Available in PDF, EPUB and Kindle. Book excerpt: Many Americans believe that people who lack health insurance somehow get the care they really need. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book DOES MANAGED CARE REDUCE PREVENTABLE HOSPITALIZATIONS IN THE MEDICARE POPULATION THE IMPACT OF MEDICARE ADVANTAGE ON AMBULATORY CARE SENSITIVE HOSPITAL ADMISSIONS AND USE OF PREVENTIVE SERVICES

Download or read book DOES MANAGED CARE REDUCE PREVENTABLE HOSPITALIZATIONS IN THE MEDICARE POPULATION THE IMPACT OF MEDICARE ADVANTAGE ON AMBULATORY CARE SENSITIVE HOSPITAL ADMISSIONS AND USE OF PREVENTIVE SERVICES written by Gregory Berger and published by . This book was released on 2013 with total page 134 pages. Available in PDF, EPUB and Kindle. Book excerpt: One controversial option to control growth in the rate of federal spending on health care is to transform Medicare into a "voucher" program for beneficiaries to purchase health insurance from managed care plans in the private market. Approximately one-quarter of Medicare beneficiaries are currently enrolled in a private health plan through Medicare Advantage, which has higher average costs than the traditional fee-for-service program but potentially reduces costly and unnecessary medical services by increasing the use of preventive care. Using a nationally representative sample of Medicare beneficiaries from the Medical Expenditure Panel Survey (MEPS) from 2006 to 2010, I compared the prevalence of potentially avoidable hospital admissions and use of preventive services for beneficiaries in Medicare Advantage to beneficiaries in the traditional fee-for-service program. Using propensity score estimation to control for observable bias, I found no relationship between enrollment in Medicare Advantage and potentially avoidable hospital admissions, and a significant but small positive relationship with use of select preventive services. Considered together, these findings suggest that Medicare Advantage may not achieve better health outcomes for beneficiaries than the traditional fee-for-service program. These findings have implications for policy discussions surrounding the future expansion of private managed care plans in the Medicare program.

Book Improving the Medicare Market

Download or read book Improving the Medicare Market written by Institute of Medicine and published by National Academies Press. This book was released on 1996-11-01 with total page 384 pages. Available in PDF, EPUB and Kindle. Book excerpt: Medicare beneficiaries are rapidly moving into managed care, as attempts to restrain the growth of this costly entitlement program progress. However, advocates for patients question whether the necessary information and structures are in place to enable Medicare consumers to select wisely among private-sector managed care options. Improving the Medicare Market examines how to give Medicare beneficiaries the same choice of health plan options enjoyed in the private sectorâ€"yet protect them as consumers and patients. This book recommends approaches to ensuring accountability and informed purchasing for Medicare beneficiaries in an environment of broader choice and managed careâ€"how the government should evaluate and approve plans, what role the traditional Medicare program should play, how to help to elderly understand their options, and many other practical matters. The committee discusses the information requirements of Medicare beneficiaries and explores in detail how best to respond to their special needs. And it examines the procedures that should be developed to provide the necessary protections for the elderly in a managed care system.

Book Extending Medicare Coverage for Preventive and Other Services

Download or read book Extending Medicare Coverage for Preventive and Other Services written by Institute of Medicine and published by National Academies Press. This book was released on 2000-04-07 with total page 410 pages. Available in PDF, EPUB and Kindle. Book excerpt: This report, which was developed by an expert committee of the Institute of Medicine, reviews the first three services listed above. It is intended to assist policymakers by providing syntheses of the best evidence available about the effectiveness of these services and by estimating the cost to Medicare of covering them. For each service or condition examined, the committee commissioned a review of the scientific literature that was presented and discussed at a public workshop. As requested by Congress, this report includes explicit estimates only of costs to Medicare, not costs to beneficiaries, their families, or others. It also does not include cost-effectiveness analyses. That is, the extent of the benefits relative to the costs to Medicareâ€"or to society generallyâ€"is not evaluated for the services examined. The method for estimating Medicare costs follows the generic estimation practices of the Congressional Budget Office (CBO). The objective was to provide Congress with estimates that were based on familiar procedures and could be compared readily with earlier and later CBO estimates. For each condition or service, the estimates are intended to suggest the order of magnitude of the costs to Medicare of extending coverage, but the estimates could be considerably higher or lower than what Medicare might actually spend were coverage policies changed. The estimates cover the five-year period 2000-2004. In addition to the conclusions about specific coverage issues, the report examines some broader concerns about the processes for making coverage decisions and about the research and organizational infrastructure for these decisions. It also briefly examines the limits of coverage as a means of improving health services and outcomes and the limits of evidence as a means of resolving policy and ethical questions.
