Seuls 39% des libéraux réalisant la consultation post-hospitalisation pratiquent une titration des traitements. Une inertie dans l'optimisation et la titration des traitements s'observe. 33% des interrogés ne connaissaient pas l'UTIC (défaut du lien ville-hôpital). L'étude confirme que l'amélioration des campagnes de d'information des professionnels médicaux paraît nécessaire dans le secteur, comme l'optimisation thérapeutique. Les filières de soins organisées peuvent être une réponse hospitalière. Une prise en charge ambulatoire pluridisciplinaire locale, organisée semble également être bénéfique, utilisant la télémédecine comme socle de fonctionnement.

Book excerpt: L'éducation thérapeutique est devenue un des fondements du traitement de toute maladie chronique, notamment de l'insuffisance cardiaque. En effet, il s'agit d'une pathologie fréquente et responsable de nombreuses hospitalisations pouvant être évitées pour certaines. L'éducation thérapeutique a pour but d'autonomiser le patient dans la gestion de sa maladie (traitement, signes de décompensation cardiaque, régime hyposodé, activité physique). Mais pour cela, sont requises certaines compétences propres à l'éducation thérapeutique : une formation adéquate du personnel, des outils de support, du temps à consacrer, et éventuellement un lieu dédié. Notre travail a consisté à suivre pendant 6 mois une cohorte de 40 patients : 20 patients éduqués à l'insuffisance cardiaque et 20 patients non éduqués. Après avoir comparé les connaissances des 2 groupes concernant leur maladie, nous avons étudié différents paramètres tels que, entre autres, le nombre de consultations chez le médecin traitant, chez le cardiologue, le nombre d'hospitalisations, les motifs de consultations et d'hospitalisations, les modifications de traitement. Notre étude a permis, outre de montrer l'intérêt évident de l'éducation thérapeutique, de définir les difficultés rencontrées dans ce projet à tous les niveaux (patients, professionnels de santé, financements, matériel, etc) dans la pratique quotidienne médicale de l'éducation thérapeutique.

Book excerpt: L'insuffisance cardiaque est une pathologie chronique et lourde, touchant de plus en plus de patients. Chaque année, des centaines de milliers d'hospitalisations pour décompensation sont observées, à l'origine d'un coût important pour la société. L'éducation thérapeutique, processus centré sur le patient, est une discipline récente destinée à aider le patient à acquérir et maintenir les compétences dont il a besoin pour gérer au mieux sa vie avec sa maladie chronique. Son application en cardiologie en est à ses débuts et déjà ses bénéfices en terme de réduction du nombre d'hospitalisations pour décompensation, d'amélioration de l'observance et de la qualité de vie, ont été démontrés dans de nombreuses études françaises ou étrangères. Cependant, ces études n'impliquent que rarement le pharmacien et encore moins l'officinal, les piliers actuels de l'éducation thérapeutique étant les médecins et infirmières. Les études disponibles montrent l'intérêt de la participation du pharmacien aux projets d'éducation thérapeutique notamment au travers de la pharmacie clinique mais aussi grâce à la mise en valeur de l'observance, le conseil téléphonique et les visites à domicile. Le rôle du pharmacien est malgré tout mal défini et pourrait également s'étendre à d'autres domaines tels que les règles hygiéno-diététiques et à l'information sur l'OTC. Pour appréhender le rôle de l'officinal dans la prise en charge thérapeutique du patient insuffisant cardiaque en Franche-Comté et son éventuel rôle dans l'éducation thérapeutique, une étude basée sur des questionnaires distribués aux pharmaciens d'officine d'une part, et aux patients adhérents à une association de malades d'autre part, a été réalisée. Les taux de participation ont été respectivement 37 % et 48 % pour les pharmaciens et les patients. Les résultats émanant des pharmaciens ont montré une implication déjà importante dans l'éducation thérapeutique (37 %) surtout dans l'éducation au traitement et à la maladie et l'aide à l'observance. L'éducation thérapeutique à l'officine est déjà une réalité. Il existe cependant des difficultés face aux patients à cause d'une formation universitaire sur l'insuffisance cardiaque jugée plutôt insatisfaisante (57 %) et une formation continue délaissée sur ce thème (37 % de participation), malgré un niveau de connaissance relativement bon. Les résultats concernant les patients, population âgée à prédominance masculine, traduisent leurs attentes vis à vis de leur pharmacien. Ils le considèrent sans conteste comme le spécialiste du médicament mais éprouvent des difficultés à voir en lui un référent concernant le suivi et la surveillance médicale. Une petite majorité méconnaît l'éducation thérapeutique démontrant les efforts restant à fournir pour promouvoir la discipline. Malgré les obstacles, les patients font confiance au pharmacien pour la réaliser et certains seraient même prêts à tenter l'expérience. L'éducation thérapeutique à l'officine est appelée à se développer devant le nombre croissant de patients, constituant un nouveau défi pour le pharmacien. Le décloisonnement ville/hôpital ou l'appartenance à un réseau seraient des éléments favorables à cette pratique de même que la formation à l'éducation thérapeutique dans la formation initiale des pharmaciens.

L'objectif de cette thèse était de décrire la prise en charge ambulatoire des patients insuffisants cardiaques chroniques et de vérifier son adéquation avec les données actuelles de la science. Après avoir rappelé les données épidémiologiques, quelques définitions et les étiologies de l'insuffisance cardiaque, les recommandations sur l'approche diagnostique et la stratégie thérapeutique actuelles sont exposées. Afin de connaître les pratiques des médecins généralistes concernant les patients insuffisants cardiaques chroniques, une enquête a été réalisée auprès des médecins généralistes des départements du Val de Marne et de Seine et Marne. L'hypothèse était que les patients ne bénéficiaient pas d'une échographie ou d'un avis cardiologique et que les traitements prescrits étaient décalés par rapport aux recommandations européennes (IEC, bêtabloquants et spironolactone en particulier). Les résultats ont montré que le suivi cardiologique était satisfaisant. En revanche, les traitements prescrits n'étaient pas en adéquation avec les recommandations en particulier pour la trithérapie IEC, diurétique et bêtabloquants. Il y a un réel manque d'information ou un déficit de pratique des médecins généralistes et des cardiologues qu'il faudrait compenser par un programme de formation continue adapté

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
