Book excerpt: Why is there no national health insurance in the United States of America? This question became popular again when President Bill Clinton's Health Security Plan of 1993 proved to be a failure. Throughout the twentieth century, every attempt to enact a national health insurance program failed. The majority of the working population is covered by private, employer-based health insurance, the elderly and welfare poor by the government programs Medicare and Medicaid of 1965, while a growing number of Americans remain uninsured. This study focuses on two important decisions that have shaped American health care policy: the exclusion of national health insurance from the Social Security Act of 1935 and the shift of focus from a health insurance program for the working population to a hospital insurance program for the elderly and the welfare poor. Based on presidential archives and the papers of social security policymakers, this study examines the incremental strategy to achieve health insurance coverage for all Americans. The result is a compelling history of political compromise that will be of interest to both the scholars of the welfare state and the scholars of American ideology and exceptionalism.

Available in PDF, EPUB and Kindle. Book excerpt: A history of U.S. public health emergencies and how we can turn the tide. Despite enormous advances in medical science and public health education over the last century, access to health care remains a dominant issue in American life. U.S. health care is often hailed as the best in the world, yet the public health emergencies of today often echo the public health emergencies of yesterday: consider the Great Influenza Pandemic of 1918–19 and COVID-19, the displacement of the Dust Bowl and the havoc of Hurricane Maria, the Reagan administration’s antipathy toward the AIDS epidemic and the lack of accountability during the water crisis in Flint, Michigan. Spanning the period from the presidency of Woodrow Wilson to that of Donald Trump, American Health Crisis illuminates how—despite the elevation of health care as a human right throughout the world—vulnerable communities in the United States continue to be victimized by structural inequalities across disparate geographies, income levels, and ethnic groups. Martin Halliwell views contemporary public health crises through the lens of historical and cultural revisionings, suturing individual events together into a narrative of calamity that has brought us to our current crisis in health politics. American Health Crisis considers the future of public health in the United States and, presenting a reinvigorated concept of health citizenship, argues that now is the moment to act for lasting change.

Available in PDF, EPUB and Kindle. Book excerpt: The New Deal placed security at the center of American political and economic life by establishing an explicit partnership between the state, economy, and citizens. In America, unlike anywhere else in the world, most people depend overwhelmingly on private health insurance and employee benefits. The astounding rise of this phenomenon from before World War II, however, has been largely overlooked. In this powerful history of the American reliance on employment-based benefits, Jennifer Klein examines the interwoven politics of social provision and labor relations from the 1910s to the 1960s. Through a narrative that connects the commercial life insurance industry, the politics of Social Security, organized labor's quest for economic security, and the evolution of modern health insurance, she shows how the firm-centered welfare system emerged. Moreover, the imperatives of industrial relations, Klein argues, shaped public and private social security. Looking closely at unions and communities, Klein uncovers the wide range of alternative, community-based health plans that had begun to germinate in the 1930s and 1940s but that eventually succumbed to commercial health insurance and pensions. She also illuminates the contests to define "security"--job security, health security, and old age security--following World War II. For All These Rights traces the fate of the New Deal emphasis on social entitlement as the private sector competed with and emulated Roosevelt's Social Security program. Through the story of struggles over health security and old age security, social rights and the welfare state, it traces the fate of New Deal liberalism--as a set of ideas about the state, security, and labor rights--in the 1950s, the 1960s, and beyond.
