Book excerpt: Clinical hematologists are frequently faced with the intriguing problem of agnogenic myelo dysplastic syndromes; these syndromes are peculiar in their failing or poor response to therapy and in that they frequently terminate in acute leukemia. An international symposium held in Innsbruck in 1978 was dedicated to clinical and pathophysiologic aspects of the preleukemic syndromes. This monograph consists of articles that correspond to the papers presented at the symposium and of the edited discussions. It was the aim of the symposium and of this monograph to outline comprehensively the pathophysiologic background of the preleukemic syndromes and to summarize the present information about their clinical management. The first part of the monograph covers that pathology of these disorders: Disturbed maturation and proliferation of the hemopoietic cells are the causes of cyto penias, of structural and ultrastructural anomalies, and of enzymatic and functional defects. Further articles are concerned with the evaluation of these features and of chromosome anomalies as possible tools for the diagnostic and prognostic classification of myelodysplasias. Book excerpt: During the past decade, there have been numerous direct and indirect scientific contributions to both the etiology and therapy of aplastic anemia and related bone marrow failure syndromes. Clinical observations, such as autologous bone marrow recovery after conditioning with immunosup pressive agents for bone marrow transplantation; failure to achieve en graftment in some identical twins without prior immunosuppressive ther apy; and hematologic response to immunosuppressive agents, have led to the concept of immune-mediated etiology of acquired aplastic anemia. Such a concept was further strengthened by laboratory findings, implicat ing the role of activated cytotoxic T lymphocytes and abnormal produc tion of inhibitory lymphokines. The immunologic mechanisms may also apply to the idiosyncratic bone marrow aplasias associated with drugs, toxic chemicals, and viruses. These agents may alter normal cellular recog nition sites by interacting with cellular components and result in loss of self tolerance. Immunologic mechanisms have long been advocated in many other organ failures, and the hemopoietic organ is no exception. It is of interest that parallel clinical and laboratory investigations in juvenile diabetes mellitus type I and in rodent models of this disease have yielded results compatible with the same pathogenic mechanisms. The infiltration of pancreatic islets by activated T lymphocytes, functional and morphological alterations of islet cells upon incubation with lymphokines such as gamma interferon and tumor necrosis factor, and clinical response to cyclosporine are a few examples.

Book excerpt: The cloning of hemopoietic cells in semisolid medium began some 12 years ago, and when colonies of T = and B = lymphocytes were grown by several groups last year, the last major subclasses of hemopoietic cells had been successfully cultured in vitro. The time seemed appropriate to write a short general account of this exciting new area of hematology, emphasizing particularly the potential value of these new techniques for direct studies on patients with leukemia and allied diseases. I had hoped to refer to, or at least record, every paper published in this field, but the literature proved to be larger than I had realized, and furthermore, some areas were so active that chapters had to be rewritten over a period of less than a year. I regret that ·what follows is an incomplete account and no doubt, by the time this book becomes available, the material will be even more out of date. However, I have described the basic techniques in some detail and have tried, where possible, to explain how and why the techniques can fail. It is already possible to see patterns, perhaps even principles, emerging from this work which may well be applicable to other cell populations. These have been emphasized in the text in an attempt to make this review as creative as possible.
