The striking contrast between the sophistication of diagnosis and the crudeness of preventive surgery forms the basis of historian Ilana Löwy’s important study. Löwy traces the history of prophylactic amputations through a century of preventive treatment and back to a long tradition of surgical management of gynecological problems. In the early twentieth century, surgeons came to believe that removing precancerous lesions—a term difficult to define even today—averted the danger of malignancy. This practice, Löwy finds, later led to surgical interventions for women with a hereditary predisposition to cancer but no detectable disease. Book excerpt: The 2002 National Security Strategy suggested preventive attacks, diplomacy, deterrence, and other policies as means of curtailing threats presented by the spread of nuclear, biological, and chemical (NBC) weapons to terrorists and rogue states. The author analyzes which mix of these policies might best and most cost effectively address the NBC threat, with special focus on preventive attacks. The past performances of preventive attacks, diplomacy, deterrence, and other policies as means of curtailing the NBC threat are analyzed. The central findings are that preventive attacks are generally unsuccessful at delaying the spread of NBC weapons; that deterrence, especially nuclear deterrence, is highly successful at preventing the use of NBC weapons by states; and that diplomacy has had moderate and perhaps unappreciated success at curtailing the spread of NBC weapons. The author also discusses how funds spent on preventive wars, which are much more expensive than diplomacy or deterrence, might be better spent to combat threats from terrorism and proliferation, on initiatives such as fissile material recovery, ballistic missile defense, and port security.

This work seeks to argue that while it was clear that the Bush Doctrine certainly qualified as a preventive war policy, it is apparent that the adoption of this strategy did not mark a total break with American tradition or earlier Administrations. Warren seeks to dispel arguments pertaining to the supposed "radical" nature of the Bush Doctrine – based on comparisons with previous National Security Strategies and previous Administrations’ penchant for prevention. However, the work also highlights that what was new and bold about the Bush Administration’s National Security Strategy of 2002, was its willingness to embrace reinvigorating a nuclear option that could ultimately be used in the context of preventive war. While Obama has struck bold rhetorical notes and promises in relation to limiting the role of nuclear weapons, he has stopped short of changing the status quo on critical issues that have lingered since the Cold War – such as tactical nuclear weapons and keeping missiles on alert. This book’s final section examines the extent to which Obama has attempted to ‘adjust’ the nuclear option with the recent release of the congressionally mandated Nuclear Posture Review (NPR). Offering new insights into the Bush doctrine and providing a comprehensive analysis of the current status of the US nuclear weapons strategy, this volume will be of great interest to scholars and students of American foreign policy, security studies and international relations.
