Il démontre avec force que les ultrasons peuvent jouer un rôle très important dans l’imagerie moderne tant diagnostique que thérapeutique en raison de leur performance diagnostique mais également de leur caractère non invasif. Nous espérons que vous prendrez autant de plaisir à y rechercher l’information utile que nous en avons eu à découvrir une imagerie fonctionnelle ultrasonore en temps réel et écrire ces différents chapitres.

Book excerpt: The value of ultrasound contrast agents (USCA) in everyday clinical practice depends on the pharmacokinetics, the signal processing, and the contrast-specific imaging modalities. Second-generation USCA, are blood pool agents that do not leak into the organ tissue to be examined but remain in the intravascular compartment increasing the Doppler signal amplitude during their dynamic vascular phase. Taking advantage of the stability of their microbubbles, they can withstand the acoustic pressure of insonation much better than first-generation contrast media, which results in an increased half-life of the agent and, consequently, in a prolonged diagnostic window. Concomitant with the improvement of contrast agents, different contrast-specific imaging modalities have been developed which, used in combination with USCA and a low mechanical index, allow continuous real-time grey-scale imaging. These recent technical improvements have opened new possibilities in the use of USCA in a variety of indications. A large amount of literature has been published on the medical applications of such contrast agents. However, there is no textbook giving a broad overview of the physics and acoustics of the agents. This monograph aims to fill this gap. The book is written by a physicist, from a physics point of view, and it tries to draw links from the physics and acoustics to the medical imaging methods, but medical applications are mainly included for background information. The book consists of nine chapters. The first three chapters give a broad overview of the acoustic theory for bubble-sound interaction, both linear and nonlinear. Most contrast agents are stabilized in a shell, and this shell can have a strong influence on the interaction between the bubbles and the ultrasound. The effect of the shell is given special attention, as this is not easily found in other bubble literature. The following chapters, 4, 5, 6, and 7, describe experimental and theoretical methods used to characterize the acoustic properties of the agents, and results of studies on some agents. Chapter 8 shows how the theory and the experimental results can be combined and used to model various phenomena by means of computer simulations. The main purpose of the simulations is to get insight into the mechanisms behind the described phenomena, not to get accurate predictions and values. The book is aimed at both newcomers into the field, as well as those who are more experienced but want better insight into the acoustics of the contrast bubbles.

Depuis quelques années, les transducteurs capacitifs micro-usinés (cMUTs) se présentent comme une alternative intéressante aux transducteurs piézoélectriques classiques et offrent certains avantages comme une large bande passante. Nous proposons dans cette thèse d'évaluer le potentiel de cette technologie pour l'imagerie de contraste. Dans un premier temps, notre étude s'est orientée vers l'adaptation des cMUTs à l'imagerie non linéaire. Ensuite, de nouvelles méthodes de détection de contraste, basées sur le comportement spécifique des microbulles, ont été développées pour exploiter les avantages de la technologie cMUT. Comparés aux méthodes conventionnelles, les résultats obtenus montrent une meilleure visualisation des agents de contraste par rapport aux tissus environnants. L'utilisation de cMUTs améliore l'efficacité de ces méthodes démontrant, ainsi, leur intérêt pour l'imagerie de contraste.

Elle consiste en l'amélioration des performances de l'imagerie classique par injection intraveineuse d'un produit de contraste, constitué par une population de microparticules gazeuses (diamètre inférieur à 10 pm). L'agent de contraste ultrasonore est strictement vasculaire, il permet d'augmenter l'intensité vidéo sur 1 'image écho graphique et le signal Doppler des zones vascularisées dans le but d'améliorer la visualisation de structures internes du corps humain et d'aborder l'imagerie fonctionnelle telle que la perfusion myocardique. Les performances acoustiques in vitro de ces produits sont avérées. Cependant, l'utilisation de ceux ci dans le corps humain n'a pas permis d'obtenir la même efficacité. Ainsi, ce travail à pour but de mieux connaître le produit et ses interactions avec le milieu intracorporel. Les int1uences du milieu (filtrage pulmonaire, dilution du bolus de l'agent de contraste, diffusion du gaz interne des particules, pression cardiaque, ...) et de l'onde acoustique sont abordées pour connaître les modifications physiques du produit in vivo. L'impact de ces paramètres physiques sur la réponse acoustique des particules est étudié à partir d'un modèle acoustique linéaire dont nous précisons le domaine de validité. L'évaluation clinique se réalisant sur l'image échographique, nous présentons une simulation des signaux échographiques des produits de contraste. Cette approche qualitative est complétée par une approche quantitative portant sur la mesure de la perfusion. En premier lieu, nous comparons un modèle basé sur la mécanique des fluides, décrivant la dilution d'un bolus d'agent, à des mesures acoustiques effectuées sur un système circulant tubulaire. En second lieu, nos conclusions sur ce fantôme sont appliquées à l'étude de la perfusion myocardique sur des cœurs isolés de porcs.

Book excerpt: In the last few years, the development of sonographic contrast agents - or "microbubbles" - has stimulated increasingly intensive studies on the rela tionships between ultrasound and contrast media. As a result, "contrast -spe cific" hardware and software systems have been introduced by different ultra sound manufacturers with impressive speed. This has finally led to the birth of a very new imaging modality -"contrast-enhanced sonography" (CEUS). Since 1999, the introduction of second-generation contrast agents has represented a decisive step towards the extensive clinical use of CEUS and has simultaneously made obsolete most, if not all, scientific publications available so far. This book is, to our knowledge, the first to deal entirely with second generation contrast agents and the most updated contrast-specific software for noncardiologic uses. The reasons why the liver has been chosen as the only "target" of the book are easily understandable by radiologists and hepatologists alike. The study of vascularity is the only purpose of CEUS, and the liver has a unique vascular system, with two different inflow systems resulting in a single outflow. Furthermore, the pathologically different focal liver lesions (FLLs) are mostly characterized by different "models" of vascularity: CEUS can mimic contrast enhanced computed tomography and magnetic resonance imaging, basing differential diagnosis on the morphological and temporal characteristics of enhancement, but with the additional unique advantage of the study being done in real-time.
