Cependant, elle fait le plus souvent l'objet d'une découverte fortuite par le chirurgien-dentiste. Cette détection est importante car plus de la moitié des patients présenteront une anomalie sur leurs dents définitives. Quand la fusion touche les dents permanentes, le motif de consultation des patients est l'aspect inesthétique de la ou des dents atteintes. Les complications liées à cette anomalie sont nombreuses : pathologie carieuse, problèmes parodontaux, malocclusion. Un examen clinique et radiographique rigoureux est indispensable pour diagnostiquer cette anomalie. La prise en charge des dents fusionnées est souvent complexe et pluridisciplinaire. Le principal objectif de ce travail est de mettre l'accent sur la multiplicité des solutions thérapeutiques et d'éclairer le praticien quant au choix du plan de traitement grâce à la présentation de cas cliniques tirés de la littérature et d'un cas clinique personnel que j'ai suivi lors de mon externat au Havre.

Un suivi doit cependant être instauré car dans plus de la moitié des cas, une anomalie est retrouvée au niveau des dents permanentes : agénésie, dent surnuméraire, macrodontie... Au niveau des dents permanentes, la prise en charge est souvent complexe avec une problématique esthétique et fonctionnelle. Les solutions thérapeutiques retrouvées dans la littérature sont multiples et le plus souvent pluridisciplinaires. Il n'existe en effet pas de consensus ou de « guidelines » pour le traitement de la fusion ou de la gémination. Celui-ci associe fréquemment des techniques orthodontiques, chirurgicales, endodontiques, conservatrices et prothétiques.

In this classic of French cultural studies, Georges Didi-Huberman traces the intimate and reciprocal relationship between the disciplines of psychiatry and photography in the late nineteenth century. Focusing on the immense photographic output of the Salpetriere hospital, the notorious Parisian asylum for insane and incurable women, Didi-Huberman shows the crucial role played by photography in the invention of the category of hysteria. Under the direction of the medical teacher and clinician Jean-Martin Charcot, the inmates of Salpetriere identified as hysterics were methodically photographed, providing skeptical colleagues with visual proof of hysteria's specific form. These images, many of which appear in this book, provided the materials for the multivolume album Iconographie photographique de la Salpetriere. As Didi-Huberman shows, these photographs were far from simply objective documentation. The subjects were required to portray their hysterical "type"—they performed their own hysteria. Bribed by the special status they enjoyed in the purgatory of experimentation and threatened with transfer back to the inferno of the incurables, the women patiently posed for the photographs and submitted to presentations of hysterical attacks before the crowds that gathered for Charcot's "Tuesday Lectures." Charcot did not stop at voyeuristic observation. Through techniques such as hypnosis, electroshock therapy, and genital manipulation, he instigated the hysterical symptoms in his patients, eventually giving rise to hatred and resistance on their part. Didi-Huberman follows this path from complicity to antipathy in one of Charcot's favorite "cases," that of Augustine, whose image crops up again and again in the Iconographie. Augustine's virtuosic performance of hysteria ultimately became one of self-sacrifice, seen in pictures of ecstasy, crucifixion, and silent cries.

Since then, the Congresses have met more regularly, making the one of which this is the report, the 9th. The programs of these Congresses have become increasingly rich through the years, with many of the older problems still with us but now studied more dispassionately in the light of new knowledge and new scientific methods in the design of investigations and the vali dation of scientific findings.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
