L'apparition des techniques de revascularisation (angioplastie et fibrinolyse) a permis d’améliorer considérablement le pronostic vital à court terme. Mais après la phase aiguë, commence la mise en place des mesures dites de prévention secondaire visant à diminuer le risque de complications et de récidives. Cette prévention secondaire consiste à instaurer un traitement médicamenteux suivant les recommandations (BASIC) et à éduquer le patient afin qu'il suive des règles hygiéno-diététiques (nutrition et activité physique) qui sont essentielles à la réussite du traitement. Une étude a été réalisée auprès de 39 patients victimes d'IDM afin de voir ce qui peut être fait pour optimiser le succès du traitement et voir quelle place a le pharmacien dans cette prise en charge globale.

Book excerpt: La maladie coronarienne reste de nos jours le première cause de mortalité en France, avec 18000 décès par an. Néanmoins, l’amélioration de la prise en charge a diminué ce chiffre par 2 en 10 ans, notamment par des actions de prévention secondaire en sortie de phase aigüe. Depuis quelques années un nouveau mode de prise en charge du patient vise à le placer en tant qu’acteur de sa santé par l’acquisition de connaissances, pratiques et modes de vie le rendant autonome après un accident coronarien. Néanmoins, ces pratiques récentes et nouvelles de prévention secondaire, sont-elles mises en place de manière pertinente et efficace pour diminuer le risque de récidive et de complications du patient ? Le patient, quelques semaines après un infarctus, a-t-il acquis l’ensemble des compétences qui lui permettent de prendre le contrôle de ses facteurs de risque et d’améliorer l’observance de son traitement médicamenteux ? Au cours de cette thèse, nous décrirons dans un premier temps le phénomène d’infarctus du myocarde avec notamment sa physiopathologie, son diagnostic, ses complications et ses traitements. Nous aborderons ensuite la place de la prévention dans la prise en charge de l’infarctus, et de sa récidive. Nous verrons aussi que l’éducation thérapeutique du patient tient une place importante dans la prise en charge de la maladie coronarienne, et qu’elle a un rôle clé de prévention. Enfin, l’étude menée au long de cette thèse fera un état des lieux sur l’impact d’un programme d’ETP mis en place à l’hôpital Albert6Chenevier pour des patients coronariens, en évaluant d’une part leurs connaissances, et en étudiant la manière dont ces informations ont pu leur être transmises.

Book excerpt: La prise en charge du post-infarctus du myocarde avec sus-décalage du segment ST (IDM ST+) repose sur une prévention secondaire bien codifiée. Elle vise, au moyen d'une quadrithérapie médicamenteuse (BASI) validée et une éducation à des règles hygiéno-diététiques stricte, à contrôler l'évolution de la maladie athéromateuse. Les études européennes montrent des écarts parfois importants à ces recommandations. Objectifs : il s'agit d'évaluer le niveau de la prévention secondaire un an après un infarctus du myocarde ST+ en 2012 en Côtes d'Armor, à partir des données de l'observatoire ORBI (Observatoire Régional Breton des Infractus). Le deuxième objectif est d'évaluer sur ce même échantillon, l'impact de la réadaptation cardiovasculaire sur le niveau d'observance et le contrôle des facteurs de risque cardiovasculaires. Matériels et méthode : inclusion de manière prospective de tous les patients hospitalisés pour IDM ST+ entre janvier et décembre 2010 au centre hospitalier de Saint-Brieuc. Les patients décédés durant l'hospitalisation ont été exclu. Les données ont été recueillis au moment de l'hospitalisation dans l'observatoire ORBI, puis par téléphone à un an de l'IDM. Résultats : cent trente neuf patients ont été inclus. Le pourcentage de prescription du traitement recommandé BASI à la sortie de l'hôpital puis à un an était de 58,3% puis 54,6%, pour les antiagrégants plaquettaires de 95% puis 97,5%, pour les statines de 96,4% puis 92,4% pour les bêta-bloquants de 92,8% puis 84%, pour les IEC/ARA II de 64% puis 73,9%.Le taux de patients fumeurs est passé de 36% au moment de l'hospitalisation à 12,6% à un an, le taux de patients en surpoids est passé de 56,1% à 51,3%. 16,7% des 48 patients initialement hypertendus le demeurent à un an. 62,5% des patients diabétiques ont un taux d'HbA1c>6,5% à un an et 31,2% des 93 patients dont le LDL-c a été contrôlé à 12 mois sont au dessus de l'objectif de 1g/L. Les patients ayant bénéficié d'une réadaptation cardiovasculaire sont plus jeunes, ont une FEVG plus basse. Ils reçoivent significativement plus le traitement BASI et ont significativement plus augmenté leur activité physique. Ils ont une tendance plus importante à avoir corriger leurs facteurs de risque cardiovasculaires que ceux qui n'y ont pas séjourné. Conclusion : les résultats sont encourageants car globalement supérieurs à ceux des registres Français et des études européennes mais demeurent perfectibles. La réadaptation cardiaque semble être une étape importante dans la phase de prévention secondaire. L'éducation thérapeutique, primordiale en post-infarctus, pourrait être optimisée en utilisant par exemple d'autres moyens de diffusion des recommandations de prévention secondaire.
