Book excerpt: The Department of Health has reported that the NHS achieved £5.8 billion of savings in 2011-12, virtually all of the forecast total of £5.9 billion. Most of the savings were generated through the pay freeze for public sector staff, and reductions in the prices primary care trusts pay for healthcare. NHS bodies also made savings by cutting back-office costs. However, there is limited assurance that all the reported savings were achieved. The NAO substantiated a total of £3.4 billion of NHS efficiency savings. Although the savings made by NHS providers as a percentage of operating costs are increasing, it is not clear what level of savings is sustainable over time. Changes to transform services take time to implement and the Department has always expected that these savings will predominantly come in the latter half of the four-year period. The NHS is seeking to maintain the quality of, and access to, healthcare at the same time as making efficiency savings. In 2011-12, the NHS performed well against headline indicators of quality, including waiting times and healthcare associated infection rates. The indicators focus mainly on hospital care and the Department faces a significant challenge in monitoring quality across the NHS as a whole. The Department does not know whether the demand for healthcare is being managed in ways that inappropriately restrict patients' access to care. Reducing demand and redesigning care pathways to treat patients in the most appropriate setting are key ways of generating savings

Book excerpt: The NHS has achieved its financial savings target, but this has in large part come from freezing wages and there is concern that other savings are being achieved by rationing patients' access to certain treatments. These include cataract surgery and hip and knee replacements. These procedures are described as being 'of low clinical value' but they can make a real difference to a patient's quality of life. Furthermore, the finances of some trusts are fragile, and there is a risk they may resort to simple cost-cutting rather than finding genuine efficiency savings. The NHS must fundamentally change the way that healthcare is provided to secure the level of savings needed in the future, for example by moving services out of hospitals and into the community. The Committee is not satisfied that the Department and the NHS Commissioning Board is doing enough to help the NHS transform services. Local people are understandably resistant when proposals are made to close their local hospital or reduce the range of services it provides. It is down to the Department to make a clear case for change from the patient's point of view, demonstrating the benefits in terms of the quality and safety of care as well as cost savings. Although the Department reported that the NHS made savings in 2011-12 of £5.8 billion, virtually all of that year's forecast of £5.9 billion, that data is not fully reliable. The care provided by the health and social care system will break down if quicker progress is not made to develop more integrated health and social care services which focus on meeting the needs of individual patients. It is unlikely that public expenditure on health and social care services will increase significantly in the foreseeable future. This means that the only way to sustain or improve present service levels in the NHS will be to focus on a transformation of care through genuine and sustained service integration. There must be a much more joined up approach to commissioning health and care services. Book excerpt: 15 million NHS patients in England with long-term conditions such as diabetes, arthritis and asthma account for 70% of the annual expenditure of the NHS in England. One projection estimating that the bill for treatment of long-term conditions will require the NHS to find £4 billion more each year by 2016. Increasingly, patients do not have a single long-term condition but live with two or more conditions, complicating treatment and adding to its cost. The Committee strongly supports the development of individual care planning for people with long-term conditions, based on the principles successfully demonstrated in the NHS House of Care programme. Care planning approaches will involve GPs, community health services and specialists sitting down with the patient to draw up a personalised plan for the care required, which includes the support needed to help the patient manage his or her own condition. The challenge, though, of introducing personalised care planning for 15 million people is substantial. The Committee looked at the prevailing view that services to treat long-term conditions should be moved out of hospitals and into primary and community care. To provide effective care for these conditions, services have to be maintained across all settings, from support in the home through to acute specialist care, and many conditions will continue to require specialist services delivered in hospital. Effective management of long-term conditions also requires collaboration with other government providers, such as housing and transport services.

This book explores the various management knowledges and associated texts apparent in English health care organizations, and considers how the local reception of these texts was influenced by the macro level political economy of public services reform evident during the period of the politics of austerity. The research outlined in this volume shows that very few evidence-based management texts are apparent within health care organizations, despite the influence of certain knowledge producers, such as national agencies, think tanks, management consultancies, and business schools in the industry. Bringing together the often disconnected academic literature on management knowledge and public policy, the volume addresses the ways in which preferred management knowledges and texts in these publicly funded settings are sensitive to the macro level political economy of public services reform, offering an empirically grounded critique of the evidence-based management movement.

Each year central Government offices produce approximately 2.3 million tonnes of CO2 emissions (around 0.4 per cent of the UK total) and 309,000 tonnes of waste. Central Government spends £60 billion on goods and services each year and through sustainable procurement it could accelerate the take-up of environmentally friendly products. This report examines progress relating to: carbon emissions; renewable energy; carbon neutrality; energy consumption; Government response to the Sustainable Development Commission (SDC); the role of the Sustainable Development Commission. The record is poor on carbon emissions. Excellent progress has been made on increasing the use of green electricity, but progress on generating its own electricity from on-site or district renewables has been very disappointing. The target of making the Government Estate carbon neutral by 2012 will depend very heavily on buying offsets. It is important the Government does as much as possible to reduce its own emissions. The newly-announced Greening Government IT programme is encouraging, given that increased use of IT would appear to be the biggest single factor in the upward trend in emissions from civil departments. The Government has, encouragingly, announced significant reforms in its response to the SDC's report. The Commission is having a significant influence in galvanising Government to improve its structures.

Book excerpt: Government departments have been set a series of targets for implementing efficiency gains of £21.5 billion a year by 2007-08, as part of the Treasury's Efficiency Programme following on from the Gershon Review of public sector efficiency. This NAO report finds that considerable progress has been made by departments towards achieving the efficiency targets, and the Efficiency Programme has ensured a greater focus on value for money issues among senior staff. Progress has also been made in addressing measurement issues, supported by new guidance issued by the Office of Government Commerce, although some reported efficiency gains still carry a significant risk of inaccuracy. The OGC has also initiated a new reporting system to improve the accuracy of reported efficiency gains, but this would benefit from a greater review process to assess whether they meet good practice, either through a stronger internal audit within departments or by the OGC. Departments must improve the transparency of the reporting process, and do more to encourage staff to put forward ideas for efficiency improvements. A companion volume is available separately (HCP 156-II, session 2006-07, ISBN 9780102944372) containing twelve opinion pieces on options to improve efficiency in the public sector, written by commentators from the fields of business, trade unions, academia and management consultancy.
