The transformation of the environment and of the institutional order has created quite a challenge: maintaining some sort of adequacy between these evolving realities and the frames of reference in use by public sector executives. Complexity is often nothing more than a name for a new order calling for a new frame of reference, and the reluctance to abandon old conceptual frameworks is often responsible for fundamental learning disabilities. Through a series of conversations with Canadian federal senior executives about more and more daunting problems - from coping with an evolving context, to engaging intelligently with a new modus operandi, to trying to nudge and tweak programs in order to correct toxic pathologies, to reframing perceptions and redesigning organizations to meet the new challenges—weaknesses of the capabilities of the Canadian federal executives to respond to current challenges were revealed, and suggestions made about ways to kick start a process of refurbishment of these capabilities.

The quest for merit-based bureaucracy stemmed from the idea that good politics could be established through the "development of people"--the training of people to be politically useful. Centuries before civil service examinations emerged in the Western world, these three Asian countries were basing bureaucratic advancement on examinations in addition to patronage. But the evolution of the mandarinates cannot be accommodated by our usual timetables of what is "modern." The history of China, Vietnam, and Korea suggests that the rationalization processes we think of as modern may occur independently of one another and separate from such landmarks as the growth of capitalism or the industrial revolution. However, some groups within these states are distinctly more effective and public oriented than the rest. Why? Patchwork Leviathan explains how a few spectacularly effective state organizations manage to thrive amid general institutional weakness and succeed against impressive odds. Drawing on the Hobbesian image of the state as Leviathan, Erin Metz McDonnell argues that many seemingly weak states actually have a wide range of administrative capacities. Such states are in fact patchworks sewn loosely together from scarce resources into the semblance of unity. McDonnell demonstrates that when the human, cognitive, and material resources of bureaucracy are rare, it is critically important how they are distributed. Too often, scarce bureaucratic resources are scattered throughout the state, yielding little effect. McDonnell reveals how a sufficient concentration of resources clustered within particular pockets of a state can be transformative, enabling distinctively effective organizations to emerge from a sea of ineffectiveness. Patchwork Leviathan offers a comprehensive analysis of successful statecraft in institutionally challenging environments, drawing on cases from contemporary Ghana and Nigeria, mid-twentieth-century Kenya and Brazil, and China in the early twentieth century. De Jong examines the roots of this dysfunction and presents a novel approach to solving it. Drawing from academic literature on bureaucracy and problem solving in the public sector, and the clinical work of the Kafka Brigade—a social enterprise based in the Netherlands dedicated to diagnosing and remedying bureaucratic dysfunction in practice, this study reveals the shortcomings of conventional approaches to bureaucratic reform. The usual methods have failed to diagnose problems, distinguish symptoms, or identify root causes in a comprehensive or satisfactory way. They have also failed to engage clients, professionals, and midlevel managers in understanding and addressing the dysfunction that plagues them. This book offers conceptual frameworks, theoretical insights, and practical lessons for dealing with the problem. It sets a course for rigorous public problem solving to create governments that can be more effective, efficient, equitable, and responsive to social concerns. De Jong argues that successfully remedying bureaucratic dysfunction depends on employing diagnostics capable of distinguishing and dissecting various kinds of dysfunction. The “Anna Karenina principle” applies here: all well functioning bureaucracies are alike; every dysfunctional bureaucracy is dysfunctional in its own way. The author also asserts that the worst dysfunction occurs when multiple organizations share responsibility for a problem, but no single organization is primarily responsible for solving it. This points to a need for creating and reinforcing distributed problem solving capacity focused on deep (cross-)organizational learning and revised accountability structures. Our best approach to dealing with dysfunction may therefore not be top-down regulatory reform, but rather relentless bottom-up and cross-boundary leadership and innovation. Using fourteen clinical cases of bureaucratic dysfunction investigated by the Kafka Brigade, the author demonstrates how a proper process for identifying, defining, diagnosing, and remedying the problem can produce better outcomes.

Five basic parts of the contemporaryorganization (the operating core, the strategic apex, the middle line, thetechnostructure, and the support staff), and five theories of how it functions(i.e., as a system characterized by formal authority, regulated flows, informalcommunication, work constellations, and ad hoc decision processes) aretheorized. Organizations function in complex and varying ways, due to differing flows -including flows of authority, work material, information, and decisionprocesses. These flows depend on the age, size, and environment of theorganization; additionally, technology plays a key role because of itsimportance in structuring the operating core. Finally, design parameters aredescribed - based on the above five basic parts and five theories - that areused as a means of coordination and division of labor in designingorganizational structures, in order to establish stable patterns of behavior.(CJC).

In the last two decades, there has been discussion around the maltreatment of patients, corruption and poor quality of mental health treatment in these institutions. This ethnography scrutinizes the management of madness in one of these asylum-like institutions in the context of national change and the global mental health movement. The author explores the assembling and impact of psychiatric, bureaucratic, gendered and queer narratives in and around the hospital. Finally, the author attempts to reconcile social anthropology and psychiatry by scrutinising their divergent approaches towards ‘mad narratives’.
