Book excerpt: Contexte : L'une des missions du médecin généraliste est d'accompagner les jeunes filles dans leur puberté et début de vie sexuelle, notamment à travers la première consultation contraception. Objectifs : Explorer les ressentis du médecin généraliste concernant le déroulement de cette consultation : quel est son rôle ? quels sont les principaux freins et obstacles au bon déroulement de cette consultation ? quelles sont les pistes d'amélioration possibles ? Méthode : Etude qualitative par entretiens individuels semi-dirigés, auprès de 14 médecins généralistes installés dans le Morbihan avec retranscription puis analyse thématique. Résultats : Tous les médecins pensaient avoir un rôle essentiel à jouer, se sentaient concernés par ce sujet et dans l'ensemble étaient à l'aise dans cette consultation. Les principaux freins et obstacles dégagés côté médecin étaient parfois la présence parentale, le manque de formation ou de supports pédagogiques. Côté jeune fille, il s'agirait de la peur de subir un examen gynécologique, la peur d'informer ses parents d'une demande de contraception et le manque de connaissances préalables. Les pistes d'amélioration possibles évoquées étaient la délivrance d'une information qualitative collective à l'école (comme prévu dans la loi) et la création d'une consultation obligatoire « sexualité-contraception » pour filles et garçons. Conclusion : Le renforcement de l'information des jeunes filles en amont de cette consultation paraît indispensable et faciliterait le travail des médecins pour mener une consultation personnalisée plus qualitative, répondant mieux aux besoins sociétaux d'accompagnement des jeunes.

La couverture contraceptive de ces jeunes filles reste insuffisante. Malgré la multiplicité des méthodes contraceptives et la mise en place de mesures spécifiques pour faciliter l'accès à la contraception, le taux d'IVG reste globalement stable. Une consultation de contraception et prévention, dédiée à la première prescription de contraception, a été prévue pour les mineures. Ce travail a pour but d'évaluer l'impact de la revalorisation de la cotation de cette consultation depuis le 1er novembre 2017 sur la pratique des médecins généralistes. Méthode : une étude épidémiologique observationnelle a été réalisée de janvier à mars 2019 chez les médecins généralistes installés en libéral dans les départements du Nord et du Pas-de-Calais à l'aide d'un questionnaire mis en ligne. Book excerpt: Contexte - Depuis 2016, de nouvelles mesures visent à faciliter l'accès des mineures à une contraception et depuis 2017, une nouvelle Consultation Contraception et Prévention a été créée, peu protocolisée et peu évaluée. Objectif - Connaître les attentes des mineures concernant cette nouvelle consultation afin d'adapter celle-ci à leurs besoins. Méthodes - Etude qualitative avec 25 entretiens semi-dirigés. Echantillon raisonné d'adolescentes lycéennes du Val d'Oise. Approche interprétative phénoménologique. Résultats - Ces nouvelles mesures semblent utiles aux adolescentes mais peu connues. Une offre d'information adaptée, privilégiant une information présentielle ainsi qu'un retour d'expérience, est souhaitée. Le médecin en charge de la consultation devra faire preuve d'une communication adaptée à la mineure, sans jugement. Loin d'une prise en charge type, cette consultation devra être totalement personnalisée et ouvrir un panel de thématiques, comme la contraception mais aussi les maladies sexuellement transmissibles, la sexualité, l'identité sexuelle ou les violences, pour offrir une consultation préventive diversifiée. La jeune fille devra être rassurée, mais aussi savoir se protéger de divers risques et prendre part de manière active à la gestion de sa santé. Vectrice d'autonomisation, cette consultation doit s'inscrire dans un cadre plus large et être proposée au public masculin, dans une optique égalitaire hommes-femmes. Conclusion - Si ces mesures semblent favoriser l'accès à la contraception chez les mineures, la formation des acteurs de soins concernés doit être améliorée et l'information diffusée de manière adaptée afin que son utilisation soit pleinement profitable.

Book excerpt: Introduction : Le taux d'IVG a diminué juste après la loi de 1975 sur la légalisation de la contraception, mais depuis plusieurs décennies son taux stagne malgré plusieurs actions des pouvoirs publiques pour y remédier. Dans ce contexte nous avons souhaité cibler les attentes des jeunes filles lors de le leur première consultation pour une demande de contraception afin peut-être trouver des explications à ce problème. Matériels et Méthodes : Nous avons réalisé une enquête qualitative dans le Jura, dans des cabinets de médecins généraliste, au moyen d'entretiens semi-dirigés, auprès de jeunes filles venant ou étant venues récemment chez leur médecin généraliste pour une première demande de contraception. Nous avons ensuite retranscrit les entretiens et avons effectué une double analyse du verbatim à l'aide du logiciel Nvivo. Nous avons obtenu une saturation des données au bout de treize entretiens. Résultats : Nous avons constaté que les jeunes filles avaient de nombreuses connaissances sur le sujet de la contraception mais qu'elles étaient superficielles. Elles souhaitaient donc des informations précises, notamment sur les effets secondaires des différentes méthodes contraceptives, sur leur fonctionnement et bien sûr savoir quoi faire en cas d'oubli. Elles souhaitaient que la consultation se fasse dans un climat de confiance et de rassurance par leur médecin. D'ailleurs, elles attendaient de lui qu'il soit pédagogue, à leur écoute afin de pouvoir discuter librement, qu'il ne les juge pas et qu'il respecte le secret médical. Par rapport à la place des parents lors de cette consultation, même si elles étaient plusieurs à ne pas souhaiter leur présence, celles qui y étaient favorables avaient l'espoir que leur médecin de famille soit un médiateur auprès de leurs parents et qu'il permette de briser le tabou de ce sujet au sein de leur famille en répondant aux questions et inquiétudes de leurs parents. Conclusion : Notre étude a mis en évidence l'importance du médecin généraliste dans la prévention et l'information des jeunes filles au sujet de la contraception et de l'éducation sexuelle. Néanmoins il intervient sans doute trop tard dans la vie sexuelle des jeunes filles qui pour la moitié ont déjà eu un rapport sexuel avant leur première consultation de contraception.

Book excerpt: Mitochondrial replacement techniques (MRTs) are designed to prevent the transmission of mitochondrial DNA (mtDNA) diseases from mother to child. While MRTs, if effective, could satisfy a desire of women seeking to have a genetically related child without the risk of passing on mtDNA disease, the technique raises significant ethical and social issues. It would create offspring who have genetic material from two women, something never sanctioned in humans, and would create mitochondrial changes that could be heritable (in female offspring), and therefore passed on in perpetuity. The manipulation would be performed on eggs or embryos, would affect every cell of the resulting individual, and once carried out this genetic manipulation is not reversible. Mitochondrial Replacement Techniques considers the implications of manipulating mitochondrial content both in children born to women as a result of participating in these studies and in descendants of any female offspring. This study examines the ethical and social issues related to MRTs, outlines principles that would provide a framework and foundation for oversight of MRTs, and develops recommendations to inform the Food and Drug Administration's consideration of investigational new drug applications.

In considering the book's conclusions about the impact of population growth on development, four caveats should be noted. First, the effects of population growth vary from place to place and over time. Thus, blanket statements about overall effects often cannot be made. Where possible, the authors note the contexts in which population effects are strongest and weakest. Second, all of the outcomes examined in this book are influenced by factors other than population growth. Moreover, the impact of population growth may itself vary according to the presence or absence of other factors. This again makes bl anket statements about the effects of population growth difficult. Throughout the chapters, the authors try to identify other relevant factors that influence the outcomes we discuss or that influence the impact of population growth on those outcomes.
