Book excerpt: Several recent studies at the U.S. Army Aeromedical Research Laboratory (USAARL) and the U.S. Army Safety Center (USASC) have highlighted the significant contribution of Spatial Disorientation (SD) to helicopter accidents. In the U.S. Army the cost can be approximated at $58M and 14 lives each year. Following some local training initiatives by USAARL and the U.S. Army School of Aviation Medicine (USASAM), the first Triservice Symposium on Spatial Disorientation in Rotary-Wing Operations was held from 24 September 1996 through 26 September 1996 at USASAM. This symposium sought to address three main areas: (1) the seriousness of the SD hazard; (2) current methods to control the hazard; and (3) the associated safety and risk management concerns. This report contains the proceedings of the symposium. The symposium was considered to be a success in raising the awareness of the impact of SD on rotary-wing flying operations in the aeromedical and safety communities of the services. It was clear that SD imposes a particular hazard to rotary wing operations which differs in many respects to that experienced by fixed wing operators. There was unanimous agreement that initiatives to overcome the problem must be made. In order to maintain the impetus established by the symposium and secure funding for the various initiatives, the report contains a memorandum detailing the important factors and makes recommendations for future activity in the area. Work is required in education, training, research, and equipment procurement. Control factors are discussed and recommendations made according to whether the approach should be solely directed towards the U.S. Army, or on a triservice basis.

Accidents and their prevention. A careful reading of the papers presented at the symposium will enable both specialists and decision-makers to make a summary assessment of the constraints of the missions on the one hand, and on the other, of the new methods of protection and training and the new technologies which will enable aircrew to carry out their missions in a safer environment. In addition, they will perhaps discover that helicopter operation can be optimised by a series of different devices and above all by judicious matching of "type of helicopter-type of aircrew training-type of additional on-board equipment".

Available in PDF, EPUB and Kindle. Book excerpt: A survey of 299 aircrew in current flying' practice was performed at five U.S. Army airfields within CONUS. Respondents were asked a number of questions about their worst ever episode of spatial disorientation (SD) and their worst episode in the 4 months prior to the survey. They also were asked to classify episodes as minor, significant, or severe according to the threat to flight safety. Opinions also were sought on various aircraft types and flight conditions. In the final part of the questionnaire, they were asked about their experience of break-off and giant hand phenomena. Analysis of the data showed that 78 percent had suffered SD during their flying career (8 percent had suffered severely enough to put flight safety at risk). Twenty-two percent had suffered in the 4 months prior to completing the questionnaire (2 percent severely enough to put flight safety at risk). In 33 percent of worst ever episodes, the event had affected conduct of the mission while flying accuracy had been affected in 64 percent. (The figures for the 4-month data were 35 percent and 68 percent.) Brownout, whiteout, and inadvertent entry to instrument meteorological conditions (IMC) were the most easily identifiable source of severe episodes of SD, but they accounted for 13 percent of the worst ever episodes. The leans accounted for 44 percent of episodes. Few other episodes could be linked to well known problems. Aircrew were not initially aware of being disoriented in 43 percent of worst ever episodes (38 percent of episodes when they were looking out of the aircraft cockpit). Experience had no apparent protective effect against either the incidence or severity of episodes.

Book excerpt: Over the past fifteen years, the People's Liberation Army Navy's (PLAN's) missile fast-attack craft and amphibious fleets have been significantly modernized. While these two types of vessels have not increased in numbers, their capabilities have increased exponentially. This publication examines the People's Liberation Army's (PLA's) doctrine and training strategy in order to analyze present and predict future missions by these military vessels. China's deterrence posture is improved greatly by these ships and boats, which aid coastal water defense, and threaten Taiwanese attempts to gain independence. In addition, these two fleets improve China's long-range sealift capabilities, and help with the PLA's traditional, and new, nontraditional security practices. These fleets allow the PLAN to continue offshore operations, and begin "blue-water", or "far-seas" operations, helping the United States to predict the future nature of Chinese maritime missions.
