Book excerpt: Ce travail présente les résultats d'une série continue de 42 prothèses totales de cheville ayant eu un suivi clinique et radiologique prospectif. Le modèle prothétique utilisé était un implant non cimenté à composants avec une pièce intermédiaire en polyéthylène et un re surfaçage malléolaire externe optionnel. L'indication principale était l'arthrose post-traumatique invalidante (27 cas sur 42). L'âge moyen des patients était de 56 ans. Au recul moyen de 3 ans et demi, nous déplorons trois conversions par arthrodèse tibio-talienne pour échec douloureux ou descellement. Le taux de révision des prothèses en place est de 94 % et de 93% des patients se déclaraient satisfaits ou très satisfaits du résultat de l'intervention. L'évaluation des résultats cliniques permet de faire le point sur ce que l'on est en droit d'attendre en terme d'indolence, de mobilité et de fonction. 74 % des patients avaient un score clinique à la révision jugé comme bon ou excellent. Les complications per-opératoires à type de fractures malléolaires ont régressé par rapport aux séries antérieures. Nous n'avons pas noté de dégradation de la mobilité des articulations sous-taliennes ou du médio-pied. L'analyse radiologique permet de faire le point sur la qualité du positionnement des implants et sur l'évolution dans le temps de l'implant et de sa fixation osseuse. Nous avons aussi étudié le gain réel en mobilité de l'articulation tibio-talienne. La pose d'une prothèse de la cheville sur les chevilles enraidies permet d'obtenir une amplitude articulaire tibio-talienne de 23 ʻ. La fréquence des liserés radiologiques retrouvés est faible mais nous avons observé l'apparition de géodes radiologiques tibiales dans 29 % des cas, géodes qui ne semblent pas influencer le résultat clinique de la prothèse. La revue de la littérature permet d'actualiser les notions relatives à la prothèse de cheville en terme : - d'indication ; - de contre-indication ; - de complications ; - de résultats à moyen terme.

Book excerpt: Les prothèses totales de cheville (PTC) associées à une arthrodèse de l'arrière-pied ainsi que les reprises de PTC sont peu fréquentes. L'objectif de cette étude est de comparer les résultats de PTC avec implant talien de révision Salto XT avec ceux de PTC Salto Talaris standard. Entre septembre 2014 et décembre 2018, 126 PTC ont été incluses et réparties en 3 groupes. Les groupes étaient comparables sur l'âge, l'IMC et les comorbidités. Les étiologies étaient dominées par les pathologies inflammatoires et les reprises de PTC dans les groupes XT (1A et 1B) et par l'arthrose post-fracture et sur laxité chronique dans le groupe 2-talaris. Il y a eu 94% de gestes associés dans le groupe 1A, 67 % dans le groupe 1B et 74% dans le groupe 2. Le suivi fonctionnel, clinique et radiologique a été fait à 1 an avec un scanner puis au dernier recul. 25 PTC XT ont été analysées à 25,2 mois ± 12,1 (11-58) dans le groupe 1 et 101 PTC Talaris à 27,4 mois ± 13,8 (11-60) dans le groupe 2. Le taux de complication précoce à 2 ans était respectivement de 25% , 11% et 13% dans chaque groupe (p=0.38), dominé par des retards de cicatrisation. Le taux de survie sans explantation était de 96% pour le groupe 1, 98,6% pour le groupe 2 (p=0.26). Il était de 96% dans le groupe 1 et 90,9% dans le groupe 2 pour la survie globale sans reprises (p=0.93). Le score AOFAS au dernier recul était de 71,5 ± 22,3 (13-89) dans le groupe 1A, 71,5 ± 32,4 (13-100) dans le groupe 1B et 85 ± 10,8 (44-100) dans le groupe 2 (p=0.032). Il n'était pas significativement différent pour le score AOFAS douleur, respectivement 29,4, 28,8 et 31,4 (p=0.49). Les amplitudes articulaires en flexion plantaire étaient meilleures dans le groupe 2-talaris, et comparables en flexion dorsale. Les résultats à court terme au niveau de la douleur et de la survie des implants XT sont comparables à ceux des implants standards et constituent une solution prothétique en cas d'arthrodèse sous-talienne associée ou de changement prothétique.

Les deux séries ont été revues avec un recul moyen de 23 mois L'analyse fonctionnelle ne montrait pas de différence entre les deux séries. Aucune migration de la pièce tibiale n'a été objectivée. Des géodes et des liserés tibiaux étaient observées dans respectivement 16,2% et 32,5% des cas. La fréquence des images d'ostéolyse n'était pas statistiquement différente entre les deux séries. En conclusion, la suppression du canon de la quille tibiale n'a pas réduit statistiquement l'incidence des phénomènes d'ostéolyse observés. Le mécanisme de l'ostéolyse périprothétique est vraisemblablement multifactoriel, associant des phénomènes biologiques et mécaniques.
