Book excerpt: The percentage of Canadian children and adolescents that are overweight or obese has increased dramatically since the 1980s, with approximately 25% of youth in Canada now characterised as overweight or obese. Canadian youth are engaging in multiple risk-taking behaviours, some of which are associated with an increased risk of overweight and obesity. The school environment has the potential to influence student body mass index (BMI) through implementation of comprehensive healthy school policies, programs, and practices in the domains of physical activity and healthy eating. Guided by a socio-ecological framework, this dissertation research aimed to examine: (1) how modifiable risk behaviours cluster into unique behavioural patterns in youth, and how these behavioural patterns are related to BMI; (2) the effect of engaging in unique clusters of risky behaviours on youths' BMI trajectories; and, (3) the effect of modifying obesity-related school policies, programs, and practices on youths' BMI trajectories. Three manuscripts addressed these objectives using linked student- and school-level data from the COMPASS Study (COMPASS). The first manuscript represents a cross-sectional analysis (2012/13), while the second and third manuscripts were longitudinal (2012/13 to 2014/15) in nature. The first manuscript assessed the prevalence and clustering of 15 modifiable risk behaviours using latent class analysis in a sample of 18,587 youth in grades 9 to 12. Four distinct classes emerged: Traditional School Athletes, Inactive Screenagers, Health Conscious, and Moderately Active Substance Users. Youth belonging to the Traditional School Athlete, Inactive Screenager, and Moderately Active Substance User clusters were all significantly more likely to be overweight or obese, compared to the Health Conscious group. The second manuscript examined the effect of engaging in the four clusters of risky behaviours at baseline on youths' BMI trajectories, using a linked longitudinal sample of 5,084 students in Grades 9 and 10. Using linear mixed effects models, results identified a significant difference in BMIs only at baseline in the four clusters; despite these differences, BMI increased across all clusters annually by the same amount. The third manuscript examined the effect of modifying physical activity and nutrition-related programs, policies, and practices on BMI trajectories using a linked longitudinal sample of 4,951 students in Grades 9 and 10 attending 41 COMPASS schools. Between 2012/13 and 2013/14, 26 of 41 schools implemented distinct new programs or policies, none of which used a Comprehensive School Health Approach. Results indicate that none of these school modifications were associated with improved or reduced BMI trajectories. Findings of this dissertation research have implications for future public health and school-based interventions, and highlight the need for future research in this area, particularly focused on a Comprehensive School Health approach to obesity prevention. COMPASS is uniquely positioned to evaluate similar naturally occurring school-based interventions in a cost-effective and efficient, yet scientifically robust manner by following the same students as they progress through school and are exposed to a changing school environment.

First, the study predicted that Latino adolescents have higher BMI compared to Non-Latino adolescents. Second, the study predicted that Latino adolescents living in rural areas have higher BMI compared to Latino adolescents living in urban areas. Lastly, the study predicted that overweight and obesity are more prevalent in Latino adolescents compared to Non-Latino adolescents. The Statistical Package for Social Services (SPSS) was utilized to analyze the 2011-2012 California Health Interview Survey (CHIS) data using t test and Chi Square statistics. The results of this study showed a significant relationship between the incidence of overweight and obesity among the Latino adolescent population.

Book excerpt: Background: Of children and teenagers aged 6-19, 16% (almost 9 million) were obese (i.e. Body Mass Index at or above the 95th percentile for children of the same age and sex) and this number has continued to increase. Increased understanding of proper nutrition and the need for physical activity among adolescents will likely prevent health problems associated with overweight and obesity, and improve quality of life into adulthood. In particular, when considering determinants of obesity responsive to change, a focus on modifiable, prevalent, and relevant behaviors among adolescents should be taken. Purpose: The purpose of this study was to examine the extent to which constructs of the Theory of Planned Behavior (TPB) (behavioral intent, attitudes, social norms, and perceived behavioral control) predicted four behaviors linked to childhood obesity in middle school children: daily physical activity, sedentary behaviors, fruit and vegetable intake, and water versus sweetened beverages consumption. In addition, a valid and reliable instrument which adequately reified the constructs of the TPB in relation to the aforementioned behaviors was developed. Methods: A cross-sectional design was implemented to obtain a convenience sample of 318 middle school students. Prior to the administration of the instrument to the entire student population, factor analysis and reliability estimates were conducted with a sub-group of students. The 129-item instrument was administered during health, physical education, and art classes. Multiple regression and structural equation modeling were used to establish the predictors for each behavior. Results: Almost 25 % of the students were considered overweight or obese. Students did not meet the recommended five servings of fruit and vegetables per day (M = 3.67); participated in more than the 1-2 hours recommended screen time (M = 187.42 minutes); and had an increased risk of obesity with their sweetened beverage consumption (M = 1.59 glasses) per day. The mean behavioral intention scores for the behaviors were as follows: screen time behaviors (M = 12.31, SD = 5.42); physical activity (M = 12.84, SD = 7.18); fruit and vegetable consumption (M = 12.18, SD = 5.74); sweetened beverages versus water consumption (M = 12.42, SD = 6.07) - denoting a moderate intent to participate in the behavior. Book excerpt: In Canada, the prevalence of adolescent overweight and obesity has increased significantly and currently accounts for about 20% of the adolescent population in the 12 to 17 year old age group. Adolescents with a higher weight status experience a host of physical, social and psychological challenges and have been shown to have an increased likelihood of engaging in various high-risk behaviors. However, the relationship between adolescent overweight and obesity and risky sexual behavior has received scant attention in the literature. Studies have shown that there is a significant association between obesity and lower levels of self-esteem as well as body image satisfaction. In turn, a number of studies have demonstrated that lower levels of self-esteem and body image satisfaction are predictors of risky sexual behavior. Accordingly, the aim of this study was to unveil any direct associations between adolescent overweight and obesity and risky sexual behavior, and explore the possibility of body image satisfaction and self-esteem as psychological pathways. Data from the 2009/2010 cycle of the Canadian Community Health Survey (CCHS) were used, where a total of 8 168 participants between the ages of 15 to 19 were included. The results of this study showed that obese female adolescents were less likely to have ever had sex, both in the 15 to 17 year old age group as well as the 18 to 19 year old age group. In the 15 to 17 year old age group, overweight females were more likely to be diagnosed with a sexually transmitted infection (STI) and in the 18 to 19 year old age group, obese females were less likely to have used a condom the last time they had sex and less likely to have used birth control in the past 12 months. Overweight and obese adolescents had significantly lower levels of body image satisfaction in both age groups, while obese adolescents experienced lower levels of self-esteem in the 15 to 17 year old age group. Implications for intervention and prevention strategies are discussed and recommendations for future studies provided.

Book excerpt: Abstract Background: Recent evidence suggests perceptions of overweight account for the psychosocial consequences typically associated with obesity. Previous research indicates the presence of an obesity achievement gap, yet limited research has explored weight perception in association with academic achievement. Previous studies have focused on grades and degree attainment, without consideration of student aspirations and perceived support and ability to achieve higher levels of education. This thesis examined how Body Mass Index (BMI) classification and weight perception relate to academic performance and postsecondary aspirations and expectations in a large cohort of Canadian adolescents. Additionally, the interaction between BMI status and perceptions of weight was examined in relation to academic achievement outcomes. Methods: Two-year survey data from 25,673 grade 9-12 students attending the 122 Canadian schools that participated in Year 6 (2017/2018) and Year 7 (2018/2019) of the COMPASS study were used. Generalized estimating equation models were used to examine associations between students' BMI classification and weight perception and their math and English/French course grades and post-secondary academic aspirations and expectations. All models were stratified by gender and adjusted for sociodemographic variables and school clustering. Results: Boys and girls with BMI of obesity and missing BMI classification reported lower grades and post-secondary aspirations and expectations when compared to those with Normal BMI. Similarly, boys and girls with overweight BMI reported lower math and language grades than those with Normal-weight BMIs. Relative to their peers with normal-weight BMI and "about right" perceptions, those with overweight perceptions and BMI of overweight/obesity reported lower academic grades and post-secondary aspirations and expectations. There was evidence of an additive effect for girls and boys with overweight perceptions and BMI of overweight/obesity on academic outcomes. About right perceptions of weight were protective against lower math grades for boys and girls with overweight/obesity BMI. Results varied by gender and across academic outcomes. Conclusions: Overall, this thesis demonstrates that an obesity achievement gap remains when controlling for students' perceptions of their weight. Perceptions of overweight had a detrimental effect on academic performance and aspirations/expectations for students with BMI classifications of overweight and obesity, as well as grade outcomes for those with BMI of normal-weight. Results suggest that barriers to academic success exist for students with larger bodies. Future studies should explore the role of internalized and externalized weight bias.

It is estimated that 5-7 percent of White and Black children are obese, while 12 percent of Hispanic boys and 19 percent of Hispanic girls are obese. Some data indicate that obesity among children is on the increase. The second National Children and Youth Fitness Study found 6-9 year olds to have thicker skinfolds than their counterparts in the 1960s. During the same period, others documented a 54 percent increase in the prevalence of obesity among 6-11 year olds. This book examines leading research in this area of great concern to society.
