Enfin, comme toute maladie chronique, elle représente un coût éc onomique non négligeable, que ce soit en terme de consultation, hospitalisation, médicaments ou encore terme d'absentéisme scolaire ou professionnel. Face à ce constat et dans le cadre d'une politique de santé préoccupée par la maîtrise des dépenses se développe depuis les années 1990 de nouvelles structures dites "réseaux de soins". Nous proposons à travers ce travail de refaire le point sur la prise en charge du patient asthmatiqu comme définie dans les dernières recommandations de l' ANAES puis nous reprendrons la notion de réseaux de soins en analysant leurs origines, leurs évolutions à travers le 20ème siècle, leurs actualités. Dans un deuxième temps, nous rapporterons une enquête réalisée auprès de médecins généralistes et de patients asthmatiques de la région bordelaise dont l'objectif principal était de préciser certains comportements et certaines notions concernant la maladie, l'éducation, les intervenants, la connaissance ou non des réseaux de soins...l'ensemble étant ensuite comparé aux publications et aux recommandations nationales et internationales. En parallèle, nous transcrirons également les résultats d'une étude des structures de prise en charge de l'asthme déjà existantes en Gironde ainsi que de réseaux de soin "asthme" de référence : le réseau REALIS dans l'Eure. A l'issue, une synthèse de l'ensemble des données sera faite afin de clarifier les bénéfices qu'une structure telle que les réseaux de soins peut apporter à la prise en charge du patient asthmatique.

Malheureusement, d'après plusieurs études, celui-ci est souvent incomplet. L'objectif était de mettre en évidence ces obstacles empêchant les bonnes pratiques de suivi d'un patient asthmatique par un médecin généraliste Méthode : il s'agit d'une étude qualitative par théorisation ancrée menée auprès de médecins généralistes des Côtes-d'Armor par entretiens semi-dirigés Résultats : la saturation des données fut obtenue au bout de 10 entretiens auprès de médecins avec des profils différents. Parmi les multiples obstacles, étaient retenus surtout le manque de temps, présent à tous les stades du suivi, le défaut d'éducation thérapeutique et la difficulté d'acceptation des patients. L'asthme était souvent banalisée par les patients et médecins et était fréquemment un motif de consultation secondaire, évoqué rapidement en fin de consultation. Un défaut de formation était évoqués avec d'anciens schémas décisionnels et de trop nombreuses spécialités médicamenteuses. La pollution atmosphérique n'avait été que peu mentionnée, dû à la patientèle rurale et semi-rurale. Discussion : les techniques de prise médicamenteuse étaient souvent mal comprises de leurs patients et beaucoup se reposaient sur les pharmaciens pour l'enseignement. Une mauvaise éducation initiale compliquait la suite de l'ensemble de la prise en charge. Chez aucun des médecins, les plans d'action écrits n'étaient utilisés chez l'adulte. Les services ambulatoires d'éducation thérapeutique étaient soit absents de la région soit méconnus des médecins.

Book excerpt: De part sa prévalence en augmentation constante depuis vingt ans, ses conséquences en terme de qualité de vie et sa mortalité, l'asthme constitue un véritable problème de santé publique. Après avoir détaillé dans une première partie les différentes caractéristiques de la maladie asthmatique ainsi que la prise en charge thérapeutique, nous avons dans un second temps présenté notre étude réalisée sur 40 patients asthmatiques à l'officine. Cette étude repose à la fois sur un questionnaire et sur l'historique médicamenteux des patients. Elle a pour but d'évaluer le mode de prise en charge et le suivi des patients asthmatiques, de comparer les pratiques médicales aux recommandations officielles, d'évaluer les connaissances et le comportement des asthmatiques face à leur maladie et à leur traitement et d'apprécier le rôle actuel du pharmacien dans leur prise en charge. Les résultats obtenus montrent un contrôle de la maladie chez une majorité des patients et une prise en charge conforme aux recommandations. En revanche, le niveau d'information des asthmatiques sur leur maladie apparaît nettement insuffisant. En effet, plus de deux tiers des patients évaluent leurs connaissances comme faibles à moyennes. De même, seuls 35,7 % des patients interrogés utilisent la mesure du DEP comme outils dans le suivi de leur maladie. Il devient alors urgent que les professionnels de santé réagissent. En effet, l'éducation thérapeutique de l'asthmatique fait partie intégrante de la stratégie de prise en charge et le pharmacien, de part sa relation privilégiée avec le patient, a un rôle tout particulier à jouer dans ce domaine. C'est pourquoi nous avons détaillé dans une troisième partie les différentes actions que peut mener le pharmacien à l'officine, telles que l'aide à la compréhension de la maladie et des traitements, la promotion du bon usage et de l'observance des traitements, l'apprentissage des techniques d'inhalation et de l'auto surveillance afin d'optimiser la prise en charge des patients asthmatiques.

Les propriétés de l’eau thermale, les techniques utilisées et l’encadrement pluridisciplinaire permettent aux curistes de mieux gérer leur maladie au quotidien tout en profitant de l’environnent et du dépaysement de la ville de Saint Honoré les Bains, historiquement la station thermale des voies respiratoires en Bourgogne. Une enquête réalisée auprès de 28 patients asthmatiques en cure à Saint Honoré les Bains a permis d’établir les caractéristiques spécifiques de la population fréquentant l’établissement et de recueillir leurs avis personnels sur cette thérapeutique. Nous avons conclu que les curistes sont en majorité des femmes, généralement en retraite et âgées de plus de 60 ans. Cette cure a été principalement prescrite par le médecin à la vue d’améliorer leur qualité de vie et d’agir en complément et aussi efficacement que leurs médicaments mais sans les effets indésirables.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.

Environmental justice advocates argue that these communities are on the front lines of environmental and health risks. In Noxious New York, Julie Sze analyzes the culture, politics, and history of environmental justice activism in New York City within the larger context of privatization, deregulation, and globalization. She tracks urban planning and environmental health activism in four gritty New York neighborhoods: Brooklyn's Sunset Park and Williamsburg sections, West Harlem, and the South Bronx. In these communities, activism flourished in the 1980s and 1990s in response to economic decay and a concentration of noxious incinerators, solid waste transfer stations, and power plants.
