Book excerpt: At present there are a growing number of biomolecules under investigation to understand their potential role as cancer biomarker for diagnostic, prognostic and therapeutic purposes. Intriguingly, the state of art on cancer biomarkers research shows interesting and promising results together to clamorous failures. Also from a clinical point of view, there are contradictory results on routine clinical use of the present cancer biomarkers. Some patients may be simply monitored in their course by a periodic blood sample, but sometimes this monitoring shows dramatic limits. A lot of patients show serious and extensive relapses without significant change in serum concentrations of biomarkers tested. Often the physician who should utilize these biomarker does not entirely know their limits and the total potential applications as well and sometimes this knowledge is influenced by economical and marketing strategies. This limited and "polluted" knowledge may have dramatic consequences for patient. The aim of this book is to diffuse all aspects of cancer biomarkers, from their biochemical peculiarities to all clinical implications by passing through their physiology and pathophysiology. This critical approach towards old and new cancer biomarkers should foster a deepened and useful understanding of the diagnostic and prognostic index of these fundamental parameters of laboratory medicine and in the same time facilitating the research of new and more sensitive-specific signals of the cancer cell proliferation.

Book excerpt: In the era of precision oncology, liquid biopsy techniques, specifically the use of circulating tumor cells (CTCs), cell free circulating tumor DNA (ctDNA) and extracellular vesicles particularly exosomes, represent a paradigm shift in the conventional use of tissue based biomarkers. Compared to tissue biopsy, liquid biopsy is cost-effective, readily accessible and less invasive, minimizing the bias of sampling and offering the opportunity for serial monitoring. Circulating biomarkers detected in liquid biopsy (blood and urine, etc.) are helping in understanding cancer genomic landscape and evolution, thus have been largely studied in term of cancer screening, risk stratification and early detection of minimal residual disease. Castration-resistant prostate cancers (CRPCs) are the late stage of prostate cancers. With significant progress towards the treatment of CRPCs, for example, androgen receptor-signalling (ARS) targeted therapy and immunotherapy, there is an urgent need to identify novel biomarkers to improve the survival rate and prognosis of CRPCs. The study of such biomarkers associated signal pathways and immune status has become a major focus in research to help understanding the responses to drug resistance and ineffective treatments. The discovery of these biomarkers in selecting potentially responsive patients will improve outcomes, and reduce treatment costs and, most importantly, significantly improve patient’s quality of life.

Book excerpt: Prostate cancer is the most common malignancy in males, and the third leading cause of male cancer-related death in the Western world. Although most prostate cancers are diagnosed at an early and treatable stages, predicting the outcome of prostate cancer progression and treatment has proven to be challenging because of the heterogeneous nature of the disease. Recent cancer genome studies have identified novel alterations as well as the potential actionable targets. Among these novel alterations is chromodomain helicase DNA-binding protein 1 (CHD1). CHD1 deletion occurs in as many as 20% of prostate cancers and may be associated with genomic instability. To validate the function of CHD1 in prostate cancer in vivo, we created the Pb-Cre+;Chd1L/L mouse model, whereby Chd1 is deleted in mouse prostate epithelial cells, and engineered CHD1-deleted prostate cancer cell lines. We found that while Chd1 deletion alone does not induce prostate cancer in vivo, it confers DNA damage sensitivity and homologous recombination impairment, making CHD1-deficient cells sensitive to PARP inhibitors and Platinum-based drugs. A metastatic castrate-resistant prostate cancer patient whose tumors had CHD1 deletion was responsive to carboplatin, suggesting that CHD1 status may be a biomarker for PARP inhibitor and platinum treatment responsiveness.
