Book excerpt: En France, le concept de santé environnementale a été introduit dans le Code de la Santé Publique qu' en 2004 et a été à l' origine des Plans Nationaux Santé Environnement (PNSE). Ce mémoire de thèse s' attache à montrer la légitimité du pharmacien d' officine dans la promotion de la santé environnementale. Il bénéficie d' une proximité avec les patients et les accompagne au travers de nouvelles missions officinales implantées dans son quotidien. Actuellement, le pharmacien d' officine peut se positionner au sein de nouvelles structurations d' exercice pluridisciplinaire, comme les Communautés Professionnelles Territoriales de Santé qui participent aux actions de prévention ; dans ce cadre-là, la promotion de la santé environnementale trouve toute sa place. Cependant, de telles avancées semblent ralenties par un manque de formation des pharmaciens, comme le montre l' analyse des formations proposées en santé environnementale dans les 24 unités de formation et de recherche (UFR) de Pharmacie en France. Pour élargir les perspectives de développement de la santé environnementale dans les officines françaises, des modèles d' exercice de pharmaciens à l' étranger ont été étudiés. A l' officine, le pharmacien peut aborder des thèmes de santé environnementale par différentes approches : par les milieux (par exemple, la pollution de l' air), par les populations vulnérables (par exemple, les femmes enceintes et les jeunes enfants) ou par les pathologies. Pour ces problématiques environnementales, une description des effets sur la santé, des exemples d' outils à utiliser sous différents formats (numérique, papier ou atelier) et des conseils destinés aux patients, sont présentés.

Reconnu depuis 1994 par l'Organisation Mondiale de la Santé, le concept de Santé Environnementale n'est réellement apparu en France qu'en 2004, suite à la publication du Plan National Santé Environnement 1 (PNSE 1). Ainsi, après une définition du concept de santé environnementale, ce mémoire de thèse présente son émergence et son développement au sein des politiques gouvernementales. Les Programmes Nationaux Santé Environnement 1 et 2 sont donc décrits. Dans une deuxième partie, quelques grands risques en santé environnementale sont présentés. Sont ainsi analysés des risques qualifiés d'"anciens" et "connus" (pollution atmosphérique urbaine, rayonnements ultra-violets, plomb, amiante, températures extrêmes, energie nucléaire) et des risques qualifiés d'"émergents" (téléphonie mobole, bruit, diodes électroluminescentes, résidus de médicaments dans l'eau destinée à la consommation humaine, pollution de l'air intérieur, nanomatériaux). Pour chaque risque, une définition, un état des connaissances, un bilan des politiques de prévention et une synthèse des recommandations actuelles sont présenté. L'approche permettant d'introduire la santé environnementale dans le domaine de l'officine fait l'objet de la dernière partie. La formation initiale et continue du pharmacien d'officine et des autres professionnels de santé est évaluée. Puis, le rôle du pharmacien d'officine est défini en termes de prévention, d'éducation et d'identification des population à risques. Enfin, une réflexion sur la faisabilité de l'éducation à la santé environnementale à l'officine est conduite ; des outils et des exemples sont proposés pour illustrer la démarche à suivre.

Ils proposent des objectifs à atteindre, des mesures à mettre en place pour mieux comprendre et réduire les risques liés aux différentes nuisances (physiques, chimiques ou microbiologiques) et encouragent la recherche. Au fil des années, quatre plans se sont succédés de 2004 à nos jours, et de nombreux plans annexes sont venus les compléter. Ces Plans Nationaux Santé-Environnement explorent de manière non exhaustive de nombreux thèmes de santé environnementale : la qualité de l'air intérieur et extérieur, la qualité des eaux et milieux aquatiques, l'alimentation, la santé mentale, l'activité physique, la santé au travail, les ondes électromagnétiques, la lumière bleue, les nanomatériaux, les populations vulnérables (femmes enceintes et enfants), la lutte contre la consommation de tabac, les perturbateurs endocriniens, les pesticides, les résidus de médicament, la gestion des déchets, les substances cancérigènes, mutagènes et reprotoxiques, le bruit, les transports, la conservation de la biodiversité et le changement climatique. Après une présentation de chacune de ces thématiques d'après les Plans Nationaux Santé-Environnement et les plans annexes correspondant, ce travail analyse le rôle que peut jouer le pharmacien dans l'application au quotidien de la santé environnementale à l'officine. Déjà par sa proximité avec les patients et les conseils qu'il délivre, le pharmacien et son équipe peuvent permettre une meilleure compréhension des liens entre santé et environnement et participer au développement des comportements favorables à la santé environnementale. De plus par l'aménagement de son officine et des alentours, il peut développer un environnement favorable à la santé environnementale ou encore, par l'adaptation de ses politiques d'achats et de gestion des déchets, il peut encourager le développement d'un fonctionnement socio-économique favorable à la promotion de la santé environnementale à l'officine. Enfin, si les institutions professionnelles et décisionnaires en construisent le cadre opérationnel, le pharmacien pourrait inscrire comme nouvelle mission de sa profession, le développement d'un réseau social favorable à la santé environnementale.

Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.
