Nurses caring for patients on mechanical ventilators should be aware of the risk factors and include a plan of action in reducing these factors as part of nursing care. There are many things that can be done to prevent the patients from acquiring VAP or to treat them for VAP. Research has indicated that one of which is to utilize a VAP Care Bundle.

Prevention of ventilator associated pneumonia (VAP) is one of the top priorities for all health care institutions and health care workers should take the responsibility to prevent the infection to provide quality care for patients. This project focus on the causes of VAP in mechanically ventilated patients in Pediatric Intensive Care Unit (PICU) and explores the different prevention strategies. The key objective of this project is to implement standard VAP care bundle to decrease VAP among PICU patients and thus lessen the length of hospital stay, health care cost, mortality and morbidity associated with VAP. The proposed solution is to improve the knowledge level of the multidisciplinary team in PICU through educational preparation there by improve the adherence of standard bundle practice with the supporting evidence of Lewin's change theory. Because of poor immunity in critically ill children, mechanically ventilated patients develop bacterial colonization of oropharynx; stomach and sinus meticulous oral hygiene is essential to prevent aspiration of secretion containing microorganism to lungs causing VAP (Cooper and Haunt, 2013). Different review literature shows VAP bundle practice is the best recommended practice for VAP prevention and the key factor is improving the knowledge level of the multidisciplinary team working in PICU. The occurrence of VAP is a serious health issue which cause adverse effects for the healthcare system and increases the burden on the patients.

More than eighty percent of the worldwide deaths related to sepsis occur in resource-limited settings in low and middle-income countries. Current international sepsis guidelines cannot be implemented without adaptations towards these settings, mainly because of the difference in local resources and a different spectrum of infectious diseases causing sepsis. This prompted members of the Global Intensive Care working group of the European Society of Intensive Care Medicine (ESICM) and the Mahidol-Oxford Tropical Medicine Research Unit (MORU, Bangkok, Thailand) - among which the Editors – to develop with an international group of experts a comprehensive set of recommendations for the management of sepsis in resource-limited settings. Recommendations are based on both current scientific evidence and clinical experience of clinicians working in resource-limited settings. The book includes an overview chapter outlining the current challenges and future directions of sepsis management as well as general recommendations on the structure and organization of intensive care services in resource-limited settings. Specific recommendations on the recognition and management of patients with sepsis and septic shock in these settings are grouped into seven chapters. According to studies, Ventilator Associated Pneumonia (VAP) is one of the leading causes of death in ICUs and it occurs in 9% to 28% of patients treated with mechanical ventilation, and the percentage of mortality rates of this problem are from 24% to 50%' (Berry and others, 2007). There are many studies done to find out about the onset of VAP in mechanically ventilated patients. According to one of the studies, 'Ventilator-associated pneumonia (VAP) occurs within 24 hours of intubation and mechanical ventilation' (Stonecypher, 2010). Due to the assistive-breathing devices and the medications used to keep the mechanically ventilated patients sedated, normal defense mechanisms change, and they become more vulnerable to respiratory infection such as VAP (Pear, 2007). By implementing evidence based oral care into clinical practice, staff members can reduce the cases of VAP and also can improve the quality of care in ICUs. The purpose of this proposal is to implement a comprehensive oral care protocol in critically ill patients such as mechanically ventilated patients in ICU to reduce or prevent the incidences of VAP. Through this proposal the author will also talk about the consequences of inadequate oral care in ventilated patients. The nursing theory, which supports the writer's proposal, is Florence Nightingales environmental theory. A healthy environment is essential for the faster and better healing and recovery according to the Nightingale theory. Evidence based practices and 15 articles containing information about the importance of comprehensive oral care will also supports the author's proposal of reducing or preventing the cases of VAP in ICUs.
