Motivational interviewing (MI) is an emerging intervention for adolescent health risk behavior that has promise. Despite its potential fit in primary health care contexts, primary care providers tend to express reticence in learning or adopting MI practices. This program evaluation project assessed the usefulness of Motivational Interviewing (MI) by primary care providers with adolescents in school based health centers that was implemented by a large randomized clinical trial (RCT) in southwest United States high schools. Methods: As part of the RCT, the primary care providers were required to do MI sessions with 66, 45, 25, and 91 youth participants respectively at their School-based site. It was anticipated that providers would be increasingly comfortable with MI between their first and final participant and that youth would be increasingly satisfied as provider comfort increased. To evaluate this, as part of the RCT, a set of primary care providers working with youth in school based health centers were trained through didactic sessions, homework, and role-play. School-based providers were provided an introductory training in MI, MI reading materials, a study manual, and were required to participate in bimonthly tele-coaching to ensure treatment integrity and fidelity. Primary care provider and adolescent participant survey data from session 1 of the RCT was analyzed to determine provider comfort with MI and adolescent satisfaction with MI. Summary of findings: Despite the standard training practices, providers showed a range of interest in comfort with the intervention. Book excerpt: With childhood obesity on the rise, there is increasing concern about the prevention and treatment of obesity. Obesity occurs in children/adolescents and is associated with increased health risks and psychosocial problems. Standard treatment options for obesity can include dietary, physical activity, and behavioral therapy. In this project, an enhancement to the current standard treatments was conducted. Motivational Interviewing has been shown to aid in changing weight-related behaviors for adults. The present study evaluated the effects of Motivational Interviewing on children/adolescents preceding an obesity intervention. A multi-method survey approach was utilized. Results indicated that four out of the sixteen participants showed improvement in their readiness to change weight-related behavior. Furthermore, the majority of parents reported that their children showed increased awareness in weight related behaviors, improvement in mood, and positive lifestyle changes. Parental involvement appears to be one significant component in obesity treatments, such that parents supported and reinforced children's healthy weight-related behaviors. Future studies should include a larger pool of participants with a control group and a specific parental involvement component.

Book excerpt: Abstract Purpose: Childhood obesity is a significant health problem associated with increased adult morbidity and mortality. Rural areas like Central Valley California demonstrate disproportionate rates of obesity. Children in rural and underserved populations are at a higher risk of developing obesity. Data suggest that obesity is often under-addressed during routine health care visits. Primary care practitioners are the first point of health care contact and have a unique opportunity to prevent and address obesity through health monitoring and education. In response to escalating rates of obesity, clinical recommendations have been established for primary care providers including the use of patient-centered approaches to motivate families on behavior change. The use of Motivational Interviewing (MI) encourages the patient to recognize their readiness for change, assess the benefits of embracing healthy behavior, and evaluate their confidence and perceived ability to make the change. Recommended behavioral changes to address childhood obesity include encouraging consumption of nourishing foods rich in whole grains, low fat or fat-free foods, fruits and vegetables, lean meat and other protein sources, and daily physical activity of 60 minutes or more for ages 6 to 18 with emphasis on aerobic, muscle, and bone strengthening. This DNP student developed a health promotion project that focused on educating Family Nurse Practitioner (FNP) students from the University of San Francisco (USF) and San Jose State University (SJSU) on best practices for addressing childhood obesity in the primary care setting. A prevention strategy was designed to use MI to address childhood obesity within the context of a routine pediatric health screening. Method: An in-service was conducted for the FNP students including (a) MI techniques and Change Talk interactive scenario to elicit change in behavior, physical activity, and diet; (b) BMI screening guidelines; (c) 5-2-1-0 rule educational tool; (d) the Choose My Plate educational tools; and (e) the Healthy Eating and Activity Together (HEAT) educational tool. Interventions were implemented at Reedley and Orange Cove High Schools in Kings Canyon Unified School District in Fresno County during the sports physical assessment program. Measures: Evaluation metrics included a pre/post-test questionnaire to measure student FNP (n=15) knowledge and confidence after delivery of the MI education to students from Reedley and Orange Cove High Schools. The survey evaluated the knowledge of MI techniques, childhood obesity prevention readiness, confidence in the use of MI, and potential barriers to participating in the prevention strategies. Results: Project outcomes showed an increased mean of 4.13 (SD 1.06) in knowledge, while confidence increased from mean of 2.6 (SD .06) to 4.13 (SD 0.64) in MI techniques, childhood obesity screening guidelines, and BMI screening. Conclusions: Outcomes indicate that an education program can increase student FNP's knowledge and confidence in using the MI techniques to address childhood obesity in the primary care setting. This research proposal will specifically ask if integrating motivational interviewing into a 6-week, family-based, weight management program located in Los Angeles serving the Latino families, is an effective, feasible, and cost-effective intervention that will result in increased weight loss for participating children.

Book excerpt: Background: Dyslipidemia, overweight and obesity (OW/OB) in the pediatric population contributes to metabolic complications and future cardiovascular disease. The prevalence varies but the problem is a global issue. Adolescence is a challenging developmental stage with many factors influencing adolescent health. Lifestyle management is the preferred treatment. Motivational interviewing is a promising counselling approach; however, evidence is limited for this population. Objectives and Methods: The substantive focus involved exploring MI for dyslipidemic adolescents. Study A was a retrospective cohort comparison aimed to assess a collaborative educational approach with MI elements for pediatric patients (n=38) newly referred to a specialized lipid clinic, versus historical patients who had received a more didactic educational approach (n=31). Study B involved theoretical research to build and graphically represent a conceptual framework for MI, including special applications to adolescents. Study C was a mixed-methods, randomized controlled trial (N=32) to evaluate the outcomes of MI counselling, comparing adolescents together with a parent versus adolescents alone. A qualitative component occurred following the quantitative data collection to explore the perceptions of participants. Findings: Despite some limitations, sufficient promise was observed in the Study A findings to move forward with a detailed plan to rigorously explore MI. Significantly improved lifestyle and anthropometry outcomes were observed favouring the collaborative education approach with MI. The theoretical work in Study B defined four key elements for a MI framework: the Person, the Problem, the Process, and the MI Counsellor. The between group results for Study C were equivocal; however, with both groups combined in a secondary analysis, there were significant improvements in nearly all outcomes. The interviews enriched an understanding of the participantsâ lives, lifestyle change, and the counselling process. Conclusion: Motivational interviewing is an effective and desired counselling strategy for adolescents with dyslipidemia and overweight/obesity. Further research with a longer period of observation is needed to determine whether achieved lifestyle improvements can be sustained in the long-term.
