Book excerpt: Compared to telehealth delivery of interventions for physical health issues, the implementation of evidence-based psychosocial interventions using technology has less support. Video-conference delivery (VCD) has the potential to increase accessibility to effective treatments, although its use remains limited and understudied. This study employed a mixed methods approach in surveying mental health practitioners about their attitudes regarding the use of video-conference methods to deliver evidence-based interventions. One hundred and eleven practitioners were sampled from several national and regional practice organizations and administered quantitative surveys about their use of and attitudes towards VCD of evidence-based interventions. The relationship between clinician-level technology access, experience, and training with technology fluency and acceptability of using VCD was examined. A subset (n = 20) of respondents were then selected for qualitative interviews to further investigate accessibility, appropriateness, and feasibility of delivering evidence-based interventions via video conference. The results of this study have important implications for telemental health dissemination efforts which seek to extend services to underserved populations.

Book excerpt: We live in an age where technological innovation provides new opportunities for increasing the public's access to quality mental health care. Video teleconferencing (VTC) is being used as a reliable and effective means of providing mental health services to people in rural and underserved areas. The current literature and available training protocols for VTC psychotherapy often focus on technical issues rather than clinical considerations. The aim of this qualitative study was to gain a better understanding of the major concerns, limitations, and barriers to the effective treatment of patients with this technology, and to highlight possible solutions to these barriers that may assist providers in clinical decision-making when working with patients through VTC. Participants were eight clinical psychologists, recruited from within the Veterans Health Administration (VA) system, who have worked extensively with VTC as a means of psychotherapy delivery. Grounded theory methodology was used to analyze the data collected in individual semi-structured interviews. Six emergent constructs were identified from the data: I) How clinicians experience VTC, II) What clinicians need in order to start using VTC, III) Establishing a therapeutic alliance through VTC, IV) Considerations for working with different patient populations, V) Adapting interventions to fit the modality, and VI) Preparing for and handling emergencies through VTC. Findings indicate that participants have had a very positive experience with VTC and view this technology as a valuable tool for reaching patients in rural communities. Participants discuss some of the unique advantages of this technology while also noting many barriers and difficulties that must be surmounted if clinicians are to successfully implement treatments through VTC. Results provide preliminary support for the idea that a meaningful and sufficient therapeutic alliance can be attained through this technology and further indicate that appropriate modifications may allow clinicians to successfully treat most clinical populations with most psychotherapeutic interventions. Until best practice guidelines are available, the results of this study may help prepare clinicians for using this technology and assist in clinical decision-making when delivering psychotherapy through VTC.
