Book excerpt: Contexte : Depuis quelques années, les remboursements des traitements de substitution aux opiacés (TSO) ont augmenté en France, avec en 2013 entre 160 000 et 180 000 patients. En 2012, 72% des patients ont eu leurs prescriptions en soins primaires. La proportion de médecins prescripteurs représentait en 2009, 50% des médecins généralistes en France et 59,2% pour la région du Nord-Pas-de- Calais. Quelles sont les modalités de prise en charge des patients sous TSO en 2014 en soins primaires dans la région du Nord-Pas-de-Calais ? Méthode : Nous avons réalisé une étude descriptive, transversale par questionnaires, adressés à 450 médecins généralistes installés, en soins primaires, tirés au sort, dans la région du Nord-Pas-de-Calais. 191 questionnaires ont été retournés et analysés statistiquement. Résultats : 55% des médecins généralistes répondants avaient au moins 1 patient sous TSO dans leur patientèle au moment de l'étude, et 82,69 % de ces médecins étaient des hommes. La buprénorphine haut dosage (BHD) était, comme au niveau national, le traitement le plus prescrit : à 65%. Les posologies et durées de traitement correspondaient aux recommandations d'experts, néanmoins 6% des médecins interrogés déclaraient initier un traitement par méthadone au cabinet en ville, ce qui est illégal. Les médecins généralistes de ville restaient réticents concernant une primo-prescription de BHD, celle-ci étant laissée à 71% aux soins secondaires (services spécialisés ou réseaux comme les CSAPA). Une prise en charge conjointe avec des intervenants extérieurs (addictologues, réseaux et CMP) était plébiscitée par les médecins généralistes. La diminution du traitement par TSO était proposée pour 61% des interrogés par le médecin lui-même, en moyenne 7 mois et 24 jours après l'instauration du traitement, par paliers de 0,4 ou 2mg pour la BHD et de 1 ou 5 mg pour la méthadone. Les arrêts de suivi de patient étaient fréquents, puisque 74% des médecins y ont été confrontés. Concernant la formation pour la prise en charge de ces patients, 65 % avaient déclaré avoir une formation insuffisante, néanmoins 68 % ne souhaitaient pas en bénéficier. 87 % des médecins interrogés étaient par contre en faveur d'une formation obligatoire lors des second et troisième cycles des études médicales. Conclusion : Dans notre étude, la proportion de médecins prenant en charge des patients sous TSO était semblable à celle nationale, mais majoritairement en relais du dispositif des soins secondaires primo-prescripteurs quelque soit le TSO. Les services spécialisés et réseaux restaient plébiscités par les médecins, une aide pour la prise en charge et, semble-t-il, en lien avec leur sentiment de manque de formation dans cette spécialité.
