Book excerpt: L'insomnie est la plainte la plus fréquemment rencontrée à l'officine parmi les troubles du sommeil. Afin de palier à ce trouble, les hypnotiques se présentent comme une solution efficace pour faciliter l'endormissement et maintenir le sommeil. Ils sont ainsi largement prescrits et leur consommation augmente avec l'âge. En effet, la France se positionne au 3ème rang parmi les pays d'Europe les plus consommateurs d'hypnotiques. Cependant, malgré les recommandations émises par les autorités de santé dans le but de promouvoir le bon usage et limiter leurs prescriptions, ils constituent un véritable problème de santé publique quant à leur durée d'utilisation et les effets indésirables qu'ils comportent (somnolence, chute, accident, déclin cognitif, dépendance). Ces effets indésirables peuvent avoir des conséquences préjudiciables notamment chez le sujet âgé à haut risque d'iatrogénie en raison de la polymédication et des modifications physiologiques qui impactent les paramètres pharmacocinétiques. Le pharmacien a donc un rôle déterminant dans la détection des prescriptions inappropriées et le respect du bon usage du médicament. Cette thèse comporte une partie bibliographique suivie d'une étude officinale dont l'objectif est d'évaluer si les prescriptions d'hypnotiques chez les personnes de plus de 65 ans sont appropriées et conformes aux recommandations. Les résultats de cette étude serviront de base pour rappeler les conseils à délivrer lors de la délivrance d'hypnotiques et les alternatives à proposer en première intention pour prendre en charge l'insomnie du sujet âgé.

Chez le sujet âgé, l'insomnie est majoritairement secondaire à une autre pathologie, à la prise de médicaments ou à de mauvaises habitudes de sommeil qu'il convient de traiter conjointement pour obtenir un résultat sur l'insomnie. Malheureusement, on observe une consommation excessive de benzodiazépines anxiolytiques ou hypnotiques avec une balance bénéfice-risque défavorable dans cette population. Face aux nombreux effets indésirables de ces molécules, il est possible de proposer d'autres thérapies comme notamment les thérapies cognitivo-comportementales, la phytothérapie, l'homéopathie ou l'aromathérapie. Il est également primordial de rappeler au patient les règles d'hygiène du sommeil qui peuvent parfois à elles seules traiter une insomnie légère.

De plus, les troubles du sommeil augmentent avec l’âge, la prévalence est plus élevée après 50 ans. Une bonne compréhension de la physiologie du sommeil et des traitements médicamenteux des troubles du sommeil sont indispensables au Pharmacien d’Officine. Ainsi, il a pour devoir d’informer les patients sur les risques et effets indésirables de ces traitements. Et devant un renouvellement de benzodiazépines, il doit être en mesure de fournir tous les renseignements nécessaires pour en diminuer la consommation. En effet, il existe un mésusage de ces traitements qui sont prescrits à tord et sur de trop longues années. Or, cette consommation peut aggraver certaines pathologies sous-jacentes ou encore être la cause d’hospitalisations chez les personnes âgées, suite à une chute par exemple. La place du pharmacien d’officine est également dans le conseil face à une demande spontanée de comptoir. Il peut aussi réaliser des programmes d’éducation thérapeutiques du patient. Le Pharmacien d’Officine a donc un rôle d’informations envers les traitements et leurs effets indésirables. Mais il a, également, un rôle dans l’automédication et les conseils face à une demande de comptoir. Ce dernier doit avoir, par conséquent, un minimum de connaissances sur les thérapeutiques alternatives qui existent, tout en gardant un regard professionnel envers ces différentes thérapeutiques. Il est, avant tout, à l’écoute de son client et bénéficie d’un atout majeur, la proximité avec son client, qui lui confère le premier rôle d’information et de conseil. C’est un professionnel de santé au coeur de la Santé Publique.

Dans ce cadre, 960 entretiens ont été réalisés par les étudiants de 6ème année de la Faculté de Pharmacie de Lille en stage de fin d'études dans les pharmacies d'officine du Nord et du Pas-De-Calais. Ces entretiens pharmaceutiques ciblaient les personnes âgées de plus de 65 ans prenant un traitement contre les troubles du sommeil et sans antécédent de dépression ni de troubles bipolaires. L'objectif de ce travail est de réaliser un état des lieux sur la consommation des hypnotiques chez le sujet âgés et l'identification des facteurs prédictifs d'un souhait d'arrêt du traitement. Cette étude met en évidence, une population âgée fragile qui consomme des hypnotiques de manière trop systématique. On a identifié comme facteurs prédictifs : l'âge, les patients acteurs de leur traitement et les patients actifs au quotidien. Grâce à ces résultats, l'identification des patients capables d'arrêter leur traitement hypnotique est plus simple. Cette thèse vise à donner les clés pour repérer ces patients à l'officine afin que le pharmacien d'officine puisse proposer des interventions personnalisées en collaboration avec d'autres professionnels de santé. Au cours de ces actions, le pharmacien pourra mettre en place et proposer des aides pratiques pour faciliter et valoriser cette démarche d'arrêt afin d'encadrer au mieux le patient.
