Given the surge in the rate of sexually transmitted diseases including HIV, this sheds light as a new worry for sexually active older adults, raising questions about risk behaviors among this group. The issues facing older adults are three-fold in that they are 1) not identifying themselves as being at risk; 2) not identifying risk behaviors; and 3) are not largely being identified in the public realm as participating in risk behaviors. For these reasons, understanding both environmental and individual factors and their unique influences on HIV risk behaviors among older adults is very important. A study was undertaken to develop an understanding of the environmental and individual population factors that predict HIV risk behaviors among older adults using population- based data from the Behavioral Risk Factor Surveillance System and eight state level data sources, guided by Andersen's Behavioral Model of Health Utilization (2007). The model investigated the predictive ability of social and environmental factors in addition to individual factors on HIV risk behaviors. Methods: The research design for the study was a multilevel repeated cross-sectional design with a three level nested structure. The software package MLwiN was used to conduct the multilevel analysis using the Deviance Information Criterion (DIC) analytical method to assess the fit of models with Markov chain Monte Carlo (MCMC) estimation. Using a nationally representative sample of 23,950 individuals from 2008 to 2011, nested in 51 states, the trend study allowed for an understanding of the state and individual-level factors that predict participating in HIV risk behaviors. Results: In each step, the model fit improved the DIC statistic. Overall, the results indicated that there were differences between states and their levels of HIV risk behaviors. While HIV risk behaviors decreased from 2008 to 2011, there was significant variation between and within states in the probability of individual HIV risk behaviors. States that had a higher percentage white females, a higher percentage of persons uninsured, a higher percentage of whites with AIDS, a lower percentage of Asians with AIDS, and a lower percentage of Other races with AIDS had a higher probability of individuals participating in HIV risk behaviors. All of the individual predictors of HIV risk behaviors were statistically significant. Conclusion: With the increase in older adults living with HIV or becoming diagnosed or infected later in life, it is paramount that HIV risk behaviors are examined in this population. Results indicated that states that had more white females, more people who are uninsured, and whites, Asians, and other races living with AIDS were environments that had higher levels of HIV risk behaviors. Recommendations for future research are made, with education, policy, and practice implications discussed.

This thesis research project aims to add to the literature by examining the factors that influence HIV sexual risk among older adults and the prevention programs that exist to address the problem among adults and older adults. This thesis research project is guided by the following research questions: 1) what factors are associated with HIV sexual risk among individuals aged 50 and over? and 2) what effect do individual-level programs, in comparison to group-level and multi-level programs, have on HIV sexual risk among adults and older adults? The results indicated that risky sexual behaviors among older adults were correlated with substance abuse, relationship factors that involved partner-related risk and self-silencing behaviors, mental health, low HIV risk perceptions, and lack of HIV knowledge. Additionally, the literature revealed that prevention program types, such as: motivational interviewing, behavioral skills-training, counseling, HIV/AIDS education, and condom skills-training were effective in increasing condom-use and condom-negotiation skills among adults and older adults.

Part of this growing proportion is attributed to contracting the disease later in life. Older adults continue to date and have sex well into their 80s. Online dating has gained popularity with adults age 50 and older as a means for finding potential romantic partners. There is a gap in the science related to what older adults who date online know about HIV prevention as well as the factors that might put them at risk for contracting the disease.Purpose: This study explored the feasibility of conducting online research regarding HIV prevention factors (i.e., risk perception, sexual risk behaviors, and self-efficacy to take action to prevent HIV) in older adult online daters. Also, this research sought to modify and test instruments related to HIV prevention factors in an older adult population.Methods: A cross-sectional, descriptive correlational design was employed. Adults age 50 and older who had used online dating sites within the past year were recruited online to complete a survey. The study instrument included HIV prevention scale items that had undergone two rounds of expert panel review. Based on the expert panel review, the HIV prevention scale items overall were comprised of (a) original items, (b) modified items, and (c) new items from both the Condom Use Self-Efficacy Scale and Sexual Risks Scale. The data were analyzed using descriptive and correlational statistics. The 31 items from the HIV prevention scale underwent exploratory factor analysis and confirmatory factor analysis (CFA) to determine factor structure and model fit.Results: A total of N = 280 older adults participated in the study. SurveyMonkey Audience and Facebook were the best recruitment sites in terms of numbers of participants recruited. Participants were not able to be recruited from online dating sites. Most of the participants (72.8%) desired a relationship that included sex and 64.6% (n = 181) had at least one sexual relationship in the past year. Of the sexually active participants, only 29% used a condom with every sexual encounter, 45% never used a condom, and 26% had inconsistent condom use. Participants risky sexual behavior and low perceived susceptibility to HIV were incongruous with their agreement that they would take precautions to reduce their risk of contracting HIV. Higher self-efficacy had moderate to strong associations with the intention to take action to reduce the risk of HIV. In CFA, the HIV prevention items loaded on eight different factors in three different models. The factors for Model 1 (Mechanics, Advocacy, Intoxicants), Model 2 (Attitudes, Normative Beliefs, Perceived Susceptibility), and Model 3 (Intention, Expectations) had adequate to excellent model fit. Conclusions: This study was the first to examine HIV prevention factors in older online daters as well as attempt to adapt instruments that measure HIV prevention factors in older adults. Older adults are dating online and have risky behaviors related to sex. Conducting an online survey examining HIV prevention factors in older online daters was both feasible and acceptable. The finding of participants with high levels of sexual risk behavior but low levels of condom use and low HIV risk perception suggests the critical need to target older online daters for HIV prevention interventions. The HIV prevention items were found to be appropriate for an older adult population.

Book excerpt: Author's abstract: In 2009, there were 155,700 individuals coming of age in ten years or less to receive Medicare benefits who were HIV positive. There is a lack of studies that investigate HIV/AIDS in the aging population from a health policy standpoint. Methods: A non-experimental descriptive quantitative study was conducted utilizing data from the CDC's 2012 Behavioral Risk Factor Surveillance System (BRFSS) survey to identify factors that may be related to the participation in risky behaviors associated with the transmission of HIV among Georgia residents. The BRFSS question inquiring about the participation in HIV risk behaviors associated with HIV transmission served as the outcome variable while the exposures of interest were extracted from the questions regarding age, race, gender, income level, mental health morbidity, health care coverage status, and health care costs. Crude odds ratios, confidence intervals, and p-values were calculated to evaluate the associations between the categorical exposures of interest and the outcome variable. Multivariable logistic regression was used to determine relationships between the continuous exposures of interest and the outcome variable. Results: The relationship between the participation in HIV risk behaviors and mental health morbidity among the population of interest was found to be statistically significant. Discussion: Findings suggest that there is a lack of statistical power of BRFSS to discuss HIV/AIDS among aging Georgians in that the survey tool was under-representative of the larger population.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.
