Book excerpt: These Standard Bidding Documents (SBD) and its companion Technical Note (TN) have been prepared by the World Bank for use by borrowers and their implementing agencies in the procurement of pharmaceuticals, vaccines, and condoms through international competitive bidding (ICB). For the purpose of these documents, pharmaceuticals also include nutritional supplements and oral and injectable hormonal forms of contraception. The procedures and practices presented in these SBD have been developed through broad international experience and are mandatory for use in projects that are financed in whole or in part by the World Bank in accordance with the provisions of the latest edition of Guidelines: Procurement under IBRD Loans and IDA Credits. The purpose of the TN is to provide background information to the Bank's project staff and borrowers, about the complex issues in the procurement of health sector goods and to help them make well-informed decisions in each special situation.

Book excerpt: The Government of India (GoI) under its common minimum program is enhancing central funding to states for effective delivery of essential health services to underserved populations residing in the rural areas of the country. Flexible financing is being provided to states under the National Rural Health Mission (NRHM) to develop locally relevant strategies to improve service delivery and implement evidence-based interventions to enhance health outcomes of centrally sponsored schemes in the health sector. Such rapid scaling-up of central allocations for different health sector schemes will involve massive procurement of health sector goods by the states and districts that have capacity constraints to undertake health sector procurement The NRHM framework notes that continued central procurement is not a solution and proposes a phased decentralization of procurement to states and districts. This note supports the approach of phased decentralization through development of an assessment tool for systematically identifying the procurement capacity constraints at state level.

Pharmaceutical pricing policies need to be carefully planned, carried out, and regularly checked and revised according to changing conditions. Strong, well-thought-out policies can guide well-informed and balanced decisions to achieve affordable access to essential health products. This guideline replaces the 2015 WHO guideline on country pharmaceutical pricing policies, revised to reflect the growing body of literature since the last evidence review in 2010. This update also recognizes country experiences in managing the prices of pharmaceutical products.

These include posting prices, reimbursing for costs, and soliciting competitive bids. Procurement of healthcare services offers several unique challenges. The supplier can influence the quantity of services provided. It is often difficult to even specify in advance exactly what services are to be purchased. Lastly, quality is difficult to measure. Healthcare purchasers have deployed a variety of payment mechanisms to cope with these challenges. We apply the theory of procurement to the case of cataract surgery. We recommend implementing a system that combines a gatekeeper with competitive bidding among operating physicians who must perform all necessary services, including treatment for complications, within a global fee. We conclude by discussing the strengths and limitations of this proposal.
