première partie aborde la fréquence, les étiologies et l'épidémiologie des agénésies dentaires multiples.Ensuite seront traités le diagnostic, les conséquences et la prise en charge des agénésies dentaires multiples. Dans cette même partie est abordé l'aspect psychologique de l'oligodontie, ainsi que la relation de confiance qui doit être établie entre le chirurgien-dentiste, l'enfant et ses parents. Une fois cette relation instaurée se fait le choix du traitement prothétique, en fonction de l'âge de l'enfant, de la situation dentaire, du développement osseux et des données socio-économiques du patient. La troisième partie est consacrée à des cas cliniques, permettant ainsi de visualiser différents plans de traitements proposés en fonction des critères donnés précédemment.

Le recours à l'anesthésie générale est encore aujourd'hui fréquent et nécessite d'évaluer le rapport bénéfice/risque : la durée d'intervention est limitée, une toxicité cérébrale est constatée au bout d'une certaine durée. De plus, le représentant légal de l'enfant doit être informé de façon précise sur le déroulement et les conséquences de l'intervention. Les données actuelles de la littérature prennent en compte le profil postopératoire du patient et non le profil initial. En effet, nous pouvons constater à travers les différentes études que les patients traités sous anesthésie générale n'ont pas eu d'examen clinique préopératoire détaillé afin de comparer la situation avant et après l'intervention. Le but de ce protocole d'étude est d'évaluer les répercussions (croissance staturo-pondérale, fonctions physiologiques, psychologie) des soins et extractions dentaires sous anesthésie générale chez l'enfant en tenant compte de la situation initiale du patient. Autrement dit, le protocole d'étude va permettre d'évaluer l'impact de l'anesthésie générale sur l'enfant : des outils objectifs seront utilisés pour déterminer les répercussions.

The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.
