C'est un véritable fléau qui ne connait aucune frontière géographique ou sociale. Etant donné l'ampleur de ce phénomène, tout chirurgien-dentiste peut être amené à rencontrer un patient consommateur de cocaïne ou présentant des antécédents de toxicomanie. Par conséquent, et pour assurer une prise en charge efficiente, il est important de connaitre les substances utilisées, leurs effets sur la santé générale et leurs répercussions bucco-dentaires ainsi que les précautions à prendre. L'intérêt de ce travail est d'apporter, à son niveau, les informations nécessaires à l'amélioration des connaissances sur les consommateurs de cocaïne et leurs dépendances, afin d'améliorer leur prise en charge en cabinet dentaire. Après une première partie rappelant la définition des termes relatifs à la toxicomanie, la deuxième partie décrit une présentation épidémiologique, historique, pratique et pharmacologique de la cocaïne. La troisième partie rappelle les complications générales et psychologiques, de la consommation chronique de cocaïne. La quatrième partie aborde les principales répercussions bucco-dentaires liées à cet usage. Enfin, la dernière partie expose les modalités de prise en charge des patients consommateurs de cocaïne par le chirurgien-dentiste.

Mes interrogations théoriques sur l'éthique, le sens du travail, la relation à l'autre que mes cours de philosophie de terminale, mon année de SHS et mes masters de D2 et D3 ont éveillé ont rencontré des problématiques concrètes et vivantes dès lors que j'ai commencé à apprendre mon métier au centre de soins et à l'hôpital. L'éthique est devenue l'éthique médicale, le sens du travail est devenu le sens de ma pratique, la relation à l'autre est devenue la relation de soins. Comment articuler les grands principes et le principe de réalité ? Comment toujours trouver du sens quand on parle rentabilité ? Comment toujours placer le patient au centre de la relation quand celui-ci la refuse ? La dernière étape qui a été décisive pour le choix de mon sujet: le patient en odontologie ; une prise en charge à la croisée du capitalisme et de la philosophie, est la découverte de l'exercice de ma pratique en cabinet dentaire. En effet, j'ai compris à ce moment-là, la dualité que j'avais pressentie tout au long de ma formation. J'ai fait l'expérience de "la structure cabinet dentaire" qui a en fait tout à voir avec une entreprise puisqu'on y parle chiffres, investissements, rentabilité, salaires, profits .... Mais qui n'a simultanément rien à voir avec une entreprise parce qu'on y voit de la souffrance, des quêtes esthétiques parfois infondées, des difficultés financières mais aussi des joies, des familles, de la gratitude en bref de l'humain. J'ai eu à coeur de donner une modeste fécondité à cette dichotomie en cherchant quels étaient les garde-fous qui permettaient que l'équilibre entre exigence de rentabilité et qualité de soins soit respecté et que le patient reste toujours au centre de nos préoccupations et de notre pratique. Pour commencer j'ai tenté de comprendre la sphère économique dans laquelle nous évoluons au travers de l'analyse Marxienne du capitalisme et de l'analyse Wébérienne des structures sociales dans leurs relations historiques à nos attachements culturels et religieux. J'ai ainsi posé le cadre dans lequel il nous faut penser et vivre la relation de soins en odontologie. Fort de ce constat j'ai cherché les garde-fous administratifs : les principes qui les inspirent, les lois qui les garantissent et les faits qui les font exister. Cela m'a semblé plus que louable mais insuffisant pour faire vivre l'exigence d'une pratique performante et humaine en odontologie. En réalité, ce garde-fou administratif trouve son alpha et son oméga dans un garde-fou qui le fonde et le transcende : le garde-fou éthique. Celui-ci a une structure complexe : des principes, une histoire des textes fondateurs mais il a aussi une actualité, une réalité qui ne cessent de se jouer et de se rejouer dans la relation soignant, patient. Arrivé au terme de mon cheminement la très forte demande esthétique en odontologie s'est imposée comme le point névralgique qui cristallisait simultanément la course effrénée au profit de certains praticiens comme le besoin désespéré d'être, de reconnaissance de certains patients. Loin de considérer cela comme un amer constat sur la noirceur de l'âme des chirurgiens-dentistes, je veux y voir une chance pour notre profession. Il nous appartient de "rendre le sourire" à nos patients et par cela de leur permettre de renouer avec leur humanité
