Book excerpt: L'endodontie, discipline de l'odontologie, qui concerne la prévention, le diagnostic et le traitement des maladies de la pulpe et du péri-apex doit être maîtrisée par chaque omnipraticien afin de conserver une dent sur arcade le plus longtemps possible. Certaines complications bactériennes et instrumentales peuvent nécessiter l'utilisation de matériel supplémentaire afin de ne pas compromettre l'avenir de la dent. Suite à l'essor des guides chirurgicaux en implantologie permettant d'améliorer la précision lors de la mise en place d'implants, certains chercheurs se sont basés sur la fiabilité de cette technique pour concevoir des guides à indication endodontique. Ces guides ont pour but de prévenir les complications par une étude préalable à l'aide d'une imagerie tridimensionnelle (CBCT) fusionnée à une empreinte optique, permettant de réaliser une planification précise du geste endodontique. Ils permettraient ainsi de traiter les dents présentant des canaux calcifiés, des dents antérieures et postérieures présentant des anatomies complexes (dents évaginées, invaginées), ou d'améliorer la précision des fenêtres osseuses lors de chirurgie endodontique. Le traitement endodontique permettrait de réduire le temps au fauteuil, l'atteinte tissulaire, et nécessiterait ainsi une expérience moindre par un protocole standardisé et reproductible. Ces guides présentent cependant certaines limites : difficulté en cas de canaux courbés ou de limitation d'ouverture buccale, irradiation et cout de traitement augmentés et expérience de planification nécessaire... Malgré les études récentes et encore trop peu représentatives concernant ces guides, ils semblent être une technique prometteuse nous présentant des résultats fiables et reproductibles, que nous devrons prochainement ajouter à notre arsenal thérapeutique.
