Book excerpt: In response to the terrorism incidents of September 2001, the Office of the Assistant Secretary of Planning and Evaluation, Department of Health and Human Services, requested that the Centers for Disease Control and Prevention's (CDC) National Center for Health Statistics (NCHS) survey hospitals regarding their preparedness for treating patients from bioterrorism attacks or mass casualty incidents. It provided funding to field a special supplement to the 2003 National Hospital Ambulatory Medical Care Survey (NHAMCS). Data from the 2003 NHAMCS supplement will serve as a baseline for hospital terrorism preparedness because there were no major funding programs directed toward hospitals for this purpose prior to the start of the Bioterrorism Hospital Preparedness Program of the Health Resources and Services Administration (HRSA) in fiscal year (FY) 2002. Even these funds had not yet been fully awarded to hospitals at the time this survey was fielded in 2003. Prior to the inception of the HRSA program, the Domestic Preparedness Program of the Department of Defense had not allocated any funding for hospitals, although it funded emergency response agencies.

Terrorism experts have always agreed the probability of such an attack was low but added it was not a matter of if or when, but where the attack will take place. There have been several incidents throughout the world that clearly demonstrate the different forms of biological terrorism that exist and how they are used at the will of a terrorist. Terrorism experts estimate only 25 percent of hospitals are at some level of preparedness and therefore able to respond to a bioterrorism incident. A prepared facility will be better prepared to deal with unknown outcomes with greater confidence in the preparedness of the community. Health care organizations cannot delay in the development and implementation of disaster management plans in preparing for a bioterrorism event. This project evaluates the bioterrorism response capabilities at one of Houston's only two level-one trauma centers using a bioterrorism preparedness survey. The results indicate areas that must be improved or will have a profound impact on the readiness posture of the hospital. The project concludes with recommendations that should be addressed first. The facilities senior leadership should discuss all of the survey results to ensure the highest level of preparedness.

Book excerpt: Over the past decade, acts of international and domestic terrorism have demonstrated to government officials and policy makers the urgency of preparing systems to support the detection of atypical health events and the provision of preventive and interventional medical services in mass-care events. The 1995 bombing of the Alfred P. Murrah Federal Building in Oklahoma City resulted in 168 deaths and required the efforts of nearly 5,500 emergency responders dispatched to the scene over 17 days in chaotic and feverish attempts to rescue and facilitate subsequent medical and surgical interventions to save the lives of over 600 injured victims. The September 11, 2001, attacks on the World Trade Center and the Pentagon resulted in over 3,000 deaths and nearly 2,400 injured. The lethality of the attacks averted strain on hospitals but, once again, required the coordinated efforts of emergency responders and medical providers. The anthrax attacks of 2001 resulted in relatively few victims. On March 11, 2004, terrorist bombings on several trains in Madrid resulted in nearly 200 deaths and about 1,800 injured.5 Madrid's hospitals were swamped with casualties as the hospitals appealed urgently for blood donations. Acts of terrorism upon civilian populations necessitate a robust response by health and medical care systems and present particular challenges to the United States' almost 5,000 acute care hospitals. As the locus of medical diagnosis and intervention for a wide array of routine activities ranging from the provision of primary care for the uninsured to the delivery of tertiary procedures for life-threatening diseases, hospitals face the quandary of terrorism preparedness and response with limited budgets and insufficient numbers of trained human resources. These needs are even more acutely experienced by the nation's approximately 2,000 rural hospitals, which have a comparatively smaller repertoire of medical resources yet face unique vulnerabilities. The spectrum of weapons of mass destruction demands a diverse set of response capabilities. The use of an explosive device necessitates an urgent and massive medical effort to save the lives of injured persons not immediately killed. Conversely, the deployment of a biological agent in most instances unleashes a communicable pathogen, the required response to which grows greater over time as the incidence of human infection increases. The resulting epidemic is workforce consuming, requiring voluminous amounts of human resources and medical equipment to interrupt disease transmission and reduce rates of mortality through disease tracking and monitoring, verification of exposure, administration of medical countermeasures where available (for example, vaccines and preventive antibiotics), and supportive care and clinical treatment. The case studies described herein offer a view of the challenges facing rural hospitals to meet these demands as they prepare for the threat of bioterrorism as well as non-intentional, naturally occurring epidemics of emerging contagious diseases such as severe acute respiratory syndrome (SARS). The findings are sobering and include a pervasive perception among study participants that major medical emergencies in America's rural areas and heartland would quickly overwhelm the capabilities of their small hospitals. A catastrophic event, or an act of bioterrorism, will require that rural hospitals receive outside assistance to sustain ongoing operations, intervene in the potentially unremitting flow of medical emergencies, and contain the epidemic sequelae of a deliberately deployed pathogen. The delivery of medical care to infected populations and the containment of disease epidemics require that hospitals occupy a central role in community-based bioterrorism preparedness planning. The author provides this report to inform future initiatives to prepare America's hospitals against threats to homeland security.

The planning and response to disasters and catastrophes needs to take into consideration the distinction between progressive and sudden events. Insidious or slowly progressive disasters produce a large number of victims but over a prolonged time period, with different peaks in the severity of patients presenting to the hospital. For example, radiation events will produce a large number of victims who will present days, weeks, months, or years after exposure, depending on the dose of radiation received. The spread of a biological agent or a pandemic will produce an extremely high number of victims who will present to hospitals during days to weeks after the initial event, depending on the agent and progression of symptoms. On the other hand, in a sudden disaster, there is an abrupt surge of victims resulting from an event such as an explosion or a chemical release. After the sarin gas attack in a Tokyo subway in 1995, a total of 5500 victims were injured and required medical attention at local hospitals immediately after the attack. The car bomb that exploded near the American Embassy in Nairobi, Kenya, killed 213 people and simultaneously produced 4044 injured patients, many requiring medical care at local hospitals. The Madrid train bombing in March 2004 produced more than 2000 injured victims in minutes, overwhelming the city’s healthcare facilities. More than 500 injured patients were treated at local hospital after the mass shooting in Las Vegas. Finally, earthquakes may produce a large number of victims in areas in which the medical facilities are partially or completely destroyed. Sudden events bring an immediate operational challenge to community healthcare systems, many of which are already operating at or above capacity. The pre-hospital as well as hospital planning and response to sudden mass casualty incidents (SMCI’s) is extremely challenging and requires a standard and protocol driven approach. Many textbooks have been published on Disaster Medicine; although they may serve as an excellent reference, they do not provide a rapid, practical approach for management of SMCI’s. The first edition of “Mass Casualty Incidents: The Nuts and Bolts of Preparedness and Response for Acute Disasters” dealt exclusively with sudden mass casualty incidents. The second edition will expand its focus and include planning and response for insidious and protracted disasters as well. This new book is designed to provide a practical and operational approach to planning, response and medical management of sudden as well as slow progressive events. The target audience of the second edition will be health care professionals and institutions, as well as allied organizations, which respond to disasters and mass casualty incidents. Parts I and II are essentially the first edition of the book and consist of planning of personnel, logistic support, transport of patients and equipment and response algorithms. These 2 parts will be revised and updated and include lessons learned from major mass shootings that occurred recently in the United States and other parts of the world Part III will describe the planning process for progressive disasters and include response algorithms and checklists. Part IV will handle humanitarian and mental health problems commonly encountered in disaster areas. Part V will deal with team work and communication both critical topics when handling catastrophes and mass casualty incidents. This new book will be a comprehensive tool for healthcare professionals and managers and should perform demonstrably better in sales and downloads. It will be of value at the pre-hospital as well as the hospital level, to plan and respond to the majority of catastrophes and mass casualty incidents.

In a three-day workshop, convened by the Institute of Medicine's Forum on Emerging Infections, experts from each of these communities came together to identify, clarify, and prioritize the next steps that need to be taken in order to prepare and strengthen bioterrorism response capabilities. From the discussions, it became clear that of utmost urgency is the need to cast the issue of a response in an appropriate framework in order to attract the attention of Congress and the public in order to garner sufficient and sustainable support for such initiatives. No matter how the issue is cast, numerous workshop participants agreed that there are many gaps in the public health infrastructure and countermeasure capabilities that must be prioritized and addressed in order to assure a rapid and effective response to another bioterrorist attack.
