Book excerpt: The concept of using encapsulation for the immunoprotection of transplanted cells was introduced for the first time in the 1960s. "[Microencapsulated cells] might be protected from destruction and from partici pation in immunological processes, while the enclosing membrane would be permeable to small molecules of specific cellular product which could then enter the general extracellular compartment of the recipient. For instance, encapsulated endocrine cells might survive and maintain an effective supply of hormone." (Chang, Ph. D. Thesis, McGill University, 1965; Chang et aI., Can J Physiol PharmacoI44:115-128, 1966). We asked Connaught Laboratories, Ltd., in Toronto to put this concept into practice. In 1980, Lim and Sun from Connaught Laboratories reported on the successful implantation of poly-I-Iysine-alginate encapsu lated rat islets into a foreign host. [Lim and Sun, Science 210:908-909, 1980]. Now many groups around the world are making tremendous progress in the encapsulation of a multitude of cell types. Kiihtreiber, Lanza, and Chick have invited many cell encapsulation groups from around the world to contribute to this book. The result is a very useful reference book in this rapidly growing area. With so many excellent au thors describing in detail the different areas of cell encapsulation, my role here will be to briefly discuss a few points.

Classes IV and V (beta2-MG clearance greater or equal to 50 and 70 ml/min, respectively, at a blood flow rate of 200 ml/min) are the most common ones, used in more than 90% of Japanese dialysis patients. At the congress it became clear that both the technical and therapeutic sides developed very rapidly and it appeared fruitful to bring together the investigators of the different countries working in the areas. At that time immunology had come to pervade many clinical specialities, and hemapheresis, especially plasmapheresis was considered a therapeutic tool in many immunological diseases which hitherto had proved to be fatal. New methods to identify certain antibodies and circulating immune complexes in the serum and the possibilities to remove them from the blood by several techniques (filtration, centrifugation, immunoabsorp tion) led to an almost uncontrolled use of plasma exchange in a variety of diseases. Since then the technical possibilities of this technique were further recognized, as was the impact of immunology on many diseases, and the possibilities to collect specific components for therapeutic pur poses. But also we became aware of the limited contributions of anec dotal data on successes or failures of apheresis as adjuvant treatment. Therefore international prospective studies were initiated to make critical assessment possible of apheresis in various diseases.

It is my great pleasure to present Artificial Heart 3 (Proceedings of the 3rd International Symposium on Artificial Heart and Assist Devices). The third symposium was held in Tokyo on February 16 and 17, 1990. Our original intention was to invite all the principal investigators from the major artificial heart research laboratories in the world, in the first three symposia. The numbers of investigators in the fields of the artificial heart, ventricular assist systems, and biomaterials, invited for the symposia, totalled 7 in 1985, 8 in 1987, and 13 in 1990. The program of the third symposium consisted of 41 papers; 13 invited lectures, 8 papers contributed upon request, with 2 from the United States, and 20 regular selected papers including 2 from the United States, ana 1 each from Australia, Germany, and South Korea. During the last three years, the state of clinical application of artificial hearts and assist devices has changed. In the case of pneumatically driven blood pumps, we have now stepped into the age of practical use in patients. Accordingly, in this symposium we gave a special emphasis to topics of clinical application, particularly the use of an artificial heart as a bridge to heart transplantation. Sessions on implantable artificial hearts, biomaterials for the artificial heart, and heart transplantation were also included.
