In recent years, however, economic incentives have become more and more important in health care, even in welfare states such as Sweden. In a decade and a half, Sweden has experimented with different health care modalities. As far as we know, relatively little research has been done to assess how these financial incentives, over time, have affected Sweden and the views of physicians. This work analyzes the implications of these reforms on the future structure of the well-envied health care of the Swedes.

Book excerpt: Health care reforms around the world—from Europe and North America to Africa, Latin America and Asia—seem to all be market-oriented reforms driven by international business interests and right wing political parties. There seems to be a sudden and broad concern with the efficiency of medical care, with the assertion that democratically or professionally run systems are inherently inefficient. Far less concern is evident for the more traditional values held regarding medical care, effectiveness (or quality) and equity. The fact is that we have little good cross-national research that systematically addresses the reform issue. This book addresses that problem, and attempts to look at health care reforms in a number of countries, representing as wide a spectrum as possible, and using a common conceptual framework that allows for comparable information to be gathered and presented on each, despite differing levels of socio-economic development. The authors agreed on a set of models that were thought to provide reasonable guidance in answering the questions of the source of pressures for reform, the alternative modes of organization that have been found in the world in recent years, and the direction of change among those alternatives.

When the states themselves accept the fact that increasing private participation in health care in the form of health sector reforms is happening because of deliberate state policy, they will be better positioned to take decisions which ensure effective private sector regulation and universal access to quality health care within a democratic or participatory framework of governance. Increasing privatisation of health care has also led toward citizens losing their democratic spaces with regard to decisions on universal access to quality health care. This has wider implications, not only for improved health status in general, but also about how we live in a modern society with our values of liberty, equality and justice intact. The book would be a useful guide for policy-makers, researchers, students and layman across the world.

Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Globalisation, technological development and dissemination, political volatility, patterns of consumption, and employee expectations are occurring at a rate which is hard to measure. The rate of change in these spheres is far outstripping the rate of organisational innovation in both European enterprises and public governance, leading to a serious mismatch between the challenges of the 21st Century and the organisational competence available to deal with them. In this context, there is no clear roadmap. The contributors to this volume address these issues and demonstrate that building the knowledge base required by actors in this volatile environment requires continuous dialogue and learning – a context in which social partners, regional policy makers and other participants share diverse knowledge and reflect on experience rather than seeking and imitating any notion of ‘best practice’.
