I have to set out to you such new points in connexion With vaccine therapy as may seem to me to deserve attention. I have to attempt to take the measure of the achievements of this new therapeutic method. I have to discuss its limitations. And, above all, I desire to suggest to you in connexion With all these subject-matters certain canons of criticism. In present circumstances a discussion on influenza will necessarily include consideration, so far as our lack of knowledge will permit, of the conditions leading to the minor epidemics with which in recent years we have been fairly familiar, and those leading to the mysterious occurrence of the wider pandemics, such as those of 1890 and of the present year. It is necessary, in my view, to consider these minor epidemics and the endemic prevalence of influenza in this country in relation to bronchitis and pneumonia, which year by year cause a much heavier toll on health and life than is demanded by influenza. I have little h0pe of success of any personal or public measures against influenza Which will not influence even more favourably the incidence of bronchitis and pneumonia. It is not improbable, in fact, that the influenza problem may be most successfully attacked when influenza is not epidemically prevalent. IN considering to what subjects I Should devote attention in the course of my address to you this evening, it has occurred to me that I could not do better than say a few words on the relation of dental surgery to other branches Of the healing art, and on the very important bearing of dentistry upon the public health. In early times, when the principal work of the dental specialist was the extraction of decayed and Offending teeth, these matters doubtless seemed to Offer but little scope for prolonged discussion. The immediate removal of the disturber of the bodily weal, like the prompt banishment or decapitation by some Eastern tyrant of unpleasantly obtrusive courtiers, was held to be a procedure based on sound surgical principles that must in all cases be adopted. It saved the patient from a continuation of his suffering and its consequences in medical treatment also it afforded the operator who might happen to be fairly skilful an easy escape from all responsibility. What was the cause of decay in the tooth, what the loss of teeth might mean to a patient, and how far his general health had been affected by their disease were not considerations that seemed to invite reflection. The awkwardness of the professional situation when too hasty an application of the forceps had resulted in the drawing of the wrong tooth could always be remedied by the subsequent extraction of the real offender. In the good old days the condition of the teeth of the inhabitants of civilized Europe, so long as it did not entail suffering. His genius so brightly and so fruitfully illuminated medicine that it seems only right for us to meet to commemorate his centenary, chiefly because the object of this Society is the promotion of both the science and the art of medicine, but also because he was one of our foundation members and contributed two papers to the first volume of our Transactions. He was born in the Vicarage at Berkeley in Gloucestershire, on May 17, 1749. He was the third son of Stephen Jenner, the vicar. His mother was the daughter of the Reverend Henry Head who had formerly also been vicar of Berkeley. On both Sides he came of an ancient family. Many of his relations were clergymen; his father had been tutor to one of the Earls of Berkeley and, as we Shall see, this family always had a great regard for Edward Jenner. Soon after his birth, his father died and he was brought up by his eldest brother, the Reverend Stephen Jenner. Your Committee decided in the first place to Obtain what evidence they could from those who were most frequently in the position of having to attempt resuscitation in the apparently drowned. For this purpose they applied, in the first instance, to the police, and, by the kindness of the Chief Surgeon, an inspector and constable of the Thames Police attended at the first meeting of the Committee and gave evidence. They (the river police) make use at the present time of the Silvester method, and the inspector demonstrated the way in which it is applied on the constable and on some of the members Of your Committee. The medical faculty is amazed at the claims made by another body professing faith in science, who are reported to have made astounding cures without the aid Of medicine. This body, hailing from the New World, even counts among its votaries citizens of this country. Sceptics explain away these alleged cures by a theory of auto-suggestion. But in the practice Of medicine itself auto-suggestion is not excluded for among the curative factors we have the personality of the physician, the inherited memory of the medicine man and his magical practices, and the pure effect of drugs as such. This will give us some idea Of the great intricacy of the problem; and the only way in which we can evade the almost insuperable difficulties that confront us, is by taking for our experimental subject an organism whose psychic power is so undeveloped as to be incapable of reacting to a suggestion. Such a rigorous condition is amply fulfilled by the plant, for no one will impute an emotional exuberance to a turnip. Results Of experiments carried out with plants will therefore give us the pure effect of the chemical agent. Therefore, we shall be on somewhat safer ground if before hazarding a new drug on a patient we examine its potency on an inarticulate and uncomplaining piece of vegetable. Such cases are more safely treated by condenser couch diathermy fulguration.1 By this method the amount of destruction can be Observed during the whole of the treatment, and, though its action is limited in depth, by repeated séances extensive growths of considerable depth can be safely removed. We now expect the patient's hips to be raised as a matter of course for all pelvic operations, and those who have always been accustomed to this do not think of what operating without it meant. Very little consideration however is necessary to recognize that, for example, the removal of the uterus for cancer of the cervix by the abdominal route as practised at the present day would be well nigh impossible with the patient on a horizontal table. In spit-e of this convincing proof, the new system, as is the case with every innovation, met with opposition in many quarters; and one of my earliest student recollections is that Of a clinical lecture in which an eminent London surgeon declared that the new method of treatment of wounds had proved in his wards to be a failure. He was, however, soon afterwards convinced that the failure was due to imperfect methods, and became a warm advocate of antiseptic surgery. In our days it has become the only justifiable method Of treating wounds, and its effects have been studied in all directions, in the treatment Of all conditions of disease, with the greatest benefit to suffering humanity. Lord Lister's prophecy in 1869 has been amply fulfilled, when he said: The antiseptic treat ment is continually attracting more and more attention in various parts of the world, and whether in the form which it has now reached, or in some other and more perfect shape, its universal adoption can only be a question of time. The noble institutions of which our country is justly proud, admirably adapted alike for the treatment of the Sick and the instruction of the student, will then be cleared of the only blot that now ittaches to them. They have been cleansed from this blot, and it is impossible to estimate the number of valuable lives that have been saved ince that time by the universal adoption of the antiseptic treatment. Book excerpt: This book focuses on the life and work of Nathan Zuntz (1847-1920), a German physiologist, who made significant contributions to high altitude physiology and aviation medicine. He achieved fame for his invention of the Zuntz-Geppert respiratory apparatus in 1886 and the first treadmill (Laufband) in 1889. He also invented an X-ray apparatus to observe cardiac changes during exercise and constructed a climate chamber to study exercise under varying and sometimes extreme climates. Focuses on Zuntz's contribution to high altitude physiology and aviation medicine
