Available in PDF, EPUB and Kindle. Book excerpt: 'Venous thromboembolism (VTE) is the umbrella term covering deep vein thrombosis, pulmonary embolism and a group of associated chronic conditions. This vascular disease process is a common, yet serious adverse complication of hospitalisation that results in significant mortality, morbidity, and healthcare resource expenditure. VTE in hospitalised patients is preventable and there is a robust evidence base supporting the use of prophylactic therapies for at-risk patients. Unfortunately, despite the evidence, research and clinical audit reveal that these therapies are frequently underutilised or inconsistently applied. The substantial VTE prevention evidence-practice gap has been identified internationally as a priority patient safety issue. Implementation science is a relatively new field of research focused on closing evidence-practice gaps by translating research findings into routine clinical practice. This PhD thesis contains five publications from a linked series of four implementation science studies aimed at improving the uptake of research evidence on VTE prevention in hospitalised patients. The studies were conducted at St Vincent&‟s Private Hospital, a 270 bed acute care facility in Sydney, Australia.' - (Abstract)

Book excerpt: Each year, over 25,000 people in England die from venous thromboembolism (VTE) (caused by a blood clot dislodged from its site of origin in a vein) contracted in hospital, more than twenty-five times the number who die from MRSA. Unfortunately, although many of these deaths are preventable, a safe and cost effective method of preventing VTE is not being as widely administered as it should be. The Committee's report examines the scale of the problem and its causes; the availability of prophylaxis and current guidelines; lack of professional awareness of the problem and funding issues; education and risk assessment needs; and consistency in implementation of the guidelines. Recommendations made include: that the guidelines on preventative measures which have been commissioned from the National Institute of Clinical Excellence (due in May 2007) should be extended to cover the majority of hospital patients, not just surgical patients; and that all patients, on admission to hospital, should be counselled about the risk of VTE.
