Matériel et Méthode : des entretiens qualitatifs semi-directifs ont été réalisés auprès de dix femmes. Elles ont écrit un projet de naissance et accouché à la maternité du CHU MORVAN de Brest entre octobre 2008 et octobre 2009. Les thèmes abordés permettent d'avoir des renseignement sur leur choix d'établissement, le vécu de leur prise en charge ainsi que sur le respect de leur projet de naissance. Résultat et discussion : l'analyse des résultats a mis en évidence d'une part, la satisfaction des patientes et d'autre part l'importance de l'information et de la communication. Les projets de naissances servent de support aux demandes des femmes mais sont également des outils de communication avec l'équipe médicale. La plupart des souhaits des femmes sont réalisables dans la maternité. L'écoute et l'accompagnement sont les facteurs influençant le plus le vécu d'accouchements que le projet de naissance ait ou non été respecté. Le choix de la maternité s'effectue après réflexion et renseignements auprès de l'entourage et d'internet. Les femmes privilégient la concordance entre leurs demandes et les pratiques effectuées dans l'établissement ainsi que la proximité. Conclusion : les sages-femmes jouent un rôle important dans la prise en charge des femmes écrivant des projets de naissance. Les patientes souhaitent la meilleure information possible pendant la grossesse et l'accouchement. L'entretien prénatal précoce semble être un lieu approprié pour que les mères expriment leurs attentes, afin d'aboutir à l'écriture de leur projet de naissance au troisième trimestre. Un entretien systématique avec des femmes est indispensable à mettre en place pour définir des possibilités communes de prise en charge entre le couple et l'établissement de santé.
