Book excerpt: En base e dadtos recogidos por el Informe de Prevalencia Serologica de la enfermedad de Chagas en el Uruguay, se definieron 4 areas diferentes en ese pais: Region Noresta (11,6% de prevalencia, alta presencia intradomiciliaria de Triatoma infestans e insalubridad de las viviendas); departamentos riberenos del rio Uruguay (prevalencia intermedia del 3,2-2%, presencia peridomicialiaria del vector y mejores viviendas); departamentos del Sur (1% de prevalencia con presencia limitada del vector); y una region libre de vectores -que incluye a la capital del pais-, donde se destaca un 1% de donantes de sangre serologicamente positivos. La transmision congenita se evaluo en el Departamento de Artigas, estudiandose 2303 embarazadas durante 2 anos, de las cuales el 8,5% fueron serologicamente positivas, y en 3 casos se establecio transmision transplacentaria. la OLCEdeCH (Organizacion para la erradicacio de la enfermedad de Chagas), dependiente del Ministerio de Salud Publica del Uruguay, realizo la fase de ataque en cada departamento, en coordinacion con otras instituciones publicas. El objetivo es la lucha quimica contra el Triatoma infestans por medio del rociado con cipermetrina. tambien se desarrollan actividades de educacion sanitaria. La participacion de la poblacion en el planeamiento, organizacion, actividades y evaluacion del programa es decisiva para alcanzar el control de la enfermedad de Chagas. Se encargo a la localidad de Chacras de Constitucion (Departamento de Salto) un proyecto de participacion comunitaria -el que se pretende extender a toda el area rural de Salto-, en dos etapas: control quimico del vector, desarrollado por la comunidad bajo la guia de funcionarios departamentales; y la reparacion y construccion de viviendas, con la asistencia de los gobierno local y nacional, con respaldo de las instituciones educativas. 8 departamentos de area endemica estan incluidos en el programa de erradicacion. El programa de control de transmision sanguinea es efectivo desde 1985, pudiendose estudiar la infeccion chagasica en todos los donantes de sangre del pais. La investigacion serologica de mujeres embarazadas fue incluida recientemente en los protocolos de control del Programa de Salud Materno-Infantil. En el caso de la transmision por sangre y congenita, debe encararse el problema del subdesarrollo de las estructuras de salud. Otro problema es el de la tecnologia disponible, la que debe generarse en los paises del area. Es urgente la necesidad d.
