Book excerpt: Objective: Substantial increases in symptoms of anxiety and depression have been reported among the general public during the coronavirus disease 2019 (COVID-19) pandemic. Prior to the pandemic, individual risk factors for emotional disorders have been identified by a considerable body of research. However, COVID-19 has brought exposure to a variety of new stressors, including risk of infection and stress and isolation related to infection control procedures. Well studied transdiagnostic vulnerability factors as well as novel risk factors for anxiety and depression need to be examined in this context. This study utilized machine learning regularization and variable selection methods to identify key individual risk factors for anxiety and depressive symptoms over the course of the pandemic. Methods: Adults in the United States (N = 1,200) completed seven online self-report assessments over a 4.5-month period (data collected April 24-October 3, 2020). Anxiety and depressive symptoms were assessed at each time point using the Generalized Anxiety Disorder Scale-7 and the Quick Inventory of Depressive Symptomatology. Cumulative symptom severity across the assessment period was calculated using area under the curve (AUC). A machine learning approach to elastic net regularized regression was used to select predictors of 1) cumulative anxiety severity and 2) cumulative depression severity among a set of 68 sociodemographic, psychological, and pandemic-related baseline variables. Results: Using established cut scores at the initial assessment, nearly half of participants met criteria for probable generalized anxiety disorder (n = 564, 47.0%) and a quarter met criteria for probable major depressive disorder (n = 306, 25.5%). For cumulative anxiety severity, 52 selected baseline variables predicted 79.7% of the variance (predicted R2). Cumulative anxiety severity was most strongly explained by perceived stress, depressive symptom reactivity, and brooding. For cumulative depression severity, the combination of 46 selected baseline variables predicted 76.2% of the variance (predicted R2). Comorbid generalized anxiety, health anxiety, and poor sleep contributed most strongly to predicting depression. There were considerable similarities in the risk factors identified across models. Depressive symptom reactivity, full-time employment, and living with someone age 60+ were among the most important predictors of greater symptom severity in both models. Important psychological risk factors that represent potential targets for interventions include depressive symptom reactivity, brooding, instrumental social support, and hopelessness. Discussion: Many established risk factors for anxiety and depression emerged as important in the context of the pandemic, including greater stress and comorbid psychopathology. Contrary to pre-pandemic research, full-time employment and being married/cohabiting were related to greater risk of anxiety and depression. Several coping strategies which may be adaptive in other contexts, such as use of instrumental social support, were associated with symptoms over the study period. Characteristics associated with COVID-19 risk (e.g., living with someone age 60+, having a chronic health condition) also emerged as risk factors for anxiety and depression. Findings suggest that although many risk factors for internalizing psychopathology appear to generalize to the pandemic, differential relationships and novel contributors need to be considered. Findings highlight risk factors for internalizing psychopathology during the pandemic and suggest possible treatment targets for psychological interventions.

Book excerpt: The COVID-19 pandemic has led to increased levels of anxiety and depression worldwide. The current study examined predictors of anxiety and depression as well as longitudinal changes in distress at two times points during the pandemic in the United States. We also looked at disparities between White and Non-White participants. We found Non-White participants scored higher on all dependent measures than White participants in May 2020, indicating greater distress. Further, significant depression predictors for the whole sample were COVID-19 diagnosis, previous depression, and younger age, and health anxiety predictors were previous anxiety diagnosis and no COVID-19 diagnosis. Avoidant coping predicted state anxiety and depression; more time spent watching or reading COVID-19 related news predicted all three outcome variables; intolerance of uncertainty predicted state anxiety and health anxiety, and looming cognitive style predicted health anxiety. Spiritual well-being significantly mitigated all three types of symptoms. Finally, we found differences in predictors between White and non-White participants, such that for all three dependent measures there were more significant predictors for White participants, however there was substantial overlap in predictors. We further examined the impact of cognitive vulnerability factors and avoidant coping on anxiety and depression longitudinally. After testing for temporal directionality between looming cognitive style and intolerance of uncertainty, we found the best model fit and a significant serial mediation for looming cognitive style at time one leading to increased intolerance of uncertainty at time two leading to more avoidant coping resulting in increased anxiety and depression at time two. Depression scores significantly increased over time and moderation analyses revealed that when people had lower levels of depression at time one and used less avoidant coping, they had significantly less symptoms at time two; and when people had higher levels of depression at time one and used less approach coping, they had significantly higher levels of depression at time two. Results suggest a novel model in conceptualizing anxiety and depression during the pandemic, namely highlighting looming cognitive style as an underlying cognitive vulnerability factor and the impact of coping on distress. Further implications include understanding predictors and mitigating factors of psychological symptoms during COVID-19 to inform psychological treatment of depression and anxiety. Moreover, understanding the differences in predictors of anxiety and depression in White versus Non-White groups in creating treatment plans.

Insomnia is an important sleep phenomenon that predicts development of depressive and anxiety disorders, though mechanisms explaining this relationship remain unclear. Distress tolerance, the ability to withstand aversive affective experiences, is a transdiagnostic construct that may influence this relationship. It was hypothesized that greater psychological distress would predict more severe insomnia. Further, it was hypothesized that more severe insomnia and poorer distress tolerance would be associated with greater depression and anxiety. Depressive and anxiety symptoms were hypothesized to predict diagnoses of Major Depressive Disorder (MDD) and Generalized Anxiety Disorder (GAD). Lastly, distress tolerance was hypothesized to mediate the relationship between insomnia and diagnosis of MDD and GAD. Data were obtained from a prospective observational study with three nested samples. The first sample was 5,547 college students who completed a survey study in March-April 2020, the second was 615 college students who completed a survey study in September-October 2020, and the final sample was 100 college students recruited for a clinical interview study between October 2020 and April 2021. Measures included the Kessler Screening Scale for Psychological Distress, Insomnia Severity Index, Distress Tolerance Scale, Patient Health Questionnaire-9, PROMIS Emotional Distress Short Form - Anxiety, and the Structured Clinical Interview for DSM-5 Disorders. Additionally, COVID-19 has been linked to both depressive and anxious symptomatology, along with increased feelings of distress (Fitzpatrick et al., 2020). Because disordered eating is particularly prevalent among college-aged women, this study sought to determine how depression, anxiety, and stress affect eating behaviors of college women in the context of the COVID-19 pandemic. 179 women at Butler University, aged 18-24, gave informed consent before completing a questionnaire pertaining to their demographics, their stress surrounding COVID, and their weight change since March 2020. The next set of questionnaires asked about their anxiety, stress, and depression, as well as their eating behaviors, first at the time they completed the survey (September-December 2021) and next during the COVID-19 lockdown period (March-August 2020). Depression, anxiety, and stress were significantly higher during COVID-19 than during the fall of 2021, but college-aged women reported more restraint in their eating at the time of the study. No overall differences emerged in uncontrolled or emotional eating across the two time points. In correlational analyses, depression and anxiety during COVID-19 correlated with both uncontrolled and emotional eating (depression also correlated with cognitive restraint). Although change in self-reported stress levels across time did not predict changes in disordered eating, improvements in depression and anxiety from Spring 2020 to Fall 2021 correlated with less emotional eating across time. Book excerpt: Purpose: To examine how perception of community respect, care, safety and acceptance relate to feelings of depression among adults living in the U.S. during COVID-19 and determine which sources of community predict favorable perceptions of community. Methods: Bi-variate correlations were conducted examining the relationship of depression with perception of community items and overall perception of community. A binary logistic regression analysis was used to determine which sources of community significantly predicted group membership. Results: Feelings of depression were significantly related to perception of community respect, and safety and acceptance. Only extended family and neighborhood community predicted favorable perceptions of community. Discussion and Implications: Interventions which communicate associated perception items for depression are recommended. Engagement with extended family and neighborhood communities may promote mental well-being as the odds of having a favorable perception of community is higher within these two sectors.

Methods: Seventy-six patients with advanced lung cancer previously accrued for a cohort study (Clinicaltrials.gov Identifier: NCT03199651) completed measures of cancer-related stress, COVID-19 related stress, and depressive and anxiety symptoms from April 2020 – July 2020 during their routine follow ups.
