En dépit de l'évolution des lois visant à renforcer la lutte contre ces violences, du développement des structures associatives d'aide aux victimes, et des unités médico-judiciaires (UMJ), force est de constater que les données épidémiologiques restent préoccupantes. L'UMJ du centre hospitalier Bélair étant récente, mon travail a eu pour objectif d'analyser sur un an les dossiers des patientes victimes de violences afin de contribuer à une meilleure prise en charge des victimes. L'UMJ accueille les patients sur réquisition. Notre population totale d'étude, 158 patients consultant pour violences, est constituée d'une très forte majorité de femmes (67 %) dans toutes les tranches d'âge de 5 ans, ainsi que d'une forte proportion de patients âgés de moins de 40 ans. Soixante-quinze femmes ont consulté pour violences conjugales, 2 pour violences intrafamiliales, 26 pour violences commises par un tiers connu ou inconnu. Vingt-et-une femmes ont consulté pour agression sexuelle, dont 14 dans un contexte de violences conjugales. Seulement 6 femmes ont consulté le jour même ; le délai moyen entre les faits et la consultation est de 80 jours, et le délai maximum de 7 ans. Près de 80 % des victimes de violences conjugales déclarent avoir subi plusieurs épisodes de violences avant de consulter et/ou de porter plainte : le même jour jusqu'à une durée de 28 ans, avec une moyenne de 5 ans. Cinquante-huit pour cent des consultations initiales chez un médecin généraliste ont été suivies d'une consultation à l'UMJ. La durée d'ITT du premier certificat n'est mentionnée que dans 5 certificats sur les 40 en cas de violences conjugales. S'agissant des circonstances aggravantes, en matière de violences conjugales, une femme a été agressée avec arme et 2 par plusieurs individus. Huit femmes étaient enceintes et huit présentaient un handicap physique ou mental. Dans 35 dossiers, la présence d'un enfant lors des faits était notée. L'UMJ du centre hospitalier Bélair est déterminante dans le développement du maillage territorial pour la prise en charge des victimes de violences, notamment envers les femmes. La proximité de la prise en charge des patientes est primordiale, car elle contribue notamment à éviter des renoncements à s'engager dans des démarches médico judiciaires. L'UMJ contribue également à développer les liens avec les structures associatives et d'une manière générale une pluridisciplinarité de proximité. Ce travail a permis la création d'une base de données utile à une meilleure connaissance des victimes de violences notamment envers les femmes, et en conséquence à une meilleure prise en charge. Bien que ce travail n'ait pas un objectif épidémiologique, la poursuite de ce travail permettra de mesurer les améliorations de nos prises en charge. L'ensemble des données de ce travail, ainsi que certaines problématiques, telle que la fixation des durées d'ITT, soulignent l'importance de développer l'enseignement médico-légal pour les praticiens, non seulement de l'UMJ, mais également libéraux, et plus généralement pour l'ensemble des acteurs pluridisciplinaires.

Book excerpt: Introduction : Lorsqu'il est face à une femme qui lui a révélé être victime de violences conjugales mais qui ne souhaite pas déposer plainte, le médecin est partagé entre la nécessité de respecter le secret professionnel et celle de protéger une personne en danger. En effet, les violences conjugales, dont la fréquence dans la population générale est mal connue mais probablement importante, sont un facteur de risque de décès. La loi du 30 juillet 2020 a modifié les situations où le professionnel de santé peut signaler au procureur de la République une situation de violences conjugales et a introduit les notions d'emprise et de danger immédiat pour la victime, qui permettent un signalement même sans son accord. Or, il semblait évident que le danger immédiat, dont la définition n'est pas établie par la loi, était un élément que le médecin devait rechercher dans l'évaluation d'une victime de violences, même avant cette loi. Nous avons souhaité savoir si des critères de danger immédiat étaient recherchés par les médecins dans les années qui ont précédé la loi du 30 juillet 2020. Nous avons recueilli ces données dans un service de médecine légale, car les médecins légistes sont au coeur de la prise en charge des victimes de violences. Méthodes : Il s'agit d'une étude rétrospective descriptive incluant les dossiers de femmes majeures victimes de violences conjugales examinées hors réquisition judiciaire à l'Unité Médico-Judiciaire du Centre hospitalier universitaire de Tours, entre le 1er janvier 2017 et le 31 décembre 2019. Une étude de la littérature a permis d'identifier 20 facteurs de risque d'homicide conjugal. Pour chaque dossier, nous avons vérifié que le médecin légiste avait évalué le danger immédiat en recherchant ces critères, et si la consultation judiciaire avait abouti à un signalement judiciaire. Résultats : 99 femmes âgées de 19 à 99 ans ont été incluses dans l'étude. 68% étaient adressées à l'Unité Médio-Judiciaire par une association d'aide aux victimes. Les données non renseignées étaient très importantes. Les facteurs de risque identifiés dans la littérature n'étaient pas précisés dans le dossier dans 51 à 99% des cas, sans que l'on puisse déterminer s'ils n'avaient pas été recherchés ou pas notés dans le dossier. Deux signalements judiciaires ont été effectués au total. Aucune femme n'est décédée. Conclusion : L'évaluation du danger immédiat n'était pas correctement effectuée ou documentée par les médecins légistes. La loi du 30 juillet 2020, si elle ne paraît pas modifier réellement la pratique quotidienne, a permis d'engager la réflexion sur cette évaluation, qui doit à notre sens faire l'objet d'un recueil rigoureux dans le dossier médical. Nous proposons un outil d'évaluation, basé notamment sur le vade-mecum du Conseil national de l'ordre des médecins, afin de standardiser la consultation et dans la mesure du possible, la prise de décision du médecin.

Cet ouvrage parle de succès, d’échecs, d’espoirs, et il cherche avant tout à rendre sensible chaque citoyen à ce qui devrait être une évidence: prévenir la violence domestique et en faire une affaire de santé publique.

Book excerpt: Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?
