Book excerpt: INTRODUCTION : Les pathologies thrombotiques sont une problématique de médecine générale de plus en plus fréquente et particulièrement dans la population âgée de 75 ans et plus. En effet la fibrillation auriculaire concerne dans deux tiers des cas des patients de plus de 75. Depuis quelques années les indications des anticoagulants oraux directs ont été élargies en raison d'un meilleur profil de sécurité et d'efficacité comparé aux antagonistes de la vitamine K. Cependant les patients de plus de 75 ans sont sous-représentés dans les études alors que représentant la majorité des patients traités. MATÉRIEL ET MÉTHODE : Ce travail a permis de décrire le suivi médical de 133 patients de plus de 75 ans, actuellement traités par un anticoagulant oral, appartenant à la patientèle de 7 médecins généralistes installés en secteur semi-rural. L'ensemble des éléments relatifs à ces traitements ont été recensés, notamment les modifications thérapeutiques et effets secondaires notables. Des entretiens semi-dirigés auprès des mêmes médecins ont permis de recueillir leur perception sur ces traitements. RÉSULTATS : Nous avons recensé 17 événements thrombo-emboliques sous traitement anticoagulant oral, et dans près de deux tiers des cas (64,7%) sous anticoagulant oral direct. 35 saignements significatifs sont survenus, dont 16 graves, et majoritairement sous antagonistes de la vitamine K (60%). Les médecins généralistes déclarent une préférence pour les anticoagulants oraux directs quand le choix est possible, ce qui représente 93,1% des primo-prescription de traitement anticoagulant oral. CONCLUSION :L'utilisation des anticoagulants oraux directs est de plus en plus intégrée dans la pratique courante des médecins généralistes. Ce type de traitement présente un meilleur profil de sécurité mais une moins bonne efficacité en vie réelle comparativement aux antagonistes de la vitamine K. Chez ces patients âgés, le risque hémorragique est la principale préoccupation, conduisant à des adaptations posologiques. Le dépistage plus fréquent de la fragilité permettrait sans doute d'optimiser la prise en charge de ces patients et d'obtenir une meilleure efficacité, donc une réduction des événements thrombo-emboliques.

Méthode : Nous avons mené une étude descriptive observationnelle rétrospective à partir du registre bourguignon de la Mutualité Sociale Agricole. Etaient incluses, les PA d'âge > 75 ans ayant eu une ordonnance remboursée entre le 1er et le 30/9/2017. Les 2 groupes étaient comparés pour l'âge, le genre, la déclaration d'une affection de longue durée (ALD), le nombre de molécules par ordonnance, les coprescriptions, les consultations spécialisées.
