One of these is the pressure to which the food at the pylorus is subjected by the recurring peristaltic waves; the other is the action of the pyloric sphincters. Cannon has shown that gastric peristaltic waves of practically the same intensity are continuously run ning, so long as food remains in the stomach. One of the two factors, the pressure in the antrum, is, therefore, recurrently constant; and the control of the discharge must consequently reside with the other factor, the action of the pyloric sphincter. The substance of Cannon's work is the explanation of this action, as well as the differences in the rate of discharge of different foodstuffs from the stomach. It has been shown that when representative carbohydrate, proteid, and fatty foods of uniform amount and consistency are separately fed to an animal, the carbohydrates begin to leave the stomach within ten minutes after ingestion, and are passed out rapidly. Proteids commonly do not leave the stomach at all during the first half-hour, and sometimes not for an hour, and then they are expelled only slowly. Pastore's5 series of experiments were carried out to show the effect of saliva in the presence of the gastric and pancreatic juices of dogs and also the gastric and pancreatic extracts of men. The first series of cases shows the pronounced action of human saliva on starch in mild alkaline concentration, even to a dilution of one in eight thousand; but above this dilution digestion cannot proceed without neutralization. A concentration of one in two thousand always destroys the amylolytic power when ptyalin is present with hydrochloric acid alone. It is now being tried at four London hospitals, with encouraging results. The method consists in intra muscular injections containing both an antigen 31111 ferments; the effect of the latter being to dissolve the wax envelope of the bacillus and render it accessible to the white corpuscles and ferments of the blood. For febrile eases, additional carefully graduated intramuscular and intravenous injections are made of ferments in special combinations with lipoids. During the expectant interval which must intervene, it may be opportune to glance back at the latest phase of the tuberculin question immediately antecedent to that new idea.
