Le plus souvent, plusieurs causes d'échec de la prothèse initiale sont identifiées et cette chirurgie itérative pose de nombreuses difficultés techniques. Nous avons choisi de nous intéresser aux faillites mécaniques ayant nécessité le changement d'au moins un des implants. L'objectif de cette étude était double. D'une part démembrer les étiologies nécessitant une reprise après prothèse de genou et donc optimiser leur bilan diagnostique. D'autre part évaluer les résultats des reprises et rechercher des facteurs de risque d'échec. Nous présentons les résultats d'une série rétrospective continue de 56 reprises avec revue des patients à un recul moyen de trois ans. Les patients étaient majoritairement réopérés pour descellement (44% des cas). Suivaient les reprises pour raideurs (27%), les instabilités (16%) et les usures (11%). La fréquence des troubles de rotations a pu être mise en évidence puisqu'il s'agissait du seul défaut de positionnement des implants dans 11% des cas. Ceci suggère l'importance du scanner préopératoire dans le bilan d'une arthroplastie totale du genou avec un mauvais résultat, à fortiori lorsque les radiographies paraissent normales. Cette étude a montré une amélioration notable chez 93% des patients avec une augmentation du score IKS moyen de 80 en préopératoire à 148 en post opératoire. Ce gain était effectif aussi bien en terme de douleurs, de gain de mobilités articulaire que de restauration d'axe du membre inférieur. Néanmoins l'amélioration fonctionnelle était moins importante. Les résultats semblaient moins bons lorsque le patient était obèse, multi opéré ou lorsqu'il n'avait jamais été amélioré par l'arthroplastie initiale. Aucune différence statistiquement significative n'a pu être mise en évidence mais ces données incitent à la prudence en particulier chez ce type de malade. Elles mettent également en évidence la nécessité d'être attentif à certaines erreurs « évitables » lors de la prothèse initiale parmi lesquelles les malpositions rotatoires.
