People identifying as lesbian, gay, bisexual, transgender, and queer (LGBTQ+) have increased over the past few years, with continued growth expected. Consequently, nursing faculty need the necessary knowledge, skills, and attitudes to teach nursing students to provide quality nursing care to LGTBQ+ patients who are less likely to receive equitable health care because they are members of a diverse population. Therefore, addressing their healthcare needs in the nursing curriculum is essential in alleviating healthcare barriers and biases. Methodology: The equity, diversity, and inclusion (EDI) guidelines were presented to the nursing faculty, applied by simulation course chairs in clinical simulations, and used to review the associate degree nursing (ADN) program student application to encourage qualified applicant diversity. Qualtrics quantitative pre- and post-survey questions were offered to nursing faculty to assess faculty comfort in teaching nursing students how to support LGBTQ+ healthcare needs and the importance of including EDI in the nursing curriculum. In addition, the simulation course chairs and ADN director were asked verbal qualitative survey question(s). Results: Overall, nursing faculty were receptive to increasing EDI in the curriculum, simulation, and ADN application. Implications for practice: Utilizing the Nursing EDI Guidelines for the LGBTQ+ Population increase the faculty awareness and address healthcare disparities to bridge a nursing education gap. The guidelines are a resource accessible online, aiding nursing faculty in incorporating EDI into the nursing curriculum.

Book excerpt: At a time when lesbian, gay, bisexual, and transgender individuals-often referred to under the umbrella acronym LGBT-are becoming more visible in society and more socially acknowledged, clinicians and researchers are faced with incomplete information about their health status. While LGBT populations often are combined as a single entity for research and advocacy purposes, each is a distinct population group with its own specific health needs. Furthermore, the experiences of LGBT individuals are not uniform and are shaped by factors of race, ethnicity, socioeconomic status, geographical location, and age, any of which can have an effect on health-related concerns and needs. The Health of Lesbian, Gay, Bisexual, and Transgender People assesses the state of science on the health status of LGBT populations, identifies research gaps and opportunities, and outlines a research agenda for the National Institute of Health. The report examines the health status of these populations in three life stages: childhood and adolescence, early/middle adulthood, and later adulthood. At each life stage, the committee studied mental health, physical health, risks and protective factors, health services, and contextual influences. The literature report a majority of persons who are lesbian, gay, bisexual, and transgender (LGBT) have experienced discrimination from a healthcare provider. This study encouraged nurses to assess their beliefs and behavior when providing health services to lesbian and gay individuals. Such self-assessment can positively impact patient outcomes. Nurses acknowledging sexual orientation and providing a safe environment for the patient and their family improves outcomes by providing patient-centered care.Aim. To assess their beliefs and behavior when providing healthcare services to gay and lesbian individuals using the Gay Affirmative Practice (GAP) scale.Methods. The study involved convenience sampling of registered nurses in a southwest urban hospital system utilizing an eNewsletter with a link to a Qualtrics survey that included a consent statement, demographic questions, and the Gay Affirmative Practice (GAP) Scale. The data was analyzed using descriptive statistics.Findings. Of 70 respondents, 89% were female, 91% Caucasian, 90% heterosexual, and 84% Christian. The average age was 49; 76% had a baccalaureate or master’s degree; 71% had a nursing certification; 60% had a friend or family member who is gay or lesbian; and 61% believed a cultural competence course would benefit them professionally. The mean GAP score was 110 (150 possible); the average belief score was 63 (75 possible); and the average behavior score was 49 (75 possible).Conclusions. Small sample size limited generalizability. Belief scores were higher than behavior scores, likely representing nurses’ positive beliefs and their requirement for education to increase behavior scores. Recommendations include future research with larger sample sizes throughout the nation; development of a tool that has language more reflective of nursing; and promote cultural competence courses with definitions of LGBT terminology and how to ask sexual orientation- and gender identity- questions in a safe and optimal manner.

Book excerpt: "Lesbian, gay, bisexual, and transgender (LGBT) patients experience significant health disparities that affect morbidity, mortality, and overall quality of life. Nurses who express negative biases in the delivery of care influence many of the disparities facing this vulnerable patient population. Educational interventions that are focused on increasing nurses' knowledge of LGBT healthcare needs have proven effective in improving patient compliance and enhancing patient care outcomes. This evidence-based practice change project was designed to educate nurses (n=25) on the healthcare needs of the LGBT population. The project included a pretest and posttest with an educational intervention. The project manager, in collaboration with subject matter experts, developed a questionnaire, which consisted of 15 knowledge and five attitude questions. The questionnaire was administered before and after an educational intervention to measure any changes in nurses' knowledge and attitudes toward LGBT patients. Findings indicated that knowledge scores increased from a baseline of 69.1%, which was below the 80% benchmark, to a post intervention score of 97.6%, which was above the intended benchmark. The average mean scores measuring attitudes increased significantly, t = 2.06 and p = .04. The findings of this evidence-based practice change project have significant implications on the effects of educating nurses on the healthcare needs of the LGBT population, that dramatically influence practice and patient care outcomes. Book excerpt: In the United States (US), lesbian, gay, bisexual, transgender, and queer (LGBTQ) individuals are at an increased risk for contracting sexually transmitted infections, are less likely to receive recommended health screenings, are more likely to abuse alcohol and other substances, are at a higher risk for attempting suicide, and experience greater health disparities compared to their heterosexual counterparts. Compounding these health-related problems is a significant discrepancy whereby LGBTQ people have poor or no access to health care. One approach to correct the health disparities the LGBTQ community faces is through LGBTQspecific health education for nursing students. A review of the literature revealed undergraduatelevel nursing programs should employ competent faculty, incorporate increased instruction time, use updated textbooks, assign projects or entire modules specific to the health care needs of LGBTQ patients, employ simulations when available, and incorporate LGBTQ topics throughout the undergraduate nursing curriculum. These changes have the potential to make a positive impact on improving health care for LGBTQ patients in the US.

Book excerpt: The focus of this capstone project is to explore the impact of registered nurses' bias and lack of knowledge associated with the care of the LGBT patient. A Quasiexperimental design was used to evaluate the cause-and-effect relationship of an educational intervention. This intervention provided LGBT cultural knowledge and provides evidence regarding how homophobia and transphobia among nurses creates and perpetuates disparities among LGBT people. Attitude and knowledge assessment tools were used to collect responses from participant's pre and post intervention. Using parametric and descriptive statistics, the participants' data were analyzed. There were no statistically significant differences between the pre and post intervention scores. Although statistical significance was lacking, clinical significance was inferred by the participants' knowledge gap in their posed questions at the conclusion of the educational intervention. The implementation of similar training sessions, offered in a recurring fashion, will likely be necessary to effectively decrease the healthcare disparities currently being experienced by the LGBT population.
