Book excerpt: But de l’étude : Analyser les facteurs de risque cardiovasculaire et les mesures hygiéno-diététiques, chez des patients coronariens, à distance d’une réadaptation cardiaque ; tout en réaffirmant le message de prévention et en proposant des solutions aux patients. Méthodes : Nous avons réalisé une étude descriptive, comparative avant-après, menée de juin à décembre 2006 auprès de 48 patients d’âge inférieur ou égal à 75 ans, ayant eu un syndrome coronarien aigu ou des pontages coronariens. Le recueil des données de la réadaptation était rétrospectif ; celui des données 6 mois après était prospectif, réalisé au cours d’une matinée en hôpital de jour. Résultats : 6 mois après la fin de la réadaptation, les prévalences de l’hypertension artérielle et de la surcharge pondérale augmentaient significativement ; 12,5% des patients avaient un tabagisme actif (stable depuis la réadaptation), 23% avaient un taux de LDL-cholestérol supérieur ou égal à 1g/l (versus 35% lors de la réadaptation), 33% avaient un niveau de stress élevé. Les patients déclaraient faire davantage d’activités physiques, et les recommandations diététiques étaient intégrées pour 69% d’entre eux. Il n’y avait pas de changement significatif de traitement entre la réadaptation et 6 mois après. Conclusion : Les objectifs de contrôle des facteurs de risque et d’adoption des mesures hygiéno-diététiques sont partiellement atteints 6 mois après une réadaptation cardiaque. La prise en charge proposée au regard des résultats obtenus s’inscrit dans une démarche de prévention par une approche multidisciplinaire adaptée à chaque patient. Ses effets sur le changement de comportement au long cours restent toutefois à démontrer.

C'est l'observance à long terme des mesures de prévention qui va déterminer en partie l'évolution de la maladie coronarienne. Mais les facteurs de risque cardiovasculaire chez ces patients ne sont souvent que partiellement contrôlés. Objectif. Evaluer l'intérêt du coaching téléphonique dans la prévention secondaire chez les patients coronariens à 1 an d'une réadaptation cardiaque, par l'analyse du risque de décès à 10 ans (SCORE) puis par l'analyse de la qualité de vie, les facteurs de risque cardiovasculaires et les mesures hygiénodiététiques des patients. Méthodes. Etude des pratiques professionnelles, descriptive, comparative entre 2 groupes de patients coronariens, conduite de manière de novembre 2016 à avril 2017. Un groupe a été suivi par téléphone par une IDE d'éducation thérapeutique à 1, 3 et 6 mois après leur séjour en réadaptation cardiaque et un autre groupe n'a pas eu de suivi après leur séjour. Les patients étaient interrogés 1 an après leur séjour hospitalier par un questionnaire téléphonique. Résultats. Aucune différence sur le SCORE n'a été mise en évidence à 1 an entre les 2 groupes. Le LDLc a été significativement mieux contrôlé chez les patients du groupe suivis par une IDE (0,7g/l vs 0,98g/l, p=0,04). Pour les autres critères, il n'y a pas eu de différence significative entre les 2 groupes. Conclusion. Au total, après un infarctus du myocarde, le bénéfice sur le risque de décès à 10 ans d'un coaching téléphonique par une IDE d'éducation n'a pas été démontré. Ce suivi renforcé améliore le taux de LDLc et aurait tendance à améliorer l'observance des patients.

En prévention secondaire, on s'attache à diminuer le nombre de facteurs de risque, lorsque ceux-ci sont modifiables : on relève notamment l'hypertension artérielle, le diabète, les dyslipidémies, le tabagisme, qui sont dits majeurs, et les autres facteurs de risque, tels que la sédentarité, le surpoids et l'obésité, et les facteurs liés à l'alimentation. Le but de notre étude a été d'observer l'impact d'une prise en charge globale des facteurs de risque cardiovasculaire modifiables, non seulement par une thérapeutique médicamenteuse adaptée, mais aussi par l'enseignement dispensé aux patients sur la pathologie et la nécessité de modifier leur mode de vie. Notre étude prospective a été réalisée sur une cohorte de 176 patients consécutifs, suivis pendant six mois au décours d'un événement coronaire, en réadaptation cardiaque dans le service du Dr Perennec de l'hôpital Albert Chenevier à Créteil. Les résultats ont été interprétés en fonction des modifications cliniques ou biologiques apportées aux facteurs de risque cardiovasculaire, selon les normes déterminées par la Haute Autorité de Santé. A l'issue des six mois, on observe une diminution globale du risque cardiovasculaire, notamment en ce qui concerne les facteurs de risque dits secondaires, liés de façon plus importante aux habitudes et aux modes de vie des patients. L'approche globale du risque cardiovasculaire, associant des thérapeutiques médicamenteuses, à l'enseignement thérapeutique des patients, permet d'élargir les champs de prise en charge des facteurs de risque cardiovasculaire en prévention secondaire. Cette prise en charge globale nécessite l'intervention de personnels médicaux et paramédicaux, dans le cadre d'un suivi rapproché et maintenu au long cours. Le médecin généraliste devrait jouer un rôle central dans cette prise en charge multidisciplinaire.
