Dans ce cadre sont réalisées des ostéotomies de correction, en particulier des ostéotomies tibiales de valgisation (OTV) en cas d'arthrose fémoro-tibiale interne sur genu varum. Cette ostéotomie résout le défaut d'axe, décharge le compartiment dégradé, mais ne stoppe pas le processus dégénératif touchant cet interligne; si la symptomatologie réapparaît avec arthrose évolutive, une arthroplastie totale de genou est une solution thérapeutique envisageable. Quarante quatre patients (47 genoux) ont bénéficié d'une ostéotomie tibiale de valgisation suivie d'une prothèse totale de genou (PTG) dans un délai moyen de 7,8 ans au Centre Hospitalier d'Amiens. Il s'agit d'une ostéotomie de fermeture dans 43 cas, à un âge moyen de 57 ans. Le recul moyen des prothèses totales de genou est de 3,9 ans (44 PTG sans conservation du pivot central). Les résultats fonctionnels ont été évalués au moyen du score de l'International Knee Society ; les prothèses après ostéotomies de notre échantillon procurent des résultats fonctionnels faibles (IKS f moyen 57/100). Le score IKS global reste stable dans le temps. Les échecs des PTG regroupent 4 descellements aseptiques et 2 septiques sans corrélation avec l'ostéotomie préalable. L'OTV est d'indication légitime chez le sujet jeune et actif, en précisant que les résultats apportés par une PTG après OTV resteront modestes sur la fonction

Book excerpt: Les résultats des prothèses totales de genou après ostéotomies tibiales de valgisation sont discordants, parfois similaires aux prothèses de première intention et parfois moins bons. Les séries publiées sont hétérogènes, et peu d’entre elles s’intéressent aux ostéotomies par ouverture interne.Notre objectif est d’évaluer les résultats des prothèses totales de genou après ostéotomie tibiale, ainsi que les facteurs pronostiques afin d’améliorer la prise en charge de la gonarthrose chez les patients jeunes et actifs.Nous avons suivi rétrospectivement 52 arthroplasties réalisées entre 1994 et 2004 sur des genoux anciennement ostéotomisés sur une durée moyenne de 9 ans. Le délai moyen entre les deux interventions était de 14 ans. Au cours de l’arthroplastie, il a été nécessaire de réaliser un ou plusieurs gestes complémentaires (relèvement de tubérosité tibiale, greffe, ostéotomie associée, release ligamentaire) dans 57% dans cas. Le score IKSS clinique moyen au dernier recul était de 87,2 et de 73,2 pour la fonction, et la mobilité globale moyenne du genou était de 103°. L’ostéotomie par fermeture externe semble entrainer plus de difficultés techniques, et une fonction moins bonne que les ouvertures internes. L’alignement post opératoire moyen était de 181°, nous n’avons pas rencontré de problème d’interligne oblique ni de laxité ligamentaire. Le taux de complications était de 23%, une révision n’a été nécessaire que dans 3 cas.L’ostéotomie tibiale de valgisation est donc une intervention fiable, qui ne semble pas altérer le pronostique ultérieur de la prothèse total de genou à long terme.

Book excerpt: Introduction : L'objectif de ce travail était de comparer les résultats à moyen terme des prothèses totales de genou (PTG) post-ostéotomie tibiale de valgisation d'ouverture médiale (post-OTOM) à ceux des PTG de première intention (PTG1). L'hypothèse était que les PTG post-OTOM avaient des résultats identiques aux PTG1. Méthodes : La série 1 était composée de 45 PTG post-OTOM, 30 hommes et 10 femmes, âgés en moyenne de 69 ± 7 ans (54-82). Celle-ci a été comparée à une série 2 composée de 45 PTG1, 30 hommes et 10 femmes, âgés en moyenne de 69 ± 7 ans (55-78). Le score IKS moyen était respectivement de 91 ± 22,5 points (42-129) et de 86 ± 18 points (38-116). L'angle HKA moyen préopératoire était respectivement de 179° ± 5° (169°-193°) et de 173° ± 7,5° (161°-193°) et l'angle mécanique tibial (AMT) moyen de 90,5° ± 4° (82 - 102°) (dont 24 genoux avec un AMT en valgus) et de 85,5° ± 3,5° (79°-93°). Résultats : Tous les patients ont été revus à un recul moyen de 47 ± 25 mois (18-114) pour la série 1 versus 57,5 ± 24,5 mois (24-102). Le score IKS moyen était de 184 ± 6 points pour la série 1 (172-200) versus 185 ± 8,5 (163-200) pour la série 2 (p = 0,872). 37 et 38 patients étaient respectivement très satisfaits ou satisfaits de l'intervention. L'angle HKA moyen était de 180,5° ± 2,5° versus 181° ± 2° (p = 0,122) et l'AMT moyen de 89° ± 1,5° versus 90° ± 1° (p=0,001). Les résultats des 24 genoux avec AMT en valgus étaient statistiquement non différents. Conclusion : Les PTG post-OTOM, même avec un AMT préopératoire en valgus, ont des résultats identiques aux PTG1.
