The new statute included two related insurance programs to finance substantial portions of the hospital and physician expenses incurred by Americans over the age of sixty-five. Public attempts to improve American health standards have typically precipitated bitter debate, even as the issue has shifted from the professional and legal status of physicians to the availability of hospital care and public health programs. In The Politics of Medicare, Marmor helps the reader understand Medicare's origins, and he interprets the history of the program and explores what happened to Medicare politically as it turned from a legislative act in the mid-1960s to a major program of American government in the three decades since. This is a vibrant study of an important piece of legislation that asks and answers several questions: How could the American political system yield a policy that simultaneously appeased anti-governmental biases and used the federal government to provide a major entitlement? How was the American Medical Association legally overcome yet placated enough to participate in the program? And how did the Medicare law emerge so enlarged from earlier proposals that themselves had caused so much controversy? Theodore R. Marmor teaches politics and public policy in Yale University's management and law schools as well as in its political science department. Book excerpt: President Clinton has proposed allowing people ages 62 through 64 to buy into Medicare if they do not have access to employer-sponsored or federal health insurance. Displaced workers ages 55 through 61 generally would also be allowed to buy in if they lost employer-sponsored insurance. In addition, retirees ages 55 and over whose former employers terminated or substantially reduced retiree health insurance would be permitted to extend their COBRA coverage (defined on page 6) until age 65. The proposals have been introduced as H.R. 3470 (Representative Stark) and S. 1789 (Senator Moynihan). The Presidentâ€TMs proposals would help people in these age groups obtain health insurance. While most of them already are covered by employment-based, government, or private non-group plans, nearly one in seven is uninsured. Many without coverage find that private non-group insurance is too expensive or, if they have pre-existing medical conditions, unavailable. However, the cost of buying into Medicare (estimated to be $300 to $400 a month) or continuing COBRA coverage may also exceed what most uninsured can afford. Questions have been raised about whether Medicare buy-ins would result in costs to the federal government and whether Medicare should be expanded in light of the anticipated insolvency of the Part A trust fund. Some argue that legislative action should be deferred until the issue is considered by the National Bipartisan Commission on the Future of Medicare. Proponents argue that the proposal is largely self-funded and is needed now to address a growing problem.

The President stated that the discount card program would be an interim measure until a broader Medicare prescription drug benefit for seniors can be created. Many seniors do not have adequate prescription drug coverage. In 1998, an estimated 10 million elderly people, or 27% of Medicare beneficiaries, did not have any form of prescription drug coverage. The proposed card program would be similar to prescription drug discount card programs that are currently available from a number of sources. The Administration's card would endorse and promote a number of qualified privately-administered prescription drug discount card plans at a maximum enrollment rate of $25 per plan. Since the announcement of President Bush's proposal, several pharmaceutical companies announced that they would offer their own senior discount card plans for low-income seniors beginning in early 2002. The Administration has stated that the Medicare-endorsed card plans would offer discounts in the range of 10% to 25% on retail prescription drug prices.
