Targeting intracellular signalling molecules is one of the novel approaches in which basic and translational researches meet clinical practice to define the best management of malignancies. The goal for target therapies is to identify agents that target tumor-specific molecules, thus sparing normal tissues. Those molecules are called biomarkers, and their identification is recommended for correct therapeutic planning. Hence, the contribution of clinical and surgical pathology to define cancer prognostic groups and treatment responsive tumors is essential not only for the efficacy of the cures, but also for ethical, economical, and social reasons. The promises of tailored (personalized) therapy are now almost certainly realistic and can give concrete hope to many cancer patients in the world. In this book, we have tried to describe the "state of art" in these fields of oncology, specifically considering prognostic and predictive response therapy factors in colorectal, breast, lung, gastric, renal, liver and prostate cancers.

The identification of germline mutations in BRCA1 or BRCA2 has led to the development and introduction of PARP inhibitors for the treatment of advanced and early-stage breast cancer, along with personalized follow-up and prophylactic surgical procedures for patients with or without cancer, carrying such mutations.

This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Historically and still with some types of cancer, stage IV is perceived by patients as “terminal.” Fortunately, recent molecular therapies have extended the lives of patients with advanced cancer and reassuringly people living with metastatic disease increasingly visit our clinics. What is the path forward? Given that the consilience of science and medicine is a dynamic art from which therapies arise, it would be misguided to consider any single work adequate at capturing the horizon for research. These proteins enhance the growth of cancer cells and protect tumors from treatments such as drugs or surgery. However, new drugs have recently been developed particularly those targeting heat shock protein 90. As heat shock protein 90 functions to stabilize many of the oncogenes and growth promoting proteins in cancer cells, such drugs have broad specificity in many types of cancer cell and offer the possibility of evading the development of resistance through point mutation or use of compensatory pathways. Heat shock proteins have a further property that makes them tempting targets in cancer immunotherapy. These proteins have the ability to induce an inflammatory response when released in tumors and to carry tumor antigens to antigen presenting cells. They have thus become important components of anticancer vaccines. Overall, heat shock proteins are important new targets in molecular cancer therapy and can be approached in a number of contrasting approaches to therapy.

In 2018, we saw the first “site-agnostic” drug approvals in cancer (for microsatellite unstable cancer (PD-1 inhibitors) and NTRK-fusions (TRK inhibitors)). Research on targetable mutations, determination of genetic “signatures” that can use multiple individual genes/pathways, development of targeted therapy, and insight into the value of new technology remains at the cutting edge of research in this field. We are soliciting papers that present new technologies to assess predictive biomarkers in cancer, original research (pre-clinical or clinical) that demonstrates promise for particular targeted therapies in cancer, and articles that explore the clinical and financial impacts of this paradigmatic shift in cancer diagnostics and treatment.
